Generalized anxiety disorder: connections with self-reported attachment.

Even though generalized anxiety disorder (GAD) is one of the most common of the anxiety disorders, relatively little is known about its precursors. Bowlby's attachment theory provides a framework within which these precursors can be considered. According to Bowlby, adult anxiety may be rooted in childhood experiences that leave a child uncertain of the availability of a protective figure in times of trouble.Furthermore, adult "current state of mind with respect to attachment" is thought to relate to adult anxiety. Both attachment-related components were assessed with 8 subscales of the Percept…

A transdiagnostic evaluation of contrast avoidance across generalized anxiety disorder, major depressive disorder, and social anxiety disorder.

BACKGROUND: The contrast avoidance model (CAM) proposes that persons with generalized anxiety disorder (GAD) are sensitive to sharp increases in negative emotion or decreases in positive emotion (i.e., negative emotional contrasts; NEC) and use worry to avoid NEC. Sensitivity to and avoidance of NEC could also be a shared feature of major depressive disorder (MDD) and social anxiety disorder (SAD). METHODS: In a large college sample (N = 1409), we used receiver operating characteristics analysis to examine the accuracy of a measure of emotional contrast avoidance in detecting probable GAD, MD…

Thyroid hormones in panic disorder, panic disorder with agoraphobia, and generalized anxiety disorder.

Fifty-two patients with generalized anxiety disorder who had symptoms persisting for at least 6 months, 41 patients suffering from either panic disorder (32 patients) or panic disorder with agoraphobia (9 patients), and 14 control subjects were screened for thyroid disease. Total serum thyroxine (TT4), serum-free thyroxine index (FT4I), and triiodothyronine resin uptake (T3RU), were examined for the entire sample, using a one-way analysis of variance (ANOVA). No significant differences were found in TT4 (p = .24), FT4I (p = .24), and T3RU (p = .19). Thyroid-stimulating hormone (TSH) was examin…

Barriers to mental health treatment among individuals with social anxiety disorder and generalized anxiety disorder.

Individuals with social anxiety disorder (SAD) or generalized anxiety disorder (GAD) are at risk for not utilizing mental health treatment. The purpose of this research was to examine barriers to treatment in a sample of adults with clinically significant SAD or GAD. Participants were 226 nontreatment-seeking adults with SAD or GAD who underwent semistructured diagnostic interview and received a clinician assessment of symptom severity as part of a clinical research study. Participants completed a self-report measure of barriers to treatment. Individual and combined associations of demographic…

Distinguishing generalized anxiety disorder, panic disorder, and mixed anxiety states in older treatment-seeking adults.

Eighty treatment-seeking adults age 60 or over with panic disorder, generalized anxiety disorder, and mixed anxiety states (generalized anxiety with panic attacks, panic disorder with secondary generalized anxiety) completed a clinical assessment and battery of self report measures. Several hypotheses were tested from the domains of distinguishing symptoms, associated features, and rates of comorbidity with other disorders. Greater between- than within-group variance was found on a subset of measures suggesting that the distinction between GAD and PD is generally valid in the older adult popul…

Response Inhibition, Cognitive Flexibility and Working Memory in Obsessive-Compulsive Disorder, Generalized Anxiety Disorder and Social Anxiety Disorder.

This study analyzed response inhibition, cognitive flexibility and working memory in three groups of patients diagnosed with obsessive-compulsive disorder, social anxiety disorder and generalized anxiety disorder, considering some variables that may influence results (nonverbal reasoning, comorbidity, use of pharmacotherapy). Neuropsychological measures were completed using a computerized Wisconsin card sorting test, Stroop color word test, go/no-go task, digits and Corsi. Significant differences were obtained among groups in cognitive flexibility and working memory variables. The obsessive-co…

Specifically altered brain responses to threat in generalized anxiety disorder relative to social anxiety disorder and panic disorder.

BACKGROUND: Despite considerable effort, the neurobiological underpinnings of hyper-responsive threat processing specific to patients suffering from generalized anxiety disorder (GAD) remain poorly understood. The current functional magnetic resonance imaging (fMRI) study aims to delineate GAD-specific brain activity during immediate threat processing by comparing GAD patients to healthy controls (HC), to social anxiety disorder (SAD) and to panic disorder (PD) patients. METHOD: Brain activation and functional connectivity patterns to threat vs. neutral pictures were investigated using event-…

The hyperventilation syndrome in panic disorder, agoraphobia and generalized anxiety disorder.

The symptom complex of panic disorder and generalized anxiety disorder suggests an etiological role for hyperventilation. The present study investigates the overlap between DSM-III-R panic disorder, panic disorder with agoraphobia and generalized anxiety disorder with hyperventilation syndrome (HVS). The anxiety disorder diagnoses were based on a structured interview, and HVS syndrome (HVS). The anxiety disorder diagnoses were based on a structured interview, and HVS determined by the so-called hyperventilation provocation test (a brief period of voluntary hyperventilation with recognition of …

An examination of core beliefs in generalized anxiety disorder.

