[Bacterial meningitis as first manifestation of a patient with multiple myeloma. A case report].

BACKGROUND: Multiple myeloma (MM) is a plasma cell neoplasm characterized by clonal proliferation of plasma cells in the bone marrow and monoclonal protein in serum and/or urine in association to target organ damage. Infections are a frequent finding and they are multifactorial and presented mainly in the first months of diagnosis. CLINICAL CASE: We reported the case of a 62 years old woman, with impaired alertness, drowsiness, fever, incoherent speech and meningeal signs, cerebrospinal fluid showed polymorphonuclear pleocytosis, hypoglycorrhachia, hyperproteinorrhachia, and Gram positive coc

Isolation of Helcococcus ovis from sheep with pleuritis and bronchopneumonia.

Helcococcus ovis is a newly established species in the genus Helcococcus. The clinical significance of this organism in sheep has not been reported. In the current report, isolation of H. ovis from a 6-month-old mixed-breed ewe lamb that died of respiratory disease is described. Pathologic examination revealed severe, focally extensive, chronic necrotizing pleuritis with intralesional coccobacilli and mild, multifocal, subacute mucopurulent bronchopneumonia, indicating a bacterial etiology. A Gram-positive bacterium was isolated in heavy growth from the lung tissue. DNA sequence analysis on th

Cerebral mycotic aneurysm complicating Stomatococcus mucilaginosus infective endocarditis.

Stomatococcus mucilaginosus is a gram-positive coccus present in the normal flora of the mouth and upper respiratory tract of humans. Although traditionally believed to be an organism of low virulence, S. mucilaginosus has recently been recognized as an emerging opportunistic pathogen, especially in patients with chronic immunosuppressive diseases. This report describes the case of a patient with known mitral valve prolapse, who presented with a mycotic cerebral aneurysm revealing a non-nosocomial spontaneous S. mucilaginosus endocarditis. The spectrum of infections due to this opportunistic p

Isolation of Helcococcus kunzii from plantar phlegmon in a vascular patient.

Helcococcus kunzii has previously been considered to belong to the normal skin flora of podiatry patients. Here, H. kunzii was isolated in abundance from a pus specimen collected by incision and drainage of plantar phlegmon. This fastidious Gram-positive species was unambiguously identified with the colorimetric VITEK 2 GP card identification system. This suggests that this phenotypic identification system is able to identify promptly H. kunzii, which should be considered a potential pathogen.

Teicoplanin. A pharmacoeconomic evaluation of its use in the treatment of gram-positive infections.

Teicoplanin, a glycopeptide antibiotic, is active against Gram-positive organisms, including methicillin-resistant staphylococci. It has demonstrated similar efficacy to vancomycin in the treatment of Gram-positive infections in febrile patients with neutropenia; fewer comparative data are available in patients with other infection types. Compared with vancomycin, teicoplanin is associated with less nephrotoxicity, appears to cause fewer anaphylactoid reactions, requires less monitoring and is more convenient to administer (once daily by intravenous bolus or intramuscular injection vs 2 to 4 t

Cefditoren activity against nearly 1000 non-fastidious bacterial isolates and the development of in vitro susceptibility test methods.

Cefditoren (formerly ME-1206) is an investigational, orally administered cephalosporin ester with bactericidal activity against many Gram-positive and -negative organisms. Cefditoren potency against nearly 1000 non-fastidious species was determined by National Committee for Clinical Laboratory Standards (NCCLS) reference broth microdilution and standardized disk diffusion methods. Against staphylococci, usable cefditoren activity was completely correlated with oxacillin with respect to potency and susceptibility interpretation (mec A-negative strains). Cefditoren was very active against Klebsi

Time to First Culture Positivity Among Critically Ill Adults With Methicillin-Resistant Staphylococcus aureus Growth in Respiratory or Blood Cultures.

Background: For critically ill adults receiving empirical vancomycin, the duration of negative cultures after which vancomycin may be discontinued without risking subsequent growth of methicillin-resistant Staphylococcus aureus (MRSA) remains unknown. Objective: We hypothesized that if sputum cultures did not grow MRSA or blood cultures did not grow Gram-positive cocci on Gram stain by 48 hours, those cultures would not subsequently demonstrate MRSA. Methods: We conducted an ancillary analysis from patients enrolled in the Isotonic Solutions and Major Adverse Renal Events Trial (SMART). In thi…

Antimicrobial resistance among Gram-positive pathogens in Saudi Arabia.

Several species of Gram-positive cocci are major nosocomial or community pathogens associated with morbidity and mortality. Here, we review the antimicrobial resistance among these pathogens in Saudi Arabia. In the last decades, antimicrobial resistance has increased among Staphylococcus aureus in the Kingdom with a growing prevalence of both nosocomial and community methicillin-resistant S. aureus (MRSA) isolates. As yet, no vancomycin-resistant MRSA have been reported, although isolates with reduced susceptibility to the drug have been noted. Currently, the prevalence of vancomycin-resistant…

[Bacterial infections of skin and soft tissues in filariasis].

Adenolymphangitis is a common occurrence in filarial lymphedema. Damage to the lymphatics and lymph nodes by F. bancrofti is followed by obliteration of lymph vessels and lymph stasis. Obstruction of lymphatics prevents the bacteria penetrating skin to be evacuated with lymph stream to regional lymph nodes. Colonization of dermis, subcutis and lymphatics evokes clinical symptoms of adenolymphangitis. The question arises which strains of bacteria are responsible for the acute and chronic types of adenolymphangitis. The most probable strains responsible for this condition belong to the cocci and

Staphylococcus-induced gangrenous dermatitis in broilers.

An infectious bursal disease (IBD)-vaccinated flock of 23,900 broilers, 17 days of age, experienced sudden onset of depression, dermatitis, and mortality. Postmortem examination showed extensive subcutaneous serosanguineous fluid accumulation over the pectoral muscles, discrete hepatic whitish foci, fluid-filled intestines, and small, flaccid bursae of Fabricius. Gram-stained impression smears from the affected areas revealed numerous gram-positive cocci. Aerobic culture of liver and subcutaneous tissue consistently produced heavy growth of penicillin-sensitive Staphyloccus aureus. Histopathol…

Performance evaluation of machine-assisted interpretation of Gram stains from positive blood cultures.

Manual microscopy of Gram stains from positive blood cultures (PBCs) is crucial for diagnosing bloodstream infections but remains labor intensive, time consuming, and subjective. This study aimed to evaluate a scan and analysis system that combines fully automated digital microscopy with deep convolutional neural networks (CNNs) to assist the interpretation of Gram stains from PBCs for routine laboratory use. The CNN was trained to classify images of Gram stains based on staining and morphology into seven different classes: background/false-positive, Gram-positive cocci in clusters (GPCCL), Gr…
