Integrated optic nerve sheath diameter, glasgow coma scale and computed tomography based model for early prediction of clinically significant brain injury in head trauma: A prospective observational study.

Acute head injury requires rapid identification of clinically significant brain injury (CSBI), yet access to CT imaging is often limited, creating challenges in early triage. In this prospective study of 215 head trauma patients, optic nerve sheath diameter (ONSD) and Glasgow Coma Scale (GCS) were evaluated as predictors of CSBI. Patients with CSBI had significantly higher ONSD and lower GCS and both variables independently predicted injury severity. The combined ONSD + GCS model demonstrated better diagnostic performance, clinical utility and calibration than either parameter alone, with ONSD

Prognostic Utility of Daily Changes in Glasgow Coma Scale and the Full Outline of Unresponsiveness Score Measurement in Patients with Metabolic Encephalopathy, Central Nervous System Infections and Stroke in Uganda.

BACKGROUND: Metabolic encephalopathy (ME), central nervous system (CNS) infections, and stroke are common causes of reduced level of consciousness in Uganda. However, the prognostic utility of changes in the daily measurements of the Full Outline of Unresponsiveness (FOUR) score and Glasgow Coma Scale (GCS) score in these specific disorders is not known. METHODS: We conducted secondary analyses of data from patients who presented with reduced level of consciousness due to CNS infections, stroke, or ME to a tertiary hospital in Uganda. Patients had FOUR/GCS scores at admission and at 24 and 48

The Reverse Shock Index Multiplied by Glasgow Coma Scale Score (rSIG) and Prediction of Mortality Outcome in Adult Trauma Patients: A Cross-Sectional Analysis Based on Registered Trauma Data.

The reverse shock index (rSI) multiplied by Glasgow Coma Scale (GCS) score (rSIG), calculated by multiplying the GCS score with systolic blood pressure (SBP)/hear rate (HR), was proposed to be a reliable triage tool for identifying risk of in-hospital mortality in trauma patients. This study was designed to externally validate the accuracy of the rSIG in the prediction of mortality in our cohort of trauma patients, in comparison with those that were predicted by the Revised Trauma Score (RTS), shock index (SI), and Trauma and Injury Severity Score (TRISS). Adult trauma patients aged ≥20 years

Discriminant ability of the shock index, modified shock index, and reverse shock index multiplied by the Glasgow coma scale on mortality in adult trauma patients: a PATOS retrospective cohort study.

BACKGROUND: The shock index (SI) predicts short-term mortality in trauma patients. Other shock indices have been developed to improve discriminant accuracy. The authors examined the discriminant ability of the SI, modified SI (MSI), and reverse SI multiplied by the Glasgow Coma Scale (rSIG) on short-term mortality and functional outcomes. METHODS: The authors evaluated a cohort of adult trauma patients transported to emergency departments. The first vital signs were used to calculate the SI, MSI, and rSIG. The areas under the receiver operating characteristic curves and test results were used

Association of blood-based glial fibrillary acidic protein level with depression and suicidal ideation following traumatic brain injury with Glasgow Coma Scale score 13 to 15: a TRACK-TBI study.

Blood-based glial fibrillary acidic protein (GFAP) level within 24 h of traumatic brain injury (TBI) has been inversely associated with post-traumatic stress disorder at 6 months in the Transforming Research and Clinical Knowledge in Traumatic Brain Injury (TRACK-TBI) study. We sought to assess the relationship between day-of-injury GFAP and cumulative prevalence (CI) of depression or suicidal ideation in the first year after injury among patients presenting with Glasgow Coma Scale 13-15 who participated in Transforming Research and Clinical Knowledge in Traumatic Brain Injury (n = 1511). Analysis of dynamic changes in optic nerve sheath diameter (ONSD) with ultrasound in post-craniotomy patients: Trends and correlation with computed tomography ONSD and Glasgow coma scale in post-operative period.

OBJECTIVES: Intracranial pressure (ICP) monitoring in patients with intracranial tumors undergoing craniotomy is usually done in perioperative period in intensive care unit. Invasive measurement of ICP, though considered as the gold standard, has its own limitations such as availability of expertise, equipment, and associated complications. Period of raised ICP in post-operative period may impact patient outcomes. Post-craniotomy computed tomography (CT) assessment is done routinely and may need to be repeated if indicated during post-operative stay. Utility of sonographic serial optic nerve s

Reverse shock index multiplied by Glasgow Coma Scale score (rSIG) is a simple measure with high discriminant ability for mortality risk in trauma patients: an analysis of the Japan Trauma Data Bank.

BACKGROUND: The shock index (SI), defined as heart rate (HR) divided by systolic blood pressure (SBP), is reported to be a more sensitive marker of shock than traditional vital signs alone. In previous literature, use of the reverse shock index (rSI), taken as SBP divided by HR, is recommended instead of SI for hospital triage. Among traumatized patients aged > 55 years, SI multiplied by age (SIA) might provide better prediction of early post-injury mortality. Separately, the Glasgow Coma Scale (GCS) score has been shown to be a very strong predictor. When considering these points together, rS

A Least Absolute Shrinkage and Selection Operator (LASSO)-derived AAGC Model for In-hospital Mortality Prediction in Sepsis Incorporating Age, APACHE II Score, Glasgow Coma Scale, and Creatinine: A Prospective Study.

BACKGROUND AND AIMS: Sepsis remains a major global cause of critical illness and death. Existing scores like Acute Physiology and Chronic Health Evaluation II (APACHE II), Sequential Organ Failure Assessment (SOFA), and quick Sequential Organ Failure Assessment (qSOFA) vary widely, highlighting the need for earlier, reliable risk identification. We aimed to identify key predictors of mortality among inpatients with sepsis and develop a simplified data-driven predictive model using the least absolute shrinkage and selection operator (LASSO) method. Neuroworsening from a normal Glasgow Coma Scale Motor Score in the emergency department is an early predictor of neurosurgical intervention, hospital outcomes, and longitudinal disability in traumatic brain injury: A TRACK-TBI Study.

OBJECTIVE: Neuroworsening portends poor outcomes after traumatic brain injury (TBI) and is protocolized in intensive care unit (ICU) settings. The utility of neuroworsening assessments in non-ICU settings for intervention and prognostication requires further understanding. This study assessed relationships among neuroworsening in the emergency department (ED), clinicoradiological injury, blood-based biomarkers, neurosurgical interventions, and outcomes in TBI patients without Glasgow Coma Scale-Motor Score (GCS-M) impairment at ED arrival. METHODS: Adult subjects from the 18-center Transformi

Early rehabilitation after severe brain injury: a French perspective.

Severely brain-injured patients often suffer from disabilities and psycho-social handicaps. Early rehabilitation aims at improving their motor and functional recovery while preventing or treating complications as soon as possible. In this review we look at some issues encountered in early rehabilitation. We illustrate our discussion with data from 876 French traumatic brain injury patients admitted over the course of 1 year at 18 rehabilitation units that were asked for details of their current practice. Preservation of vital functions follows standardized protocols, but rehabilitation is more

Risk factors in the outcome of children with minor head injury.

A subset of children with minor head injury is known to develop serious neurological consequences, but identifying this subset has been difficult. In adults, multiple variables such as skull fracture, Glasgow Coma Scale score, unconsciousness, and amnesia are significant factors that determine whether to admit the patient to the hospital and the patient's outcome. As an objective assessment of neurological function, however, the Glasgow Coma Scale has limited usefulness in children, particularly those under 36 months of age. We report our experience with 937 children having head injuries using
