Goal-directed Fluid Therapy Versus Conventional Fluid Therapy During Craniotomy and Clipping of Cerebral Aneurysm: A Prospective Randomized Controlled Trial.

BACKGROUND: Fluid imbalance is common after aneurysmal subarachnoid hemorrhage and negatively impacts clinical outcomes. We compared intraoperative goal-directed fluid therapy (GDFT) using left ventricular outflow tract velocity time integral (LVOT-VTI) measured by transesophageal echocardiography with central venous pressure (CVP)-guided fluid therapy during aneurysm clipping in aneurysmal subarachnoid hemorrhage patients. METHODS: Fifty adults scheduled for urgent craniotomy for aneurysm clipping were randomly allocated to 2 groups: group G (n=25) received GDFT guided by LVOT-VTI and group …

Fluid Therapy: Double-Edged Sword during Critical Care?

Fluid therapy is still the mainstay of acute care in patients with shock or cardiovascular compromise. However, our understanding of the critically ill pathophysiology has evolved significantly in recent years. The revelation of the glycocalyx layer and subsequent research has redefined the basics of fluids behavior in the circulation. Using less invasive hemodynamic monitoring tools enables us to assess the cardiovascular function in a dynamic perspective. This allows pinpointing even distinct changes induced by treatment, by postural changes, or by interorgan interactions in real time and en…

[Parenteral fluid therapy in the pediatric surgical patient].

Perioperative fluid therapy aims to provide water, electrolytes and calorie to maintain metabolic homeostasis. The landmark article in which Holliday and Segar proposed the rate and composition of parenteral maintenance fluids for hospitalized children is used to the fluid management for the pediatric surgical patient. Critically ill patients require meticulous intraoperative fluid management to avoid acidosis, hypoglycemia, hyponatremia, and hypocalcemia as well as to maintain adequate intravascular volume. The glucose, electrolyte, and intravascular volume requirements of the pediatric surgi…

Improving intravenous fluid therapy in children with gastroenteritis.

Gastroenteritis is one of the most common medical conditions seen by pediatricians. The standard approach to intravenous fluid therapy for these children has been to administer a 0.9% sodium chloride (NaCl) bolus followed by a hypotonic solution ranging from 0.2-0.45% NaCl to replace the remaining deficit plus maintenance. We have questioned the safety of this approach as there have been reports of death or permanent neurologic impairment from hyponatremic encephalopathy. Hanna and Saberi (Pediatr Nephrol. doi: 10.1007/s00467-009-1428-y ) found the incidence of hospital-acquired hyponatremia (…

Intravenous solutions for fluid therapy in calf diarrhoea.

Five commercially available parenteral solutions were compared for their effectiveness in correcting the disturbances associated with diarrhoea induced by Escherichia coli. Each solution (saline, Hartmann's, Darrow's, Plasmalyte +/- glucose) was tested on eight Jersey calves less than a week old and weighing approximately 25 kg. Each calf received 8.5 litres over three days, at about 20 ml kg-1 h-1. Solutions such as saline or Plasmalyte which had higher concentrations of sodium were more effective at correcting dehydration and electrolyte disturbances than those with less sodium (Darrow's, Ha

Oral fluid therapy in neonatal ruminants and swine.

Definitive recommendations regarding the best method to treat the diarrheic calf cannot be made at this time. The exact recommendation will probably depend on the situation. At the present time, I do not believe it is detrimental to withdraw milk from the diet for periods up to 24 to 36 hours, as long as the calf is in good condition and is returned to a full ration (maintenance plus) of milk within 3 to 5 days from the start of milk withdrawal. Milk withdrawal is most likely to be beneficial in those situations in which the calf is depressed and has lost its sucking reflex. Oral electrolyte s

Fluid therapy for gastrointestinal, pancreatic, and hepatic disorders.

Patients with gastrointestinal, hepatic, or pancreatic disease frequently require parenteral fluid therapy. The selection of the type and amount of fluid for administration should be based on a knowledge of physiologic derangements and the controlling mechanisms. Table 4 provides suggested replacement fluids and supplements for each disease. It must be remembered, however, that individual "tailoring" is required for each case. In addition to an expert knowledge of pathophysiology, monitoring of the patient's weight, fluid losses, serum electrolyte, and acid-base status is necessary for success…

Injectable fluid silicone therapy. Human morbidity and mortality.

