Initiating Nutritional Support Before 72 Hours Is Associated With Favorable Outcome After Severe Traumatic Brain Injury in Children: A Secondary Analysis of a Randomized, Controlled Trial of Therapeutic Hypothermia.

OBJECTIVES: To understand the relationship between the timing of initiation of nutritional support in children with severe traumatic brain injury and outcomes. DESIGN: Secondary analysis of a randomized, controlled trial of therapeutic hypothermia (Pediatric Traumatic Brain Injury Consortium: Hypothermia, also known as "the Cool Kids Trial" (NCT 00222742). SETTINGS: Fifteen clinical sites in the United States, Australia, and New Zealand. SUBJECTS: Inclusion criteria included 1) age less than 18 years, 2) postresuscitation Glasgow Coma Scale less than or equal to 8, 3) Glasgow Coma Scale mot

Traumatic brain injury. Predicting course of recovery and outcome for patients admitted to rehabilitation.

OBJECTIVE: To demonstrate that the prognosis for patients with traumatic brain injury (TBI) admitted to rehabilitation can be established wi … on unit (age range, 8 through 89 years). MAIN OUTCOME MEASURES: Functional outcome measured by the Glasgow Outcome Scale at 6 and 12 months after injury. RESULTS: Posttraumatic amnesia had a clear, predictable relationship to length of coma in patients with diffuse axonal injury (R2 = .58, P < .0001). Severity measures, particularly duration of posttraumatic amnesia, …

Efficacy of nimodipine in the treatment of subarachnoid hemorrhage: a meta-analysis.

BACKGROUND: Subarachnoid hemorrhage (SAH) is an uncommon and serious subtype of stroke, which leads to the loss of the patient's ability to produce and live for many years. OBJECTIVE: To investigate the clinical effect of nimodipine in the treatment of SAH. METHODS: Electronic databases including China National Knowledge Infrastructure (CNKI), VIP, SinoMed, China Master's Theses Full-text Database (CMFD), China Doctoral Dissertations Full-text Database (CDFD), Cochrane Library, PubMed and Embase were searched from 2010 and 2021. Clinical features and treatment strategies for vertex epidural hematoma: a systematic review and meta-analysis from individual participant data.

Vertex epidural hematoma (VEDH) is a rare extradural hematoma and often misdiagnosed because of its variety of clinical symptoms and charact … . Totally, two patients (3%) died, but most patients (94%) had a favorable outcome scoring 5 on the Glasgow Outcome Scale. Although VEDH generally showed favorable outcomes, clinicians must be aware of a high rate of delayed neurological deterioration during the observation period, which can be fatal due to central downward herniation.

Epidemiology of head injuries in pedestrian-motor vehicle accidents.

Despite efforts of many countries to improve pedestrian safety, international reports show an upward trend in pedestrian-motor vehicle accidents. Although the most common cause of death of pedestrians is head injuries, there is a lack of knowledge on the epidemiology and characteristics of head injury in terms of the Glasgow Outcome Scale to be used for prevention. However, this study aimed to determine the epidemiology of pedestrian-motor vehicle accidents, the characteristics of head injury, and differences in the Glasgow Outcome Scale in terms of gender. In this retrospective analytical stu…

Prognostic Analysis of Emergency Decompressive Craniectomy for Patients with Severe Traumatic Brain Injury with Bilateral Fixed Dilated Pupils.

OBJECTIVE: For patients with severe traumatic brain injury (sTBI) with bilateral fixed dilated pupils (BFDP), the value of aggressively decompressive craniectomy (DC) treatment is still controversial. The objective of this study was to analyze and validate the outcome of DC in patients with sTBI with BFDP. METHODS: We retrospectively collected data from 44 patients with sTBI with BFDP who underwent DC treatment from July 2011 to June 2018. Outcomes used as indicators were mortality and favorable outcome. The analysis was based on the Glasgow Outcome Scale score recorded at discharge, 6, and 1…

DESIGN: Retrospective case study. SETTING A single tertiary care pediatric hospital. PARTICIPANTS: Fifty-nine cases who had nontraumatic hemorrhages. The clinical outcome of patients with severe head injuries, treated with highdose dexamethasone, hyperventilation and barbiturates.

The outcome in 45 consecutive patients following severe head injuries is presented. All patients were evaluated according to the Glasgow Coma Scale and the Glasgow Outcome Scale. All patients received a combined treatment consisting of dexamethasone, barbiturate and hyperventilation. A primary operative intervention was performed in 14 patients with intracranial haematomas. There was a good recovery in 53%, 20% were moderately disabled, 11% were severely disabled. 16% died or remained in a vegetative state. The patients were aged between 5 and 83 years, 49% were under 20 years, and the relatio…

Incidence and effects of increased cerebral blood flow velocity after severe head injury: a transcranial Doppler ultrasound study II. Effect of vasospasm and hyperemia on outcome.

Cerebral blood flow velocities were measured in 50 severely head injured (Glasgow Coma Scale (GCS) 8 or less) patients using transcranial Doppler ultrasonography (TCD). Abnormally high TCD velocities were recorded in 35 patients; in 20 this was deemed to be due to vasospasm and in the other 15 to hyperemia. Patients who developed hyperemia also had the highest intracranial pressure (ICP) and the lowest cerebral perfusion pressure (CPP) of the three groups. Outcome was assessed at six months after injury using the Glasgow outcome scale and the disability rating scale. In the normal velocity gro…
