Isolated abducens nerve palsy after closed head injury in a child.

The authors present the rare case of a 5-year-old child with an isolated right abducens nerve palsy following a minor closed head injury. Occlusion of the left eye for 4 h daily was prescribed, and the child remained under close ophthalmological follow-up. Significant improvement was noticed in the following 6 months.

Neurobehavioral functional deficits following closed head injury in the neonatal pig.

Neurobehavioral deficits in higher cortical systems have not been described previously in a large animal model of diffuse brain injury. Anesthetized 3-5 day old piglets were subjected to either mild (142 rad/s) or moderate (188 rad/s) rapid non-impact axial rotations of the head. Multiple domains of cortical function were evaluated 5 times during the 12 day post-injury period using tests of neurobehavioral function devised for piglets. There were no observed differences in neurobehavioral outcomes between mild injury pigs (N=8) and instrumented shams (N=4). Moderately injured piglets (N=7) had

Altered electrodermal response to facial expression after closed head injury.

UNLABELLED: The objective was to determine whether diffuse damage to orbital and ventromedial regions of the prefrontal cortex usually associated with moderate-to-severe closed head injury (CHI) would affect the ability to perceive and respond to socially relevant information. METHODS: Participants with CHI and age-matched non-injured controls were presented with faces that varied with respect to emotional expression while electrodermal activity (EDA) was monitored. Cognitive and general adaptive functioning was also assessed. RESULTS: CHI was associated with a failure to increase EDA in res

An examination of the impact of participation in a conversation group for individuals with a closed head injury.

This study describes the impact of participation in a conversational group for individuals with chronic closed head injury in the Department of Communication Pathology at the University of Pretoria over the period 1995-2000. The information was obtained through a combination of clinical observations by the writers; the examination of data from Pragmatic measures; and a Quality of Life Scale that was compiled and administered (to both the subjects and their significant others) examining the perceived effects of group therapy over time. The results showed that despite the plateauing of pragmatic

Damage to intracranial optic pathways in fatal closed head injury in man.

Head injury is a leading cause of visual impairment. This is partly due to direct trauma to the eye and optic nerve but much of the damage involves the intracranial optic pathways. We have studied the frequency, distribution and nature of the intracranial lesions of the optic pathways at autopsy in 45 cases of severe closed head injury, and examined the correlation between these post-mortem lesions and the ante-mortem clinical findings. Twenty-four of the patients had been involved in road traffic accidents. The ages ranged from 9 to 88 years (mean 46.4), the Glasgow Coma Score (GCS) on admiss

Sixth cranial nerve palsy following closed head injury in a child.

A five year old female had an isolated abducens nerve palsy following closed head injury. There was no associated skull fracture, haematoma, or other cranial nerve injury. The significance, frequency, and differential diagnosis of traumatic sixth cranial nerve injury is discussed, particularly in paediatric patients. Management is symptomatic; occlusion with an eye pad may be used if diplopia is significant. In young children alternate day occlusion of each eye will help prevent amblyopia. Most cases improve within three months and many resolve by six months. Residual palsy at six months is li

Closed head injury and the treatment of sequelae after a motor vehicle accident.

Approximately 2 million closed head injuries (CHIs) occur yearly in the United States. Twenty-five percent of these injuries require hospitalization, and 70,000 to 90,000 of those hospitalized suffer long-term disability. This case conference details one such case of CHI in which the patient ultimately died. Close attention is given to the pathophysiology and treatment of this process. Commonly accepted, as well as investigational, modalities of therapy are discussed.

Somatosensory evoked potential monitoring following severe closed head injury.

This paper describes the results of somatosensory evoked potential (SSEP) monitoring in 65 patients with severe head injury. Intracranial pressure (ICP) monitoring data were available for 63 patients, and arterial-jugular oxygen content (AVDO2) data for 52 patients. Eighty-nine percent of patients with no SSEP activity beyond 50 msec post-stimulus in either hemisphere died or were vegetative survivors (3 month Glasgow Outcome Score). All 17 patients with a good or moderate outcome had long latency cortical activity (i.e. > or = 70 msec post-stimulus) in both hemispheres. Among patients with ab

Blinded comparison of cranial CT and MR in closed head injury evaluation.

PURPOSE: To compare CT and MR in the evaluation of acute head injury. METHODS: One hundred seven consecutive patients who were referred to the emergency department and underwent both MR and CT cranial examinations within 48 hours were retrospectively reviewed. The films were interpreted by two neuroradiologists blinded to all patient information. RESULTS: The sensitivity of MR was significantly higher than that of CT for the detection of contusion, shearing injury, subdural and epidural hematoma, and sinus involvement. The sensitivity of CT was significantly higher than that of MR for the ev

Transcranial Doppler ultrasound following closed head injury: vasospasm or vasoparalysis?

