Foramen magnum size and involvement of its intraoccipital synchondroses in Crouzon syndrome.

BACKGROUND: Cranial sutures and synchondroses tend to close prematurely in patients with Crouzon syndrome. This influences their skull vault and skull base development and may involve in common disturbances such as increased intracranial pressure and cerebellar tonsillar herniation. The authors' hypothesis was that Crouzon patients patients have a smaller foramen magnum than controls because of premature fusion of the intraoccipital synchondroses, putting them at risk for cerebellar tonsillar herniation. Therefore, foramen magnum size and time of intraoccipital synchondroses closure were evalu…

[Case report: Rhizomelic chondrodysplasia punctata and foramen magnum stenosis in a newborn].

Chondrodysplasia punctata is a peroxisomal disorder which is a form of multiple epiphyseal dysplasia. It is characterized by calcifications of unossified cartilaginous epiphyseal centers during the first year of life. Severe autosomal recessive rhisomelic form shows bilateral proximal shortening of the upper and lower limbs with punctate epiphyseal calcifications. We report radiological findings of a patient with rhisomelic chondrodysplasia punctata. Magnetic resonance imaging showed foramen magnum stenosis that caused spinal cord compression.

Papillary (malignant) meningioma of the foramen magnum presenting as a posterior neck mass.

Meningiomas and neurofibromas are the most common extramedullary tumors in the region of the foramen magnum. The clinical symptoms are variable, including headache, occipitocervical pain, and hoarseness. Neurological symptoms are present only in 40% of cases. The age group ranges from 4 to 56 years. A number of patients are misdiagnosed as multiple sclerosis, syringomyelia, and cervical disc disease. The overall incidence of these tumors vary from 1.6 to 2.4%. The majority of meningiomas are histologically benign. Papillary (malignant) meningiomas are even rarer in this location. The present r…

Computerized tomography of the foramen magnum: achondroplastic values compared to normal standards.

Computerized tomographic dimensions of the foramen magnum of 63 achondroplastic individuals were compared to standards established for nonachondroplastic individuals. The size of the foramen magnum in patients with achondroplasia was small at all ages, particularly in those with serious neurologic problems. The data suggest that measurement of the foramen magnum may identify achondroplastic individuals at high risk of developing neurologic complications.

Chronic tonsillar herniation: an attempt at classifying chronic hernitations at the foramen magnum.

A system is presented for the classification of chronic herniations of the cerebellar tonsils in the absence of space-occupying intracranial lesions, based on a survey of the literature and 13 own cases. The Arnold-Chiari malformation in adults typically involves herniation of the cerebellar tonsils instead of herniation of the vermis as is typical when it occurs in infancy. Identification of chronic tonsillar herniation with the Arnold-Chiari malformation in adults was thought to require at least one other sign of the Arnold-Chiari complex, e.g. a medullary deformity. Cases for which chronic

Foramen magnum meningioma: long-term follow up without neurosurgery-A case report.

BACKGROUND: Foramen magnum meningiomas can cause lower cranial nerve deficits or brain stem symptoms. When they become symptomatic or when growth is documented, surgical resection is indicated. CASE PRESENTATION: In a white German 79-year-old woman, a small dorsal foramen magnum meningioma was detected by cerebral magnetic resonance imaging. After 7 years, cerebral magnetic resonance imaging showed considerable tumor growth (an increase in diameters from 15 × 16 × 20 mm to 28 × 30 × 37 mm), brain stem compression, and obstructive hydrocephalus. The clinical symptoms were however mild. The pat…

Median medullary perforators: Anatomical, surgical, and clinical significance in foramen magnum meningiomas.

The median medullary perforators, a group of perforator vessels arising from the vertebrobasilar junction, remain underrecognized. There is scarce knowledge regarding their clinical significance and relevance in foramen magnum surgery. This study elucidates the anatomy of these perforators and their clinical and surgical relevance, in analogy to the well-known vital basilar tip perforators. We highlight their potential contribution to selective pyramidal infarct or ischemia during foramen magnum tumor resection, and rarely their implication in isolated ischemic strokes of the medullary pyramid…

C1 Hemilaminectomy for Resection of Foramen Magnum Meningioma: 2-Dimensional Operative Video.

We present a case of a sizeable foramen magnum meningioma that was resected through a C1 hemilaminectomy in prone (concorde) position. The patient is a 51-year-old woman with a 3-month history of progressive paresthesia of the upper and lower extremities, followed by gait disturbance, and hand apraxia. There was no complaint of nuchal pain. On magnetic resonance imaging (MRI) a briskly enhancing extra-axial, intradural craniospinal lesion, extending from the basion of the lower clivus, over the tectorial membrane to the middle of the axis' body was discovered. There was significant transpositi…

Microsurgical Gross Total Resection of Foramen Magnum Meningioma via Far Lateral Approach.

