Young rural people at risk for schizophrenia: time for mental health services to translate research evidence into best practice of care.

Early intervention into prodromal schizophrenia has shown promise, but controversy continues regarding the ethical acceptability of identifying a group of 'ultra high risk' individuals of whom only 30 to 50% will develop a psychotic disorder. With well developed early intervention services this group faces the possibility of being labelled as 'pre-psychotic', a condition for which the well known stigma associated with the diagnosis of schizophrenia or bipolar disorder is likely to be associated. In addition, the use of potent antipsychotic and other medications (albeit usually at lower doses t

Brain injury, mental health and substance use in homeless populations: community-generated recommendations for healthcare service delivery and research.

BACKGROUND: The prevalence of acquired brain injury (ABI) and mental health/substance use (MHSU) disorders is high amongst people experiencing homelessness, yet guidance for addressing these complex comorbidities is lacking. Therefore, the objective of this study was to engage community-based stakeholders in a health priority-setting process to generate, identify and prioritize recommendations for clinical practice and research to improve healthcare services for individuals with concurrent ABI-MHSU who are experiencing homelessness. METHODS: Data were collected during a one-day workshop as pa

Diagnostic delay in soft tissue tumors: a single-center study of a university cancer center with a focus on health services research.

BACKGROUND: Lumps and soft tissue tumors (STT) are frequent reasons for consulting a physician. Most STT are benign, and lumps are not always associated with a tumor. MRI is the most advanced imaging modality to assist a provisional diagnosis of STT. Only a small fraction of STT is malignant, these soft tissue sarcomas are known for their aggressive growth. The study aims to analyze the influence of the MRI report on the speed of treatment of patients with suspected STT. METHODS: This was a retrospective, longitudinal, single-center study from 2011-2020. We included adult patients who had bio

[The infectious disease epidemiology research network--a bridge between science and public health service. The example of the network for foodborne infections].

In order to respond to emerging food borne diseases, a new data-driven approach guided by epidemiologic and microbiologic data has been developed in Germany. An interdisciplinary network of scientists at the national, regional and local levels conducted surveillance and epidemiologic research of foodborne pathogens (Shiga toxin-producing Escherichia coli (STEC), Salmonella and Campylobacter). The objectives of the network include (1) determining incidence and distribution of STEC and Salmonella, (2) assessing the importance of typing systems for Campylobacter, (3) determining risk factors for

Embedding Service User Experience ('Experts by Experience') Into Undergraduate Mental Health Nursing Education: A Co-Production Research Project.

Internationally, the need to have service user involvement (the 'voice' of recovery journeys) as an established and significant feature on the landscape of professional development has been widely discussed in the area of mental health nursing (MHN) education for over a decade. Service user involvement contributes to a different understanding, bringing 'new' ways of knowing in nursing education and potentially new ways of practicing within mental health services. The objective of this co-produced research was to investigate the current local 'state of play' of service user involvement in MHN s

EnDPoINT: protocol for an implementation research study to integrate a holistic package of physical health, mental health and psychosocial care for podoconiosis, lymphatic filariasis and leprosy into routine health services in Ethiopia.

INTRODUCTION: Neglected tropical diseases (NTDs) causing lower limb lymphoedema such as podoconiosis, lymphatic filariasis (LF) and leprosy are common in Ethiopia. Routine health services for morbidity management and disability prevention (MMDP) of lymphoedema caused by these conditions are still lacking, even though it imposes a huge burden on affected individuals and their communities in terms of physical and mental health, and psychosocial and economic outcomes. This calls for an integrated, holistic approach to MMDP across these three diseases. METHODS AND ANALYSIS: The 'Excellence in Dis

Facilitating transition of young people with long-term health conditions from children's to adults' healthcare services - implications of a 5-year research programme.

BACKGROUND: During transition from children's to adults' healthcare, young adults with long-term conditions may show delays in psychosocial development compared to their peers without long-term conditions, and deterioration of their conditions' medical control. METHODS: This paper integrates the findings, already published in 10 separate papers, of a 5-year transition research programme. IMPLICATIONS: There is an important role for funders (commissioners) of adults' services to fund transitional healthcare, in addition to funders of children's services who currently take responsibility. It is

Road map to scaling-up: translating operations research study's results into actions for expanding medical abortion services in rural health facilities in Nepal.

