Alcohol Misuse and Correlates with Mental Health Indicators among Firefighters.

Background: Stress and repetitive exposures to potentially traumatizing events are inherent to first-responder professions. Firefighters and other crisis responders are often exposed to incidents in their work environments and are at risk of developing acute or posttraumatic symptomatology, depression, and anxiety, as well as harmful patterns of alcohol consumption. Most prior research and theory suggests that many firefighters use alcohol to cope with past and current stressors, and that increased drinking accompanies ongoing exposures to stressful occupational hazards. Objectives: The primar

Etiology of Exercise Injuries in Firefighters: A Healthcare Practitioners' Perspective.

The purpose of this study was to query healthcare practitioners (HCPs) who treat firefighter injuries to identify risk factors and mechanisms associated with musculoskeletal injuries during exercise. A phenomenological design was utilized to understand the experiences of HCPs while treating firefighters' musculoskeletal injuries due to exercise. Semi-structured interviews were conducted with 14 HCPs. Two interviews were pilot-tested with HCPs to ensure reliability and validity. Interviews were transcribed and uploaded to a qualitative analysis software program. Although the study inquired abou

Behavioral and Occupational Health in Military Firefighters: An Understudied Population.

To our knowledge, no studies on health conditions in U.S. military firefighters exist. Data and demographics from the Defense Medical Epidemiology Database were analyzed on several shared medical issues among military personnel and civilian firefighters. Descriptive statistics and Chi-Square goodness of fit tests were conducted to support study aims. Between 2001 and 2015, substantial incidence rate increases (per 10,000) of tinnitus, PTSD, insomnia, and OSA (2005-2015) were observed. Modest to large increases in depressive disorders, adjustment reaction, generalized anxiety disorder, and pani

Blood pressure in firefighters, police officers, and other emergency responders.

Elevated blood pressure is a major risk factor for cardiovascular morbidity and mortality. Increased risk begins in the prehypertensive range and increases further with higher pressures. The strenuous duties of emergency responders (firefighters, police officers, and emergency medical services (EMS) personnel) can interact with their personal risk profiles, including elevated blood pressure, to precipitate acute cardiovascular events. Approximately three-quarters of emergency responders have prehypertension or hypertension, a proportion which is expected to increase, based on the obesity epide

Reasons for rejection: why people fail to qualify as firefighters.

In the decade beginning 1 January 1985, 916 individuals (including five females) were medically examined with a view to joining the full-time service of Strathclyde Fire Brigade (SFB). One hundred and nine (11.9%), including two females, were rejected. The five main causes of failure were: ocular (n = 46, 42.2%); lack of stamina (n = 21, 19.2%); 'others' (n = 12, 11.0%); cardiovascular (n = 9, 8.3%) and orthopaedic (n = 6, 5.5%). Thirty-two had chest X-rays. One abnormality was found -an azygous lobe-but it played no part in the decision to decline the applicant. There was little life- or heal

Assessment of cancer biomarkers in the Grenfell firefighter cohort study.

Firefighters are exposed to a diverse range of harmful substances, including polycyclic aromatic hydrocarbons, benzene, and other carcinogens. These toxic compounds induce DNA damage, often causing the formation of DNA adducts and other lesions that can contribute to the development of various diseases, including cancer. Recent advancements in molecular diagnostics have shown that circulating cell-free DNA (cfDNA) in plasma is a valuable biomarker for detecting DNA damage and disease states. In this study, we explored whether changes in the quantity and quality of plasma cfDNA might reveal DNA

The risk of insomnia by work schedule instability in Korean firefighters.

BACKGROUND: Firefighters are exposed to shift work, as well as unpredictable emergency calls and traumatic events, which can lead to sleep problems. This study aimed to investigate the risk of insomnia by work schedule instability in Korean firefighters. METHODS: This study used the Insomnia Severity Index to assess the insomnia in firefighters. The work schedule stability was classified with the frequency of the substitute work and the timing of notification for work schedule changes. Logistic regression analysis was used to assess the adjusted odds ratio of insomnia by work schedule stabili

Diffuse Pulmonary Ossification (DPO) in the Setting of Firefighter Occupation.

A 76-year-old man with a past occupational history as a firefighter and construction worker presented at an urgent care center with signs and symptoms of chronic dry cough, exertional dyspnea, and fatigue. His initial chest X-ray showed interstitial thickening in the middle and lower lobes with pulmonary infiltrates bilaterally. The patient was treated with an outpatient course of antibiotics. A subsequent high-resolution computed tomography (HRCT) scan of the patient's lungs revealed extensive peripheral, bibasilar linear reticulations, interstitial thickening, and subpleural cystic changes,

Dynamic Psychotherapy as a PTSD Treatment for Firefighters: A Case Study.

