This study discusses the key microvascular decompression(MVD)techniques for the treatment of hemifacial spasm(HFS). The author's experience is based on their institution's cases, highlighting three critical techniques. (1)Vertebral artery(VA)repositioning: Repositioning the VA is essential in challenging cases. This article focuses on the relocation of the proximal portion of the VA and the importance of careful dissection. (2)Relocating vessels compressing the peripheral branches of the facial nerve: HFS can result from nerve compression at various locations, including the cisternal portion. …

BACKGROUND AND OBJECTIVES: Hemifacial spasm (HFS) caused by venous compression is a rare occurrence. Currently, there is no relevant research on the venous characteristics and potential pathogenic mechanisms causing venous HFS. Exploring the venous characteristics in venous-type HFS may reduce the likelihood of repeated surgery.
METHODS: The authors presented 4 cases of HFS caused by veins of middle cerebellar peduncle (V. of Mid.Cer.Ped) successfully treated with microvascular decompression.
RESULTS: Based on intraoperative observations and abnormal muscle response (AMR) monitoring, it was …

Brainstem auditory evoked potential(BAEP)and abnormal muscle response(AMR)monitoring are widely used during microvascular decompression(MVD)for hemifacial spasm(HFS). In BAEP monitoring, the intraoperative findings of wave V do not necessarily predict postoperative hearing function. However, if a warning sign as significant as wave V change appears, the surgeon must abort the operation or inject artificial cerebrospinal fluid into the VIII nerve. During MVD for HFS, BAEP monitoring must be performed to preserve hearing function. AMR monitoring is useful in identifying the offending vessels com…

Hemifacial spasm (HFS) is a painless, involuntary twitching of the facial muscles that usually involves the orbicularis oculi muscle.1,2 It is commonly caused by the anterior inferior cerebellar artery or posterior inferior cerebellar artery.1,2 However, a dolichoectatic vertebrobasilar artery (VBA) can impinge the facial nerve.1 Macrovascular decompression with sling transposition is a common management paradigm.1-9 We present a case of a 56-year-old male who presented with left-sided HFS, pulsatile tinnitus, and hearing loss for the past 2 years. The patient underwent a retrosigmoid cranioto…

BACKGROUND: This is an update of a Cochrane Review, first published in 2005. Hemifacial spasm (HFS) is characterised by unilateral, involuntary contractions of the muscles innervated by the facial nerve. It is a chronic disorder, and spontaneous recovery is very rare. The two treatments routinely available are microvascular decompression and intramuscular injections with botulinum toxin type A (BtA).
OBJECTIVES: To compare the efficacy, safety, and tolerability of BtA versus placebo in people with HFS.
SEARCH METHODS: We searched CENTRAL, MEDLINE, Embase, reference lists of articles, and con…

The side predilections of various offending arteries in hemifacial spasm (HFS) have not been well studied. The relationship between clinical and radiological features of HFS and offending arteries were investigated in the present study. A retrospective analysis of 370 patients who underwent microvascular decompression for HFS was performed. The patients were divided into four groups based on the offending arteries, namely anterior inferior cerebellar artery (AICA), posterior inferior cerebellar artery (PICA), vertebral artery, and multiple offending arteries. Affected side, age at onset, prese…

Hemifacial spasm (HFS) or trigeminal neuralgia (TN) is a kind of hyperactivity disorder of cranial nerves caused by vascular compression. However, sometimes, the disease may arise from nerve damage produced by tumors, which was called as symptomatic HFS/TN. Until now, little is known about the exact mechanism and the necessity of microvascular decompression (MVD) regarding the tumor-induced HFS/TN, which is necessary to be retrospectively analyzed in a considerable sample. Among the 4021 patients who underwent MVD in our department between 2006 and 2014, 44 were finally diagnosed as symptomati…

BACKGROUND AND PURPOSE: Sensory tricks are sensory, often but not exclusively tactile, stimuli usually in the body part affected by the movement disorder that produce a meaningful alleviation of dystonia. The frequency and clinical features of sensory tricks in different types of dystonia are poorly studied in the literature. There is no information regarding the presence of a similar phenomenon in HFS.
METHODS: We enrolled consecutive patients who had the diagnosis of HFS (26) or the following types of dystonia: cervical dystonia (CD; 21); blepharospasm (BS; 20); and writer's cramp (WC; 10).

Intracranial endodermal cysts presenting with hemifacial spasm (HFS) are extremely rare. We report a right cerebellopontine angle (CPA) endodermal cyst in a 56-year-old man who presented with a 6-month history of right-sided hemifacial spasm. Computed tomography revealed a homogenous, well-demarcated, hyperdense lesion extending from prepontine cistern to right CPA. Magnetic resonance imaging demonstrated a right CPA extra-axial cystic lesion protruding into Meckel's cave, with compression of cranial nerves VII and VIII. Light brown, creamy cystic content was totally removed, and the thin cyst…

BACKGROUND: Hemifacial spasm is characterised by unilateral involuntary contractions of muscles innervated by the facial nerve. The usual cause is a vessel touching the facial nerve near its origin from the brain stem. Although it is a benign condition it can cause significant cosmetic and functional disability. It is a chronic disease and spontaneous recovery is very rare. The two treatments routinely available are microvascular decompression and Botulinum Toxin type A (BtA) muscular injections.
OBJECTIVES: To determine whether botulinum toxin (BtA) is an effective and safe treatment for hem…

In the last 30 years, the concept of microvascular compression has achieved widespread acceptance. Still, some remain unconvinced. A review of our 182 operations, 104 trigeminal neuralgia and 78 hemifacial spasms, shows that microvascular decompression of the trigeminal and facial nerves is effective in alleviating trigeminal neuralgia and hemifacial spasm, because it removes the actual cause of the disorder rather than simply causing local injury to the nerve. Our neurophysiological investigations, the so-called abnormal muscle response, performed intraoperatively in twenty patients with hemi…

A new surgical approach for the treatment of hemifacial spasm was carried out in 110 cases from 1991-1994. The novelty of this new procedure lies in splitting of facial nerve by making multiple cuts along the long axis of the nerve--descriptively a type of "combing". Among these cases, arachnoid thickening and adhesion occurred in 35 cases throughout the vascular course, 66 cases showed vascular compression (60%). A segment of vessel could be seen lying between VII and VIII cranial nerves in 16 cases. Under the latter circumstance steps taken for decompression would be either too risky or impo…

Hemifacial spasm is nearly always a unilateral disease of the facial musculature and is characterized by involuntary tonic or clonic cramps that considerably reduce the affected patient's quality of life. In the past, a number of different conservative and operative therapeutic procedures have been applied for the treatment of hemifacial spasm. In many cases these attempts failed to control the disease permanently or resulted in unwanted, sometimes strong, side effects. We report our own experiences with botulinum therapy in 29 patients with hemifacial spasm (78 therapeutic sessions). In our p…

In two patients, the skin over both hypothenar eminences underwent intermittent, spontaneous, irregular, dimpling contractions. The dimpling was benign, and was the result of spontaneous discharge of motor units in the palmaris brevis muscle. Electrophysiological investigations localized the site of origin of the discharge to the ulnar nerve, possibly at the wrist, but there was no clinical or physiological evidence of neuropathy or of nerve compression. In many respects, the clinical and electrophysiological features of hypothenar dimpling resemble hemifacial spasm.
