[The rheumatic forefoot].

The prevalence of rheumatic forefoot arthritis is estimated at 85-95%. Early synovitis of the metatarsophalangeal (MTP) joints is frequently neglected or overlooked. The disease leads to depression of the forefoot arch, dislocation of the MTP joints and hallux valgus with severe metatarsalgia. Operative treatment may give good results in 77-91% of cases. Our preferred treatment consists of resection arthroplasties for the smaller toes and use of Swanson spacer for the big toe, extensive capsular and tendon release from the dorsal approach, reduction of the first metatarsal bone, relocation of

[Secondary correction on the forefoot].

If the results after operation on the forefoot are insufficient, in our opinion this is due to: (1) indications that are not strict enough (cosmetic of semicosmetic indication); (2) inadequate technique: insufficient consideration of the length of the toes and the metatarsus or correction of individual toe deformities instead of homogeneous treatment of the whole forefoot. As our results show, when considering all of these points, we obtain a substantial improvement in these feet. The metatarsal length was corrected by the Helal osteotomy or by resection of the head of the metatarsal bone. Whe

Residual adduction of the forefoot. A review of the Turco procedure for congenital club foot.

Forty-four feet in 28 children previously treated by a one-stage posteromedial release operation (the Turco procedure) were reviewed clinically and radiologically to determine the cause of residual adduction of the forefoot. In 21 clinically adducted feet (48%) the main cause of residual deformity was metatarsus varus alone or metatarsus varus in spite of talonavicular overcorrection; in five feet the cause was talonavicular subluxation. There was no residual adduction in 23 feet (52%) but only 12 had normal radiographic measurements. In the remaining feet, various forms of spurious correction

Barefoot pressure distribution of diabetic patients and non-diabetic volunteer subjects after sensorimotor training with an unstable shoe construction.

BACKGROUND: Forty-three percent of all diabetic foot ulcers occur under the medial forefoot due to a medial deviation of elevated pressures and premature forefoot ground contact in neuropathic diabetic patients. A 6-week sensorimotor training period with an unstable shoe construction reduces in-shoe peak pressures and contact times under the medial aspect of the forefoot. METHODS: The study was designed as a Randomised Control Trial with two diabetic groups (one served as intervention group and one as control group) and one non-diabetic intervention group. Measurements for barefoot pressure d

Prototype Development of a Combined Padded Metatarsal Cup for High Heeled Footwear to Enhance Female Foot Musculoskeletal Safety.

This study investigated the feasibility of combined padded metatarsal cup on plantar pressures and stress distribution in the bone alignment of female foot with high heeled footwear during balanced standing. The aim of this study is to redistribute the plantar pressure away from the medial side of the forefoot. A combined padded metatarsal cup (CPMC) was developed from medium soft ethylene vinyl acetate (MSEVA) and very soft ethylene propylene diene monomer (VSEPDM) neoprene sponge. The participants of three categories were selected for the study. The peak plantar pressure and a radiographic a

Bullous pemphigoid after bilateral forefoot surgery.

UNLABELLED: The case of a 76-year-old woman who developed blisters on both forefeet within 2 weeks of bilateral forefoot surgery is reported; the diagnosis of bullous pemphigoid was made by histological and immunofluorescence studies. The pathogenesis and management of this autoimmune condition, which occurs mainly in patients older than 70 years and may occasionally be associated with surgical wounds, are reviewed. It is important for foot and ankle specialists to be aware of and consider the diagnosis when a bullous contact dermatitis-like eruption persists at a surgical site. LEVEL OF EVID

Forefoot supinatus.

The supination of the forefoot that develops with adult acquired flatfoot is defined as forefoot supinatus. This deformity is an acquired soft tissue adaptation in which the forefoot is inverted on the rearfoot. Forefoot supinatus is a reducible deformity. Forefoot supinatus can mimic, and often be mistaken for, a forefoot varus. A forefoot varus differs from forefoot supinatus in that a forefoot varus is a congenital osseous deformity that induces subtalar joint pronation, whereas forefoot supinatus is acquired and develops because of subtalar joint pronation. This article discusses the acqui

Does excessive flatfoot deformity affect function? A comparison between symptomatic and asymptomatic flatfeet using the Oxford Foot Model.

