Outcomes of silicone arthroplasty for rheumatoid metacarpophalangeal joints stratified by fingers.

PURPOSE: Previous studies have demonstrated that outcomes for the ulnar digits appear to be worse than those of the radial digits after silicone metacarpophalangeal joint arthroplasty (SMPA) for the rheumatoid hand. This study examines various components of hand deformities in an effort to understand SMPA outcomes in terms of metacarpophalangeal (MCP) joint range of motion and alignment. We hypothesize that the ulnar fingers will have less improvement, marked by greater ulnar drift, extension lag, and less MCP joint arc of motion than the radial fingers. METHODS: Sixty-eight surgical patients

Epiphyseal fractures of the finger middle joints in young sport climbers.

OBJECTIVE: As competition climbing becomes increasingly popular, younger climbers are entering the sport, and some are among the top-level athletes. This early start combined with intensive training methods can lead to radiographic changes in the fingers and even osteoarthrosis. Since 1994, we have been observing an increasing number of nontraumatic epiphyseal fractures in young athletes. METHODS: Twenty-four cases of young climbers with nontraumatic epiphyseal fractures of the finger middle joints are presented. RESULTS: The average age of the climbers was 14.5 (+/- 0.9) years; 23 were boys

A literature review of "failures" of the Swanson finger prosthesis in the metacarpophalangeal joint.

This paper reviews the use of the Swanson finger prosthesis, concentrating on clinical results from the metacarpophalangeal (MCP) joint and modes of prosthetic "failure". While "failure" is generally associated with fracture, it is recognised that fracture does not always necessitate replacement of the Swanson prosthesis. Fracture tends to occur at the junction of the distal stem and hinge of the prosthesis. Initial improvements in ulnar deviation and range of motion (ROM) tend to be gradually reduced over the duration of implantation, and there is little evidence to suggest any long-term impr

Numerical scoring systems for the anatomic evolution of osteoarthritis of the finger joints.

OBJECTIVE: To assess and score the progression of osteoarthritis (OA) of the distal and proximal interphalangeal (DIP and PIP) and metacarpophalangeal (MCP) joints. METHODS: Forty-six patients with OA of the finger joints were followed up for 3 years; 36 of them were followed up for 5 years. Posteroanterior radiographs of the hands were obtained at the start of this prospective study and at yearly intervals. The scoring systems used were based on the increased incidence of OA during consecutive years in previously normal joints, the radiologic progression of anatomic lesions in the affected f…

An isolated complex dorsal dislocation of the MP joint of the ring finger.

A rare case of isolated complex dorsal dislocation of the MP joint of the ring finger is described. Differences in the pathological findings between dislocations of the ring finger and other digits are discussed.

Boxer's knuckle--dorsal capsular rupture of the metacarpophalangeal joint of a finger.

An injury to the dorsal aspect of the metacarpophalangeal joint of a finger usually damages the sagittal fibers of the extensor tendon mechanism. Rupture of the underlying joint capsule, a far more serious injury, has received scant attention in the medical literature. The problem is often unrecognized because the extensor tendon may remain in its normal midline position. Eight dorsal capsular ruptures are described in six patients who had closed direct trauma to either the knuckle of their index or long fingers. Most of the injuries occurred in professional boxers.

Irreducible compound dorsal dislocations of the proximal interphalangeal joint of the finger.

Three cases of compound irreducible dorsal dislocation of the proximal interphalangeal joint of the finger are reported. The probable mechanism is discussed. The findings warrant the description "volar capsular boutonnière" as the condyles of the proximal phalanx buttonhole, through the volar structures. Open reduction combined with debridement, is the treatment for such compound irreducible dorsal dislocations. Dislocation of the proximal interphalangeal joints of the fingers are common and can usually be reduced by simple traction. Occasionally reduction by closed methods is unsuccessful bec

Simultaneous closed dislocation of both interphalangeal joints in one finger.

A rare case of simultaneous dislocation of both interphalangeal joints in one finger in a table-tennis player is presented. The second dislocation took place when the first dislocated joint became the fixed part of the finger as it hit a wall. Treatment was, first, hyperextension to unlock the base of the phalanx, then traction along the phalanx: its base was then pushed into contact with the head of the proximal phalanx. Splinting was applied with the joint in slight flexion.

[Habitual luxation of the extensor apparatus in regard to the metacarpophalangeal joint of the fingers].

The sagittal bands of the metacarpophalangeal joint play an important role in the control of the mobility and stability of the extensor aponeurosis of the joint. A case of dislocation of the extensor tendon from rupture of the radial sagittal fibers is reported. A method of repair of the tendon is discussed. The result at one year is excellent.

[Arthrolysis of the metacarpophalangeal joints in post-traumatic stiffness of fingers in extension].

In cases of post-traumatic stiffness of the metacarpo-phalangeal joints in extension, it is possible to safely perform an arthrolysis with or without tenolysis while preserving the continuity of the extensor mechanism. This paper describes the surgical approach and technique of arthrolysis of the metacarpo-phalangeal joints. Proper operative management of post-traumatic stiffness of the long fingers in extension can improve the results in these cases.

The adipofascial turnover flap for coverage of the exposed distal interphalangeal joint of the fingers and interphalangeal joint of the thumb.

The adipofascial turnover flap was used to cover exposed distal interphalangeal joints of 8 fingers and interphalangeal joints of 4 thumbs. The flap was raised from the dorsum of the injured digit with a base located at the edge of the defect. The flap has a random type of vascularity and was designed with a base-to-length ratio of 1:1.5 to 1:2. All flaps survived completely. Various other types of flaps that have been used to reconstruct dorsal digital skin defects were reviewed and compared with the adipofascial turnover flap.

[Arthrodesis of the Distal Interphalangeal Joint of the Finger Using an Intramedullary Implant].

PURPOSE OF THE STUDY: Osteoarthritis (OA) of the joints of the hand is one of the most common musculoskeletal disorders in the elderly population. It is a complex, degenerative disease affecting all components of the joint. Surgical treatment is indicated when conservative therapy fails. The ultimate solution is arthrodesis of the distal interphalangeal (DIP) joint. Various methods of arthrodesis have been described, ranging from the use of K-wires or compression screw to different types of intra-articular (intramedullary) implants. The aim of this study is to evaluate the surgical outcomes of

Ligament preserving technique for a locked metacarpophalangeal joint of the index finger.

BACKGROUND: Locking of metacarpophalangeal joint (MPJ) of the index finger occurs when volar radial osteophytes of the metacarpal head catch the accessory collateral ligament. We devised a ligament-preserving approach to quickly restore the MPJ motion while protecting the radial collateral ligament. METHODS: We retrospectively reviewed the results of nine patients treated for a locked MPJ of the index finger. In three patients, closed reduction was successful. In six cases, volar radial osteophytes were excised from the metacarpal head using a ligament-preserving technique through a longitudi

Degenerative and post-traumatic arthritis affecting the carpometacarpal joints of the fingers.

Symptomatic post-traumatic arthritis affecting the finger CMC joints is less common than might otherwise be expected based on the frequency of injury, especially to the fifth CMC joint. For the fifth CMC joint, the shallow concavoconvex articulation combined with the typical fracture location may provide a protective effect. Non-operative measures are typically successful, except in cases of missed fracture/dislocations and symptomatic joint instability. In these instances, re-construction emphasizes stability first, with an eye toward mobility for the ulnar column. It is common to detect a bo
