Non-transferrin plasma iron in beta-thalassaemia/Hb E and haemoglobin H diseases.

Non-transferrin plasma iron concentrations were determined in 45 normal controls and in 37 patients with Hb H disease and 104 patients with beta-thalassaemia/Hb E disease. This revealed that non-transferrin plasma iron exists in cases with severe iron overload, more striking in beta-thalassaemia/Hb E than in Hb H disease. Non-transferrin plasma iron is associated with higher transferrin iron saturation and higher plasma ferritin levels. The most striking finding was the significantly higher non-transferrin plasma iron in splenectomized patients with beta-thalassaemia/Hb E disease than in the n

Muscle oxygen utilization and ventilatory parameters during exercise in people with cystic fibrosis: Role of HbA1c.

INTRODUCTION: Glycated hemoglobin can interfere with oxygen delivery and CO2 removal during exercise. Additionally, pancreatic insufficiency increases oxidative stress and exacerbates exercise intolerance in people with cystic fibrosis (PwCF). This investigation sought to test the hypotheses that elevated Hemoglobin A1c (HbA1c) can negatively affect exercise parameters in PwCF and that reductions in oxidative stress can improve tissue oxygenation in individuals with elevated HbA1c. METHODS: Twenty four PwCF were divided into two groups; normal HbA1c <5.7% (N-HbA1c) and elevated HbA1c >5.7% (E

Hemoglobin Lepore EF Bart's disease: a molecular, hematological, and diagnostic aspects.

We report the molecular basis and hematological phenotype associated with a hitherto undescribed interaction of hemoglobin (Hb) Lepore-Hollandia, Hb E and a deletion of three α-globin genes found in a 3-year-old Thai girl. She had mild anemia with Hb 10.9 g/dl and Hct 35.9% and had never received blood transfusion. Hb analysis revealed Hb E (22.1%) with a normal level of Hb A(2) (1.9%), unusually elevated Hb F (65.9%), Hb Lepore (4.0%), and 5.4% Hb Bart's. Globin gene analyses demonstrated that she carried the Hb Lepore-Hollandia mutation in trans to the Hb E and a compound heterozygosity for

Effects of thalassaemic serum on the in vitro development of the malarial parasite Plasmodium falciparum.

Sera from thalassaemic individuals were experimentally tested for their suppressive effects on the in vitro development of asexual stages of Plasmodium falciparum. Cultures in sera collected from six patients who had classical haemoglobin H disease (alpha-thal1-alpha-thal2) and six patients who had haemoglobin H disease with haemoglobin (Hb) Constant Spring (alpha-thal1-CS) showed a significantly higher degree of inhibition of parasite multiplication than cultures of thalassaemic erythrocytes in sera from healthy individuals. Similarly, sera from 12 patients with beta-thalassaemia/haemoglobin

The clinical significance of hemoglobinopathies in the Hamilton region: a twenty-year review.

Between 1970-1990, the Laboratory tested 38,391 specimens for hemoglobinopathies, of which 7,935 were positive. The major abnormalities detected were beta thalassemia trait (4,688), alpha thalassemia trait (1,248) and sickle cell trait (847). Clinically significant hemoglobinopathies detected were Hemoglobin H disease (100), sickle cell disease (67) and sickle cell Hemoglobin C disease (79). Hemoglobinopathies are therefore common in the Hamilton area as a reflection of the cultural diversity of area citizens. Of the 49 patients with thalassemia without documented iron deficiency, 8 (16%) rece

Prenatal diagnosis of thalassemia and hemoglobinopathies in Thailand: experience from 100 pregnancies.

In this review, we describe a simple strategy to detect the three severe thalassemic diseases commonly found in Thailand. Hb Bart's hydrops fetalis can be detected unambiguously by ultrasonography at 18-20 weeks of gestation or detected early in the first trimester by the gene amplification technique. Prenatal diagnosis for homozygous beta-thalassemia is better performed in the second trimester by in vitro protein synthesis. This is because the molecular defects of some beta-thalassemias are still unknown and homozygosity of the same mutation is low. In contrast, beta-thalassemia/Hb E is easil

Clinical phenotypes and genotypes diagnosis of thalassemia in children.