Treatment of asymptomatic flexible flatfeet is a subject of great controversy. The purpose of this study was to examine foot function during walking in symptomatic (SFF) and asymptomatic (ASFF) flexible flatfeet. Thirty-five paediatric and juvenile patients with idiopathic flexible flatfeet were recruited from an orthopaedic outpatient department (14 SFF and 21 ASFF). Eleven age-matched participants with typically developing feet served as controls (TDF). To study foot function, 3D multi-segment foot kinematics and ankle joint kinetics were captured during barefoot gait analysis. Overall, alte

Assessment of plantar pressure in forefoot relief shoes of different designs.

BACKGROUND: After reconstructive forefoot surgery, patients require complete or partial forefoot relief, which can be obtained with a variety of shoe designs. The aim of this study was to evaluate the effectiveness of two different types of forefoot-relief shoes frequently used after surgery, especially their safety against unintentional forefoot load. METHODS: Ten healthy volunteers were asked to perform five trials on a treadmill at self-selected speeds. In the first trial, mean peak pressure values in mass-produced shoes and insoles were evaluated and considered as 100%. Two different shoe

Closure of central defects of the forefoot with external fixation: a case report.

Excision of 1 or more central rays of the foot may complicate soft tissue coverage because large defects may result. The authors presents a technique in which an Ilizarov external fixator was used to narrow the forefoot after resection of the central rays in a patient with diabetes. After external fixator application for 8 weeks, the central defect healed uneventfully and has not shown any recurrence of ulceration after 4 years of follow-up.

Pathomechanics of structural foot deformities.

This article presents the most common structural foot deformities encountered in clinical practice. The deformities are defined, and the expected compensations at the subtalar joint (STJ) are described. The theoretical consequences of the STJ compensations on proximal and distal tissues are presented. A biomechanical rationale for certain tissue disorders is described. The possible effects of abnormal STJ compensation on osseous development are briefly discussed.

Gait Adaptation to a Phase-Specific Nociceptive Electrical Stimulation Applied at the Ankle: A Model to Study Musculoskeletal-Like Pain.

Introduction: Lower limb pain, whether induced experimentally or as a result of a musculoskeletal injury, can impair motor control, leading to gait adaptations such as increased muscle stiffness or modified load distribution around joints. These adaptations may initially reduce pain but can also lead to longer-term maladaptive plasticity and to the development of chronic pain. In humans, many current experimental musculoskeletal-like pain models are invasive, and most don't accurately reproduce the movement-related characteristics of musculoskeletal pain. The main objective of this study was t

Variations in plantar pressure and balance in HIV-infected children in antiretroviral therapy.

Balance disorders have been poorly investigated and somewhat neglected in people infected with the human immunodeficiency virus, especially in children, whose have intrinsic and extrinsic risk factors that may compromise the balance. To evaluate the foot plantar pressures and the balance in children with acquired immunodeficiency. We recruited 53 children aged between 6 and 15 years: 33 healthy children, and 20 children with positive serology for the human immunodeficiency virus. A physical examination included anthropometric, reflexes, tactile sensitivity of the foot and orthopedic evaluation

Differences in rearfoot, midfoot, and forefoot kinematics of normal foot and flatfoot during running.

Flatfoot is a common foot deformity, which could contribute to running injuries such as medial tibial stress syndrome. Intrafoot kinematics of flatfoot during walking have often been documented using multisegment foot models. However, the intrafoot kinematics of flatfoot during running remains unclear, despite the possible relationship between flatfoot and running injuries. We aimed to clarify rearfoot, midfoot, and forefoot kinematics when running in participants with normal foot and flatfoot. Participants with the normal foot (n = 14) and flatfoot (n = 14) were asked to runover-ground at the

Accurate determination of relative metatarsal protrusion with a small intermetatarsal angle: a novel simplified method.

Several published studies have explained in detail how to measure relative metatarsal protrusion on the plain film anteroposterior pedal radiograph. These studies have demonstrated the utility of relative metatarsal protrusion measurement in that it correlates with distal forefoot deformity or pathologic features. The method currently preferred by practitioners in podiatric medicine and surgery often presents one with the daunting challenge of obtaining an accurate measurement when the intermetatarsal 1-2 angle is small. The present study illustrates a novel mathematical solution to this probl