Nine patients suffering closed head injury were studied using transcranial Doppler ultrasound recording of the basal cerebral arteries. On admission, six patients had Glasgow Coma Scores of 7 or less, while three had initial scores of 8-10. Eight of the nine patients (82%) developed abnormally high mean velocities (greater than or equal to 110 cm/s) in one or more vessels. The onset, duration, and amplitude of the altered flow velocities were somewhat different from those that are seen with vasospasm after aneurysmal subarachnoid hemorrhage, which tend to occur later. Cerebral angiography in o

Memory function after closed head injury: a review of the quantitative research.

Studies which provide quantitative analyses of memory function following closed head injury were reviewed. Specific issues covered include criteria for assessing post-traumatic amnesia (PTA), PTA duration, relation of PTA duration to later memory impairment, memory functions within PTA, and comparisons between PTA and other amnesic conditions. Issues associated with the assessment of retrograde amnesia were also reviewed. With regard to memory function after PTA has terminated, we described and discussed normative data, the time course of recovery, and issues related to the nature of the later

The effects of the endotracheal tube suctioning/manual hyperventilation procedure on patients with severe closed head injuries.

The findings of this clinical study suggest that the ETTS/MH procedure could be safely performed upon patients with severe closed head injuries whose baseline measurements of MICP were within the range of 0 to 20 mm Hg, provided the CPP was maintained at 50 mm Hg or greater. The advantages of performing the ETTS/MH procedure upon intubated patients far outweigh the possible disadvantages. The removal of pulmonary mucus plugs and secretions, which subsequently prevents hypercarbia and hypoxemia, is very important to the patient's recovery. In most situations, stimulation of the cough reflex thr

A Systematic Review of Closed Head Injury Models of Mild Traumatic Brain Injury in Mice and Rats.

Mild TBI (mTBI) is a significant health concern. Animal models of mTBI are essential for understanding mechanisms, and pathological outcomes, as well as to test therapeutic interventions. A variety of closed head models of mTBI that incorporate different aspects (i.e., biomechanics) of the mTBI have been reported. The aim of the current review was to compile a comprehensive list of the closed head mTBI rodent models, along with the common data elements, and outcomes, with the goal to summarize the current state of the field. Publications were identified from a search of PubMed and Web of Scien

[A case of closed head injury with diffuse axonal injury, and oculomotor nerve avulsion and midbrain infarction].

We report an autopsy case of 66-year-old woman with closed head injury due to a fall from stairs. She had recovered after a half day period of coma following the fall. Severe right peripheral oculomotor nerve palsy was noted from the beginning. MRI demonstrated lesions in the corpus callosum and right midbrain. She had died of suffocation due to sputum impaction after 50 hours after the fall. Autopsy examination revealed traumatic subarachnoidal hemorrhage, partial tearing and petechial hemorrhage of the corpus callosum, and many axonal retraction balls scattered in the cerebral white matter.

Early tracheostomy in closed head injuries: experience at a tertiary center in a developing country--a prospective study.

BACKGROUND: An important factor contributing to the high mortality in patients with severe head trauma is cerebral hypoxia. The mechanical ventilation helps both by reduction in the intracranial pressure and hypoxia. Ventilatory support is also required in these patients because of patient's inability to protect the airway, persistence of excessive secretions, and inadequacy of spontaneous ventilation. Prolonged endotracheal intubation is however associated with trauma to the larynx, trachea, and patient discomfort in addition to requirement of sedatives. Tracheostomy has been found to play an

Neurologic impairment following closed head injury predicts post-traumatic neurogenesis.

In the mammalian hippocampus, neurogenesis persists into adulthood, and increased generation of newborn neurons could be of clinical benefit following concussive head injuries. Post-traumatic neurogenesis has been well documented using "open" traumatic brain injury (TBI) models in rodents; however, human TBI most commonly involves closed head injury. Here we used a closed head injury (CHI) model to examine post-traumatic hippocampal neurogenesis in mice. All mice were subjected to the same CHI protocol, and a gross-motor based injury severity score was used to characterize neurologic impairmen

[Cerebral vein thrombosis secondary to closed head injury. Report of one case].

Intracranial sinus thrombosis (1ST) after closed head injury is an uncommon but potentially serious complication. It has no correlation with the severity of the injury. The symptoms and clinical course are highly variable. The most frequent but least specific symptom is severe headache. Cerebral lesions and neurologic signs develop in half of patients with IST. We report a 29 year-old male who had an IST after a severe closed head injury. The patient initially developed headache and had later 2 secondarily generalized seizures. The magnetic resonance imaging showed a superior sagittal sinus th

Return of memory and sleep efficiency following moderate to severe closed head injury.

OBJECTIVE: Sleep disturbance is common in the subacute recovery phase following brain injury. A previous study from the authors' group found 68% of patients with closed head injury (CHI) had disrupted sleep on a rehabilitation unit. In the present study, the authors investigated whether improvement in sleep efficiency correlates with duration of posttraumatic amnesia (PTA) after CHI. METHODS: Fourteen CHI patients were enrolled and followed prospectively. Mechanism of injury included motor vehicle accident, fall, and blunt assault. An actigraph was placed on each subject's wrist within 72 hou