Four patients who had received silicone injections had the following complications: migration, hepatic disease manifested as granulomatous hepatitis (previously undescribed, to our knowledge), hypopigmentation, and death. Silicone should now be considered as a possible cause of hepatic granulomas in an appropriate host.

How I personalize fluid therapy in septic shock?

During septic shock, fluid therapy is aimed at increasing cardiac output and improving tissue oxygenation, but it poses two problems: it has inconsistent and transient efficacy, and it has many well-documented deleterious effects. We suggest that there is a place for its personalization according to the patient characteristics and the clinical situation, at all stages of circulatory failure. Regarding the choice of fluid for volume expansion, isotonic saline induces hyperchloremic acidosis, but only for very large volumes administered. We suggest that balanced solutions should be reserved for …

Fluid therapy in the acute brain injured patient.

Adequate fluid therapy in the acute brain injured (ABI) patient is essential for maintaining an adequate brain and systemic physiology and preventing intra- and extracranial complications. The target of euvolemia, implying avoidance of both hypovolemia and fluid overloading (or "hypervolemia," by definition associated with fluid extravasation leading to tissue edema) is of key importance. Primary brain injury can be aggravated by secondary brain injury and systemic deterioration through diverse pathways which can challenge appropriate fluid management, e.g. neuroendocrine and electrolyte disor…

Prolonged Field Care Working Group Fluid Therapy Recommendations.

The Prolonged Field Care Working Group concurs that fresh whole blood (FWB) is the fluid of choice for patients in hemorrhagic shock, and the capability to transfuse FWB should be a basic skill set for Special Operations Forces (SOF) Medics. Prolonged field care (PFC) must also address resuscitative and maintenance fluid requirements in nonhemorrhagic conditions.

Controversies in fluid therapy: Type, dose and toxicity.

Fluid therapy is perhaps the most common intervention received by acutely ill hospitalized patients; however, a number of critical questions on the efficacy and safety of the type and dose remain. In this review, recent insights derived from randomized trials in terms of fluid type, dose and toxicity are discussed. We contend that the prescription of fluid therapy is context-specific and that any fluid can be harmful if administered inappropriately. When contrasting ''crystalloid vs colloid'', differences in efficacy are modest but differences in safety are significant. Differences in chloride…

Avoiding common problems associated with intravenous fluid therapy.

Inappropriate intravenous fluid therapy is a significant cause of patient morbidity and mortality and may result from either incorrect volume (too much or too little) or incorrect type of fluid. Fluid overload has no precise definition, but complications usually arise in the context of pre-existing cardiorespiratory disease and severe acute illness. Insufficient fluid administration is readily identified by signs and symptoms of inadequate circulation and decreased organ perfusion. Administration of the wrong type of fluid results in derangement of serum sodium concentration, which, if severe …

Fluid therapy for children: facts, fashions and questions.

Fluid therapy restores circulation by expanding extracellular fluid. However, a dispute has arisen regarding the nature of intravenous therapy for acutely ill children following the development of acute hyponatraemia from overuse of hypotonic saline. The foundation on which correct maintenance fluid therapy is built is examined and the difference between maintenance fluid therapy and restoration or replenishment fluid therapy for reduction in extracellular fluid volume is delineated. Changing practices and the basic physiology of extracellular fluid are discussed. Some propose changing the def…

Fluid and electrolyte therapy and chronic lung disease.

PURPOSE OF REVIEW: To evaluate the role of fluid and electrolyte therapy in the pathogenesis of chronic lung disease. RECENT FINDINGS: There have been no new studies since 2000, and there are minimal data addressing this issue specifically in infants at highest risk of chronic lung disease (ie, extremely low birth weight infants). Most observational studies demonstrate a significant association between increased fluid intake or differences in measures of changes in total body water balance in the first week of life and chronic lung disease. However, a metaanalysis of three randomized, control