OBJECTIVE: Generalised anxiety disorder (GAD) is characterised by uncontrollable worries. Cognitive behaviour therapy (CBT) is an effective treatment for GAD; however, manualised treatments rarely include interventions to target core beliefs. This study aimed to (1) explore the core beliefs about the self that are endorsed by individuals with GAD, (2) determine whether these beliefs are related to the severity of GAD symptoms, (3) examine if core beliefs about the self shift in response to CBT for GAD and (4) determine whether changes in core beliefs about the self are related to reductions in

Imagery rescripting for generalized anxiety disorder: A case series.

INTRODUCTION: This is the first clinical pilot to explore the feasibility, acceptability and preliminary efficacy of imagery rescripting in individuals with a diagnosis of generalized anxiety disorder (GAD). METHODS: Participants with GAD (N = 18) attended assessment and 10 weekly sessions of imagery rescripting with a psychologist in person or via Telehealth. The protocol was based on Arntz's phased protocol and common rescripting procedures with adaptations to target the future orientation of worry. Measures of trait worry and GAD symptoms were administered pre-baseline (before a two-week b…

ZDHHC5 Mediates Immune Dysregulation Driving Generalized Anxiety Disorder Risk.

OBJECTIVE: To investigate genes associated with palmitoylation modification implicated in generalized anxiety disorder (GAD), and to elucidate the mechanistic roles of these genes via immune cell-mediated pathways. METHODS: Utilizing large-scale genetic datasets, genes categorized as palmitoylation-expression quantitative trait loci (eQTL) were identified by intersecting palmitoylation-related genes (as compiled from the most recent reviews) with genome-wide eQTL datasets. A two-sample Mendelian randomization (MR) methodology was applied to investigate the causal relationship between palmitoy…

Generalized anxiety disorder patients' cognitive control in affective contexts.

BACKGROUND: Growing evidence suggests a relationship between deficits in cognitive control and anxiety. However, studies examining cognitive control within affective contexts (affective control) are limited, and the specific characteristics of affective control in patients with Generalized Anxiety Disorder (GAD) remain unclear. This study investigated whether differences exist in cognitive control under affective contexts. METHODS: We conduct our research in a population of GAD patients (n = 50) and a healthy control group (n = 50). The affective flanker task measured affective inhibition, wh…

Clinical relevance of venlafaxine ER for generalized anxiety disorder.

A randomized, double-blinded, placebo-controlled study by Otsubo et al. demonstrated a statistically significant effect of venlafaxine in patients with generalized anxiety disorder. However, the magnitude of improvement may have little clinical value. Methodological factors-including the exclusion of placebo responders, the inclusion of participants with comorbid depressive disorders, and a higher rate of discontinuation due to adverse events-further diminish the clinical significance of the findings. Overall, although venlafaxine achieved statistical efficacy, it may lack a clinically meaning…

Association between visceral adiposity and generalized anxiety disorder (GAD).

BACKGROUND AND OBJECTIVES: Due to an increased rate of inflammation in generalized anxiety disorder (GAD), insight into the mediating factors in the onset and recurrence of the inflammatory response can help to achieve novel treatments for alleviating the risk of GAD. In the current study, we aimed to evaluate the possible relationship between visceral adipose tissue (VAT) as an important intermediary in inflammation pathways and GAD in participants of the Employees' Health Cohort Study of Iran (EHCSIR). METHOD: We analyzed the data from 3889 included participants aged > 18 years in the EHCSI…

Salivary Antioxidant Capacity and Magnesium in Generalized Anxiety Disorder.

Generalized anxiety disorder (GAD) is a prevalent disorder. The search for biomarkers may contribute to new knowledge about molecular pathogenesis and treatment. Since oxidative stress and micronutrient imbalance play a key role in the development of mental disorders, we aimed to study salivary antioxidant capacity and magnesium in patients with GAD in an anxiety model of solving problems with increasing complexity. The study subgroup consisted of 15 patients with GAD, and 17 healthy volunteers of the same age made up the control subgroup. Participants took a test with six levels of difficulty…

Identifying generalized anxiety disorder using resting state habenular circuitry.

Studies identify the habenula as a key subcortical component in anxiety, with a role in predicting error coding within the evaluative system. However, no clinical reports of generalized anxiety disorder (GAD) describe resting state functional connectivity of habenular circuits. We hypothesized that resting-state functional connectivities of habenula would show differences in neuroanatomical correlates of the evaluative system (prefrontal cortex, habenula) of patients with GAD. We obtained 22 patients with GAD and 21 HCs, matched for gender, age, and years of education. Resting-state functional…

Affective neuroimaging in generalized anxiety disorder: an integrated review.

Affective neuroimaging has contributed to our knowledge of generalized anxiety disorder (GAD) through measurement of blood oxygenation level-dependent (BOLD) responses, which facilitate inference on neural responses to emotional stimuli during task-based functional magnetic resonance imaging (fMRI). In this article, the authors provide an integrated review of the task-based affective fMRI literature in GAD. Studies provide evidence for variable presence and directionality of BOLD abnormalities in limbic and prefrontal regions during reactivity to, regulation of, and learning from emotional cue…