Foramen magnum meningiomas are one of the most challenging tumors for skull base neurosurgeons due to their proximity to critical neurovascular structures. The far-lateral approach demonstrated here facilitates access to lesions involving the anterior portion of the foramen magnum. In this video, we present a 62-year-old woman with hand numbness and weakness. The patient had significant difficulty in fine motor movements of both hands. In the neurological examination, she had a significant right-hand intrinsic muscle weakness and mild quadriparesis. Magnetic resonance imaging (MRI) showed a du…

Benign Enhancing Foramen Magnum Lesions: Clinical Report of a Newly Recognized Entity.

Intradural extramedullary foramen magnum enhancing lesions may be due to meningioma, nerve sheath tumor, aneurysm, or meningeal disease. In this clinical report of 14 patients, we describe a novel imaging finding within the foramen magnum that simulates disease. The lesion is hyperintense on 3D-FLAIR and enhances on 3D gradient-echo sequences but is not seen on 2D-TSE T2WI. It occurs at a characteristic location related to the posterior aspect of the intradural vertebral artery just distal to the dural penetration. Stability of this lesion was demonstrated in those patients who underwent follo…

Foramen magnum stenosis and bilateral benign subdural collections in achondroplasia: case report.

A case of achondroplasia is presented in which cervico-medullary junction compression by foramen magnum stenosis and generalized seizures by bilateral subdural effusions were relieved by suboccipital craniectomy with C1 laminectomy and drainage of the collections. The important role played by NMR in the diagnosis of foramen magnum stenosis is stressed; the possible causes of hydrocephalus and subdural collections in achondroplasic patients are discussed. According to the authors surgical drainage of benign subdural collections in these patients is indicated in case of intractable seizures.

How I do it: endoscopic endonasal transclival resection of ventral foramen magnum meningioma.

BACKGROUND: Surgical management of ventral foramen magnum meningiomas (FMMs) is challenging due to intervening neurovascular structures. The endoscopic endonasal transclival approach provides a direct access, offering a potential route to achieve Simpson Grade 1 resection while minimizing brainstem retraction. METHOD: We detail the technical nuances of a binostril, four-handed EEA for a ventral FMM. We explain the key anatomical landmarks required for safe decompression, including clival drilling limits, the identification of pre-medullary neurovascular structures, and the execution of a mult…

Challenges in Diagnosing and Treating Foramen Magnum Dural Arteriovenous Fistulas: A Case Study.

Dural arteriovenous fistulas (DAVFs) represent a rare form of intracranial arteriovenous malformation, characterized by direct shunts between meningeal arteries and dural sinuses, dural veins, or cortical veins. DAVFs located in the foramen magnum (FM) region, including those involving the marginal sinus and condylar vein, exhibit complex arterial supply, diverse venous drainage patterns, and a range of clinical symptoms and associated risks. We present a case involving a 61-year-old male patient with a DAVF in the foramen magnum region, who clinically manifested limited abduction of the left …

Minimally invasive foramen magnum decompression using tubular retractors (MIFT) for Chiari I malformations.

BACKGROUND: Chiari malformation type 1 has been traditionally treated with foramen magnum decompression and C1 arch excision with or without duroplasty depending on the surgeon's preference. Each of the various surgical modifications has its advantages and disadvantages. METHODS: We describe a minimally invasive tubular retractor-based approach to achieve bony decompression in these cases. We have had good results comparable to the open approach. CONCLUSIONS: Strict orientation to the midline using soft tissue landmarks in between the muscles and bony landmarks in the deeper planes is import…

Pure posterior chordoid foramen magnum meningioma: A case report and review of literature.

INTRODUCTION AND IMPORTANCE: Meningiomas are extra-axial central nervous system (CNS) tumors that arise from the arachnoid cells of the dura mater. Only 1.8-3.2% of all meningiomas are located at foramen magnum (FM) and pure posterior FM meningioma are very rare. The diagnosis of malignancy in patients with meningiomas has been a controversial issue. Only a histological study can confirm this situation. CASE PRESENTATION: We report a case of A 52-year-old female presented with a history of neck pain with progressive spastic quadriparesis. CLINICAL DISCUSSION: Magnetic resonance imaging MRI T…

MRI characteristics of syringomyelia associated with foramen magnum arachnoiditis: differentiation from Chiari malformation.

BACKGROUND: It is important to distinguish foramen magnum arachnoiditis (FMA) from Chiari malformation (CM) before surgery because the operative strategies for these diseases differ. In the current study, we compared pretreatment magnetic resonance imaging (MRI) of FMA with CM and investigated the MRI findings useful to differentiate between these diseases. METHODS: We retrospectively reviewed patients with FMA or CM aged ≥ 18 years who underwent surgeries at our institution between 2007 and 2019. The morphologies of the syrinx, neural elements, and posterior cranial fossa were preoperatively…