BACKGROUND: Identifying unsafe abortion among the major causes of maternal deaths and respecting the rights to health of women, in 2002, the Nepali parliament liberalized abortion up to 12 weeks of pregnancy on request. However, enhancing women's awareness on and access to safe and legal abortion services, particularly in rural areas, remains a challenge in Nepal despite a decade of the initiation of safe abortion services. METHODS: Between January 2011 and December 2012, an operations research study was carried out using quasi-experimental design to determine the effectiveness of engaging fe

Colorectal cancer and country of birth in New South Wales, Australia: All-of-population data for prioritising health service delivery and research.

INTRODUCTION: Cancer care and outcomes differ across cultural groups in Australia. Quantifying these differences facilitates prioritisation and targeting of services and research. All-of-population data are needed by health agencies to understand and fulfil their cancer-control responsibilities. Compiling these data can be challenging while maintaining privacy. We have used data linkage to gain population-wide colorectal cancer data on stage (degree of spread), treatment, and survival in New South Wales (NSW), Australia, by country of birth (COB), and consider service implications. Laboratory challenges in the scaling up of HIV, TB, and malaria programs: The interaction of health and laboratory systems, clinical research, and service delivery.

Strengthening national health laboratory systems in resource-poor countries is critical to meeting the United Nations Millennium Development Goals. Despite strong commitment from the international community to fight major infectious diseases, weak laboratory infrastructure remains a huge rate-limiting step. Some major challenges facing laboratory systems in resource-poor settings include dilapidated infrastructure; lack of human capacity, laboratory policies, and strategic plans; and limited synergies between clinical and research laboratories. Together, these factors compromise the quality of

How, why, for whom, and in what contexts organisational and team dynamics shape, and are shaped by, service user researchers in higher education mental health research centres: a realist review protocol.

BACKGROUND: Service user researchers (SURs) are increasingly recognised as integral contributors within academic mental health research centres. However, the ways in which these roles are integrated and supported vary widely across academic contexts and are strongly shaped by organisational structures, team cultures and role clarity. While some SURs can contribute meaningfully and thrive, others experience challenges such as emotional labour and limited structural support. There remains a lack of theory-driven understanding of how organisational and team dynamics shape, and are shaped by, the

Barriers and facilitators to implementing social prescribing in child and adolescent mental health services: a qualitative analysis using the consolidated framework for implementation research.

INTRODUCTION: Interventions are urgently needed for young people waiting for Child and Adolescent Mental Health Services (CAMHS) in England. Long waits can worsen mental health, increase distress for young people and families, and place additional pressure on already stretched services. Social prescribing, a referral system for connecting individuals with resources in the community via one-to-one support from a link worker, has not been routinely implemented or evaluated for young people on CAMHS waitlists. It remains unclear whether, and under what conditions, social prescribing can be implem

Using operational research as a tool to improve eye health services and systems in low-and middle-income settings: lessons from India and Nepal.

BACKGROUND: Operational Research (OR), as part of a quality assurance program, has become a standard feature of most health institutions in most high-income countries. In contrast, in low-income settings, operational research is less common, and almost no one has asssed operational research capacity building (ORCB) as a tool to improve efficacy, efficiency and quality in these settings. This study evaluated the impact of an ORCB program on participants' research competencies and the extent to which research findings were implemented in practice. MATERIALS AND METHODS: This study combined quan

Do we all agree what "good health care" looks like? Views from those who are "seldom heard" in health research, policy and service improvement.

CONTEXT: The aim of this study was to ask whether there are shared ideas about what good health care looks like that apply across different populations and conditions. Do priorities among "seldom heard" groups differ from mainstream views and, if so, how might we understand these differences? DESIGN: Focus groups were recruited with the help of our study patient representatives. Participants discussed and prioritized a set of eight "core components" of good care. We recorded and transcribed the data for thematic analysis. SETTING AND PARTICIPANTS: We recruited people who are seldom heard in
