Paroxysmal "nightmares". Sequel of a stroke responsive to diphenylhydantoin.

A 65-year-old man had nightmares a few weeks after a right temporal lobe infarction. Electroencephalography showed no epileptic activity. Therapy with diphenylhydantoin produced complete remission of his symptoms. On the bases of their acute onset, their association with sleep, their occasional occurrence while the patient was awake, the lack of effect of diazepam and flurazepam, and the good response to diphenylhydantoin, we propose that these episodes were partial seizures secondary to the right temporal lobe lesion.

Neuropharmacologic specificity of a simple animal model for the behavioral actions of benzodiazepines.

A simple model system for the behavioral actions of benzodiazepines is analyzed for its dose-response predictiveness, using several benzodiazepines, and for its pharmacological specificity, using other non-anxiolytic classes of psychoactive drugs. The model demonstrates an increase in mouse exploratory activity between a brighty-lit open field and a small, dark compartment by clonazepam, diazepam, flurazepam, chlordiazepoxide and meprobamate, but not by the peripheral benzodiazepine R05-4864, nor by clorgyline, butriptyline, or chlorpromazine.

Withdrawal after substitution of a short-acting for a long-acting benzodiazepine.

A withdrawal syndrome occurred in two patients after substitution of a short-acting benzodiazepine for a long-acting benzodiazepine. Both patients had used long-acting benzodiazepines on a daily basis for many years. In one case, oxazepam was substituted for diazepam, and in the other, temazepam was substituted for flurazepam hydrochloride. In both cases the short-acting benzodiazepine was substituted in a once-daily dosage. Withdrawal symptoms followed and persisted for at least one month. Relative advantages and disadvantages of short-acting and long-acting benzodiazepines are discussed.

gamma-aminobutyric acidA receptor regulation: heterologous uncoupling of modulatory site interactions induced by chronic steroid, barbiturate, benzodiazepine, or GABA treatment in culture.

Prolonged administration of anxiolytic, sedative, and anticonvulsant drugs that act through the GABAA receptor (GABAAR) can evoke tolerance and dependence, suggesting the existence of an endogenous mechanism(s) for altering the ability of such agents to interact with the GABAAR. Uncoupling appears to be one such mechanism. This is a decrease in the allosteric interactions between the benzodiazepine (BZD) recognition site and other agonist or modulator sites on the GABAAR, as measured by potentiation of [3H]flunitrazepam ([3H]FNZ) binding. To investigate the mechanism(s) of uncoupling, neuronal

Effects of chronic treatment with benzodiazepine receptor ligands on cortical adrenoceptors.

We report the effects of kindling with the benzodiazepine partial inverse agonist, FG 7142 on adrenoceptor binding in mouse cerebral cortex. Twelve once-daily injections of FG 7142 caused a statistically significant increase in the density of both alpha 2- and beta-adrenoceptor binding sites, seven days after the last injection. Despite the marked effects of FG 7142 on adrenoceptors, there were no changes in either alpha 2- or beta-adrenoceptor binding after prolonged treatment with the benzodiazepine, flurazepam.

Investigating benzodiazepine receptor function in vivo using an intravenous infusion of DMCM.

A method for measuring seizure thresholds using an intravenous infusion of a convulsant beta-carboline benzodiazepine receptor ligand (DMCM) is reported. Seizure thresholds to DMCM are elevated by the benzodiazepine receptor agonist, flurazepam and antagonist, Ro 15-1788, and the proconvulsant beta-carboline, FG 7142. Various other anticonvulsant also antagonise DMCM seizures. Selectivity for the benzodiazepine/GABA receptor complex is demonstrated since bicuculline and pentylenetetrazol lowered thresholds whereas strychnine and N-methyl DL-aspartate did not.

Isobolographic analysis of the interactions between midazolam and propofol at GABA(A) receptors in embryonic mouse neurons.

