Considering the importance of haemorrhagic complications in gynaecology-obstetrics and the increasing fear of accidents during blood transfusion, the authors propose, from the literature and the study of 13 cases, a reflection on the indications of deferred autotransfusion (DAT) and of normovolemic haemodilution (NVH). Since 1977, eight children have had major liver resections with the aid of normovolemic hemodilution anesthesia. Five children were 7 months of age or less; the other three were 3 1/2, 8, and 12 years old. This includes three operations for hepatoblastoma, three for hepatoma, and two for mesenchymal hamartoma. The effect of hemodilution on plasma levels of total cortisol and free cortisol was studied in 12 patients undergoing open-heart surgery. The authors report on 52 cases of acute normovolemic hemodilution carried out in the course of surgical operations planned in advance in the period from February to September, 1975. In this connection questions of method, indication and contro-indication as well as juridical and economic aspects are dealt with discussion. The advantages of acute normavolemic hemodilution by transfusing the patient's own blood taken from him some days before the operation are particularly stressed.
