In clinical practice frontal headaches are common however are frequently incorrectly attributed to rhinosinusitis. The misdiagnosis of frontal headaches and/or facial pain has been compounded by increased accessibility and over-reliance on sinus CT imaging. We reviewed the presentation and clinical findings of 3 patients in our unit with frontal headaches misattributed to rhinosinusitis. Two patients are presented with extradural empyema originating from the frontal sinus. The infection spread was in the first case via the diploic veins through the intact sinus wall, and in the second case after the destruction of the posterior wall of frontal sinus. The early diagnosis methods and surgical drainage are discussed. Braid scan and Computed tomography are the most useful methods for the evaluation of this pathology.

The diagnostic value of Technetium 99m Methylene Diphosphate bone scintigraphy was evaluated in 21 patients, 10 of them with acute frontal sinusitis and a "fluid level" on sinus radiography. Pathologic accumulation of the radioisotope over the region corresponding to the diseased frontal sinus was observed in 9 of these 10 patients, 5 of them having a marked accumulation.