In Portugal, forest fires are responsible for disasters that tend to be repeated annually, leading to dramatic consequences, such as those that have occurred in 2017, with the destruction of hundreds of houses and the deaths of dozens of people. Firefighters who are exposed to these potentially traumatic events are considered a high-risk group for the development of stress-related disorders. The aim of this study was to monitor the progress of two firefighters with symptoms of post-traumatic stress disorder (PTSD) treated through dynamic psychotherapy (DP) and to assess the feasibility of impl

Firefighting and melanoma, epidemiological and toxicological associations: a case report.

The International Agency for Research on Cancer (IARC) classifies firefighting as possibly carcinogenic to humans, and polychlorinated biphenyls (PCBs) as carcinogens with sufficient evidence for development of melanoma in humans. We present a case report of a firefighter with melanoma and history of 33 years of occupational exposure. Based on the available epidemiological and toxicological evidence of association between being a firefighter and developing melanoma, melanoma was recognized by the Norwegian Labour and Welfare Service as an occupational disease in our patient. In 2017, melanoma

The evaluation of CBRN canisters for use by firefighters during overhaul.

Air-purifying respirators (APRs) have been proposed to provide an additional respiratory protection option for structural firefighters involved in overhaul operations and wildland firefighters, where particulate and aldehyde exposures have been documented. Previous studies (Anthony et al., 2007) developed test methods to evaluate APR cartridges and canisters for use in overhaul activities, where initial findings indicated that multi-gas cartridges may not be effective. This study evaluated the performance of three chemical, biological, radiological, and nuclear (CBRN) canisters (MSA, 3M, and S

Outpatient firefighter burn injuries: a 3-year review.

Previously, our Burn Center at the New-York Presbyterian/Weill Cornell Medical Center reported a decline during a 10-year period in the number of firefighters requiring hospitalization for burn injuries, from 53 patients per year to 15 patients per year. Because the incidence of structural fires continued at a constant rate of 26,240 to 30,841 per year during this time, it was postulated that an improvement in protective gear accounted for the decrease in injuries. However, it also was possible that more firefighters were being treated on an outpatient basis. Therefore, our Burn Center's outpa

Overload of the lower limbs of firefighters as a result of external conditions.

BACKGROUND: It is known that a firefighter's uniform with all equipment will significantly increase the load on the lower limbs during landing. The higher landing height will also increase ground reaction forces. However, it is unknown what effect the firefighters' experience (job seniority) will have on the load. Moreover, age-related changes in neuromuscular control, balance, and muscular strength may alter the way external loads are managed, potentially increasing susceptibility to overload. OBJECTIVE: The present study aimed to evaluate the overload of the musculoskeletal system resulting

Assessing carcinogenic and mutagenic hazards in firefighting: a comprehensive review.

This review aims to critically assess the carcinogenic and mutagenic risks associated with firefighting, with a focus on cancer incidence, cancer mortality and other chronic health outcomes. Firefighting is widely recognized as a high-risk occupation due to repeated exposure to hazardous substances, including polycyclic aromatic hydrocarbons (PAHs) and other combustion by-products. An extensive review of empirical studies, meta-analyses, and institutional reports was conducted, considering not only direct fireground exposure but also risks arising from contaminated personal protective equipmen

PTSD and PTG in French and American Firefighters: A Comparative Study.

Studies show that experiencing traumatic events can lead to positive psychological change, or posttraumatic growth (PTG). In the hope of promoting PTG, authors have been focusing on identifying the factors that may foster PTG. Despite these attempts, the literature shows inconsistencies, making it difficult to know which variables may be involved in the process of growth. Indeed, authors seem to disagree on the nature of the relationship between PTSD and PTG, time since the event, social support, intrusive rumination, and sociodemographics. Thus, this study aims to clarify these discrepancies,

Incidence of cardiovascular disease in a historical cohort of Danish firefighters.

OBJECTIVES: Firefighters are exposed to multiple cardiovascular hazards, but few epidemiological studies have addressed their cardiovascular morbidity. The objective of this study was therefore to examine the incidence of cardiovascular diseases (CVD) in Danish firefighters. METHODS: We used individual historical employment records on 11.691 male Danish firefighters supplied by trade unions and fire agencies. The Supplementary Pension Fund Register was used to establish two occupational reference groups (a random sample from the male employed population and military employees). Information on
