Posterior compartment prolapse: a urogynecology perspective.

Posterior compartment prolapse is often caused by a defect in the rectovaginal septum, also known as Denonvillier's fascia. Patients with symptomatic posterior compartment prolapse can present with bulge symptoms as well as defecatory dysfunction, including constipation, tenesmus, splinting, and fecal incontinence. The diagnosis can successfully be made on clinical examination. Treatment of posterior prolapse includes pessaries and surgery. Both traditional colporrhaphy and site-specific defect repair have excellent success rates. Complications from surgery can include sexual dysfunction, de n

[Laparoscopic promontofixation].

OBJECTIVES: The pathology of the pelvic floor, including the urinary incontinence, the anal incontinence and the genital prolapse, is very dominant, concerning approximately a third of the adult women. It is fundamental that this musculature supports a good function, because of the weakness of the pelvic floor produces urinary incontinence, cysto and rectocele, genital prolapses and sexual dysfunctions. The above mentioned pathology can be corrected by laparoscopic promontofixation, whatever the previous history of pelvic surgery, including the placing of prosthetic material. In this article w

[Surgical treatment of prolapse using laparoscopic promontofixation: technical principles and results].

UNLABELLED: Abdominal sacrofixation is the gold standard for the treatment of the prolapse. There are many ways to do it: technical, meshes, dissection, fixation of the mesh, associated procedures. Laparotomy is the classical procedure for sacrofixation. The basis of sacrofixation is to dissect the weak vesicovaginal and rectovaginal fascias and to replace with meshes spread out on the entire dissected surface. PROCEDURE: Suprapubic abdominal incision, dissection of the anterior vertebral ligament on the right of the promontory, dissection of the vesicovaginal and rectovaginal spaces; meshes

[Surgical treatment of prolapse using coelioscopic promontofixation: techniques and results].

The laparoscopic sacrocolpopexy is the preferred procedure for the young woman. The procedure is that used for pelvic laparoscopy. The steps are identical as during open sacrocolpopexy, with the advantages of pneumodissection and better vision. The success rate is more than 90% and redux is mostly cystocele. Mesh erosion is reported in 2,7 to 9%. The indication for a posterior mesh is recommended if there is a rectocele or if a colposuspension at the same time. The fixation of the mesh must be posteriorly on the central tendon of perineum and the levator ani with a non resorbable stitches. The

Preoperative Mechanical Bowel Preparation for Gynecologic Surgeries: A Systematic Review with Meta-analysis.

OBJECTIVE: To assess the efficacy and safety of mechanical bowel preparation (MBP) before benign laparoscopic or vaginal gynecologic surgeries. DATA SOURCES: Database searches of MEDLINE (PubMed), Embase (OVID), Cochrane Central Register of Controlled Trials, and Web of Sciences and citations and reference lists published up to December 2021. METHODS OF STUDY SELECTION: Randomized clinical trials in any language comparing MBP with no preparation were included. Two reviewers independently screened 925 records and extracted data from 12 selected articles and assessed the risk of bias with the

Laparoscopic major gynecologic surgery in patients with prior laparotomy bowel resection.

BACKGROUND AND OBJECTIVES: To review the success and morbidity of laparoscopic major gynecologic surgery in patients with prior laparotomy bowel resection. METHODS: Review of a prospective surgical database of all cases of laparoscopic major gynecologic surgery in patients with prior laparotomy bowel resection. No cases were excluded. Bowel diagnoses and procedures were total colectomy for inflammatory bowel disease (4), partial colectomy for colon cancer (6), partial small bowel resection for obstruction (1), and Whipple for pancreatic cancer (2). Two patients had 3 prior laparotomies, 8 pat

Technical aspects in the management of complex vesicovaginal fistulae.

Thirteen complex vesicovaginal fistulae were successfully closed by a technique of wide separation and water-tight closure of vagina from the urinary bladder and augmentation of the repair with a pedicled omental interposition flap.

Vaginal flap re-enforcement of vesico-vaginal fistula repair.

INTRODUCTION AND HYPOTHESIS: Vesicovaginal fistula (VVF) is a distressing urologic disorder. We describe a new technique that adds a third layer of closure during vaginal repair of VVF. METHODS: Twenty female patients with low VVF were included. A circumferential incision was made around the fistula that was completely excised. A rectangular vaginal flap was created just proximal to the site of the fistula. After closure of the urinary bladder, the distal edges of the vaginal flap were dissected and it was mobilized and interposed between the bladder and the vagina. Then the vagina was closed

Impact on Patient Care of a Multidisciplinary Center Specializing in Colorectal and Pelvic Reconstruction.

Aim of the study: Many patients with an anorectal malformation (ARM) or pelvic anomaly have associated urologic or gynecologic problems. We hypothesized that our multidisciplinary center, which integrates pediatric colorectal, urologic, gynecologic and GI motility services, could impact a patient's anesthetic exposures and hospital visits. Methods: We tabulated during 2015 anesthetic/surgical events, endotracheal intubations, and clinic/hospital visits for all patients having a combined procedure. Main results: Eighty two patients underwent 132 combined procedures (Table 1). The median age at

Minimally Invasive Gynecologic Surgery and Enhanced Recovery and Outcomes: A Literature Review.

Enhanced recovery after surgery (ERAS) protocols have significantly improved clinical outcomes in patients undergoing minimally invasive gynecological procedures. However, the absence of protocol uniformity among institutions, uneven implementation, uneven compliance, and the absence of research randomization limit the generalizability of findings. The aim of the review is to evaluate technological advancement, the efficacy of the integration of ERAS in minimally invasive gynecological surgery (MIGS) to improve clinical outcomes, challenges and controversies, and the way forward. The implement

Complex organ injuries after mid-trimester termination of pregnancy: pushing boundaries in laparoscopic management.

Surgical termination of pregnancy is one of the most often performed gynecologic procedures in the United Kingdom and worldwide. Although complications are rare, they can be devastating because they include hemorrhage and pelvic organ damage often necessitating hysterectomy. Traditionally, these complications have been managed via laparotomy; however, with increasing technological advances and surgical expertise, it is now possible to manage extreme complications of these procedures via operative laparoscopy. Herein is reported successful laparoscopic management of 3 cases of complex uterine p

Side-to-end reanastomosis after low-anterior resection (STELAR): Outcomes, feasibility, and description of procedure performed by a gynecologic oncology service.

BACKGROUND AND OBJECTIVES: Low anterior rectosigmoid resection for a gynecologic disease is usually performed in concert with other procedures and can result in significant morbidity should anastomotic complication occur. This study examined surgical outcomes of side-to-end reanastomosis after low anterior resection (STELAR) performed by gynecologic oncology service. METHODS: This is a case series examining consecutive patients who underwent STELAR for gynecologic indications by a single gynecologic oncology group from 2009 to 2018. Prospectively collected institutional surgical database was

Emergency Gynecologic Surgery Outcomes During Working and Off-Hours.

OBJECTIVE: To compare perioperative outcomes of emergency gynecologic surgeries performed during working hours and off-hours, and to evaluate temporal trends in surgical approach, particularly laparoscopy. METHODS: This retrospective cohort study included patients who underwent emergency surgery for ovarian torsion, ovarian cyst rupture, or ruptured tubal ectopic pregnancy between January 1, 2020 and October 1, 2025 at a tertiary referral center. Patients were grouped according to surgical timing as working hours or off-hours. Demographic characteristics, surgical approach, operative time, he