BACKGROUND: Clinical studies suggest that midazolam and propofol interact synergistically to induce hypnosis, but these drugs do not interact synergistically to prevent movement in response to noxious stimuli. The mechanisms underlying these interactions are not certain but may occur at the level of the gamma-aminobutyric acid A (GABA(A)) receptor. METHODS: The authors evaluated the interactions between propofol and midazolam in modulating GABA(A) receptor activity in embryonic hippocampal neurons. The effects of midazolam and propofol on peak current evoked by submaximal concentrations of GA

The action of gamma-aminobutyric acid (GABA) and ethylenediamine (EDA) on Limulus and Helix central neurones and rat cerebellar and sympathetic ganglion neurones.

Intracellular recordings were made from central Limulus and Helix neurones and extracellular recordings from rat cerebellar Purkinje cells and sympathetic ganglia. The actions of gamma-aminobutyric acid (GABA) and ethylenediamine (EDA) and related analogues on these preparations were investigated. On Limulus neurones inhibited by GABA, EDA and piperazine were 81 and 186 times respectively less potent than GABA. Both the GABA and EDA events were chloride mediated, having similar reversal potentials and were reversibly antagonised by picrotoxinin. The EDA response persisted in high magnesium Rin

Triazolam--an "abused drug" by the lay press?

For the second time in the past ten years adverse reports in the lay press have questioned the safety of triazolam. Anecdotal reports of central nervous system adverse reactions can be separated into four general categories: (1) delirium in psychiatric patients; (2) delirium in geriatric patients; (3) withdrawal reactions; and (4) anterograde amnesia. Of the four, the amnesia reactions are regarded as serious enough by the Food and Drug Administration that it is requiring hypnotic drug manufacturers to revise the labeling of temazepam and flurazepam along with triazolam to emphasize the potent

Drug-related admissions to an inpatient psychiatric unit.

The authors prospectively monitored 60 consecutive admissions to an inpatient psychiatry service to determine the role of drugs in psychiatric hospitalization. They categorized drug-related problems as 1) drug intoxication, 2) drug abuse, 3) side effects of therapeutic drugs, and 4) noncompliance with prescribed medications. During the 6-month study 25 (41.7%) of the admissions were related to drug use; in 16 (26.7%) admissions a drug-related problem was identified as the primary cause of hospitalization. Alcohol was involved in 8 admissions, and Fiorinal, cocaine, and flurazepam were each rel

Learned taste aversions induced by hypnotic drugs.

Taste aversions reflect the association of drug-produced noxious interoceptive stimuli with distinctive tastes. In the present experiments, taste aversions to sweetened condensed milk were induced in rats by a wide range of doses of the hypnotic drugs, hexobarbital, amobarbital, chloral hydrate, flurazepam and methaqualone and by anesthetization with ethyl ether. Observation of changes in motor behavior following drug administration could not be used to predict the development of taste aversions. Despite the fact that hypnotic drugs have punishing properties, which these paradox is discussed i

Allosteric modulators induce distinct movements at the GABA-binding site interface of the GABA-A receptor.

Benzodiazepines (BZDs) and barbiturates exert their CNS actions by binding to GABA-A receptors (GABARs). The structural mechanisms by which these drugs allosterically modulate GABAR function, to either enhance or inhibit GABA-gated current, are poorly understood. Here, we used the substituted cysteine accessibility method to examine and compare structural movements in the GABA-binding site interface triggered by a BZD positive (flurazepam), zero (flumazenil) and negative (3-carbomethoxy-4-ethyl-6,7-dimethoxy-β-carboline, DMCM) modulator as well as the barbiturate pentobarbital. Individually monitoring ligand-induced changes in the structure of the GABAA receptor at benzodiazepine binding site and non-binding-site interfaces.

The mechanisms by which the GABA and benzodiazepine (BZD) binding sites of the GABA-A receptor are allosterically coupled remain elusive. In this study, we separately monitored ligand-induced structural changes in the BZD binding site (alpha/gamma interface) and at aligned positions in the alpha/beta interface. alpha(1)His101 and surrounding residues were individually mutated to cysteine and expressed with wild-type beta2 and gamma2 subunits in Xenopus laevis oocytes. The accessibilities of introduced cysteines to modification by methanethiosulfonate ethylammonium (MTSEA)-Biotin were measured

Agonist-, antagonist-, and benzodiazepine-induced structural changes in the alpha1 Met113-Leu132 region of the GABAA receptor.

