A 6-year-old girl presented with necrosis of the left labium minus. An encircled hair at the base of the lesion was seen, and the diagnosis genital hair tourniquet syndrome was made. Genital hair tourniquet syndrome in children is a rare but potentially severe syndrome that requires immediate medical attention to prevent resulting necrosis.

Unwind or incise has been the standard of management for removing hair tourniquets. The hair ends are usually difficult to hold and unwind as they break at the ends easily, and using scalpels or needles to get under the hair tourniquet is difficult when the involved appendage is swollen and leads to more trauma to the injured area. This is the first case report that describes the removal of hair tourniquet using a depilatory cream.

We report on a 5-year-old girl whose hair could be pulled out with no effort and without causing pain. Occipital alopecia in a patchy pattern and with no sign of scalp inflammation or scarring was noted. A trichogram from the clinically involved area disclosed a striking predominance of anagen hair with absence of external root sheaths. Histological examination of the hair suggests abnormal premature keratinization of the inner root sheath, resulting in the loss of its anchoring function between the hair shaft and the follicle.