The structural basis by which agonists, antagonists, and allosteric modulators exert their distinct actions on ligand-gated ion channels is poorly understood. We used the substituted cysteine accessibility method to probe the structure of the GABAA receptor in the presence of ligands that elicit different pharmacological effects. Residues in the alpha1 Met113-Leu132 region of the GABA binding site were individually mutated to cysteine and expressed with wild-type beta2 and gamma2 subunits in Xenopus laevis oocytes. Using electrophysiology, we determined the rates of reaction of N-biotinaminoet

Zopiclone. A review of its pharmacological properties and therapeutic efficacy as an hypnotic.

Zopiclone is a cyclopyrrolone which is chemically unrelated to the benzodiazepines and is thought to act on the GABAA receptor complex at a site distinct from, but closely related to, the benzodiazepine binding site. The hypnotic efficacy of zopiclone administered as single oral doses has been demonstrated in patients undergoing next-day surgery and in patients with insomnia, and these studies have established an optimal dose of 7.5mg for elderly patients. Using this dose, clinical studies have shown that zopiclone improved sleep in elderly patients to a similar extent as triazolam 0.125 to 0.

Rebound insomnia: a critical review.

Rebound insomnia, a worsening of sleep compared with pretreatment levels, has been reported upon discontinuation of short half-life benzodiazepine hypnotics. This paper reviews the existing sleep laboratory studies for the presence or absence of rebound insomnia following treatment with triazolam, temazepam, and flurazepam in insomniac patients or poor sleepers and, when possible, in normals. The results indicate that rebound insomnia is a distinct possibility after discontinuation of triazolam in both insomniacs and normal controls. Compared with baseline, disturbed sleep was reported in inso

[Acute lipoid pneumonia caused by accidental aspiration of vaseline used in nasogastric intubation].

Exogenous lipoid pneumonia is a rare disease which is often underdiagnosed. Exogenous lipoid pneumonia has traditionally been considered a chronic process secondary to continued aspiration or inhalation of fatty substances. In its acute form exogenous lipoid pneumonia sometimes coincides with accidental massive aspiration of lipidic material, as has been traditionally described for fire eaters. We review the literature and report a case of acute lipoid pneumonia secondary to accidental aspiration of vaseline used to place a nasogastric tube, which had been inserted for gastric lavage after a s

Structural mechanisms underlying benzodiazepine modulation of the GABA(A) receptor.

Many clinically important drugs target ligand-gated ion channels; however, the mechanisms by which these drugs modulate channel function remain elusive. Benzodiazepines (BZDs), anesthetics, and barbiturates exert their CNS actions by binding to GABA(A) receptors and modulating their function. The structural mechanisms by which BZD binding is transduced to potentiation or inhibition of GABA-induced current (I(GABA)) are essentially unknown. Here, we explored the role of the gamma(2)Q182-R197 region (Loop F/9) in the modulation of I(GABA) by positive (flurazepam, zolpidem) and negative [3-carbom

Diazepam receptors on mouse astrocytes in primary cultures: displacement by pharmacologically active concentrations of benzodiazepines or barbiturates.

Astrocytes in primary cultures were found to bind large amounts of [3H]diazepam. More than 90% of the total binding is specific, i.e., displaceable by an excess of nonlabeled diazepam. At saturation the specific binding amounts to about 20 pmol/mg protein and the Kd value is 30 nM. Clonazepam and flurazepam displace [3H]diazepam from its specific binding sites; 20-30% displacement is brought about by 0.1-0.3 microM clonazepam, which is similar to the plasma concentrations encountered in epileptic patients treated with this drug. Barbiturates have a similar effect, but considerably higher conce