Thalassemia is a relatively common hemolytic anemia in Southeast Asia. Alpha and beta thalassemia, hemoglobin (Hb) E, and Hb Constant Spring (CS) are prevalent in Thailand. Different gene combinations lead to over 60 thalassemic syndromes. One hundred and forty-nine thalassemia families were retrospectively studied. They were 4 homozygous beta-thalassemia (beta-thal/ beta-thal), 79 beta-thal/Hb E, 22 Hb H disease, 32 Hb with Hb CS, and 6 AE Bart's disease. The first clinical manifestation and hematologic data including hemoglobin electrophoresis were analysed. Most homozygous beta-thalassemia …

Effect of canagliflozin on left ventricular diastolic function in patients with type 2 diabetes.

BACKGROUND: Type 2 diabetes mellitus (T2DM) greatly increases the risks of cardiovascular disease and heart failure. In particular, left ventricular diastolic dysfunction that develops from the early stages of T2DM is an important factor in the onset and exacerbation of heart failure. The effect of sodium-glucose cotransporter 2 inhibitors on left ventricular diastolic function has not been elucidated. We have performed the first prospective study on the effects of canagliflozin on left ventricular diastolic function in T2DM. METHODS: This study was performed to evaluate the effects of additi

The Healthy Smoker Paradox: Socioeconomic status as a fundamental cause of reversed anemia risk among Yemeni youth.

BACKGROUND: While tobacco smoking biologically elevates hemoglobin through chronic hypoxia, this study investigates an unexpected paradoxical reversal in conflict-affected Yemen, testing whether socioeconomic status may override biological pathways in extreme resource-limited settings. METHODS: We conducted a multi-center cross-sectional study of 600 Yemeni university students (aged 18-25). Data on smoking, socioeconomic proxies, and hematological parameters were collected. We employed multivariate logistic regression, causal mediation analysis with bootstrapping, and E-value sensitivity anal

Utilization of monoclonal-antibody-based assay (HemoCard in screening for and differentiating between genotypes of sickle cell disease and other hemoglobinopathies.

Sickle cell disease covers a group of conditions in which pathology may be attributed to the presence of sickle hemoglobin (HbS). The identification of HbS and other variants including those in combination with HbS is commonly achieved by cellulose acetate electrophoresis at alkaline pH. Because many hemoglobin variants with similar charges have similar electrophoretic migration patterns, they are difficult to differentiate by electrophoresis. The HemoCard assays address this concern through the use of monoclonal antibodies capable of specifically recognizing the unique amino acid substitution

Does vitamin E have a role in treatment and prevention of anemia?

Vitamin E is a highly effective fat-soluble vitamin with a variety of cellular membrane stabilizing-antioxidant and non-antioxidant functions. Vitamin E has been suggested to prevent the oxidation of polyunsaturated fatty acids in red blood cell (RBC) membrane, thus inhibiting the premature erythrocytelysis. Animal studies have shown that treatment with vitamin E results in increased number of colony forming units of erythroid precursors, enhanced erythropoiesis and improved blood hemoglobin levels in these animals. Several clinical trials have indicated that vitamin E might be used therapeuti

Iron absorption in patients with beta-thalassaemia/haemoglobin E disease and the effect of splenectomy.

Intestinal 59Fe absorption was measured in 16 patients with beta-thalassaemia/haemoglobin E disease and in 5 normal controls, using a total-body counting technique. The average iron absorption in the patients was 62% in contrast to 16.5% in the normal controls. 6 of the 9 splenectomized patients had absorption values above 65%, while only 2 of the 7 nonsplenectomized patients had comparably high values.

Heterogeneity of beta thalassaemia in Thailand.

Beta thalassaemia in Thailand is heterogeneous. Clinical, genetical, haematological and globin chain biosynthetic studies were performed in seven beta-thalassaemia families. The results showed different gene combinations. These were alpha-thalassaemia/homozygous beta 0-thalassaemia, questionable double heterozygosity between a beta-thalassaemia and a silent beta-thalassaemia genes with low Hb F of unexplained cause, silent beta-thalassaemia/beta+ - or beta 0-thalassaemia, high Hb A2 high Hb F-beta-thalassaemia - a new mutant, mild beta+-thalassaemia/Hb E, and beta 0-thalassaemia/heterocellular

Application of high-performance liquid chromatographic methodology to the analysis of hemoglobins synthesized in erythroid progenitor cells.

High-performance liquid chromatography (HPLC) has been successfully used in the quantitation of the relatively minute amounts of hemoglobin types recovered from in vitro cultures of hemoglobin-synthesizing erythroid progenitor (BFU-E) cells. This reversed-phase HPLC method uses the Vydac C4 column and water-acetonitrile-trifluoroacetic acid as mobile phases; it has been applied to the study of fetal hemoglobin synthesis patterns in ten homozygous sickle cell anemia patients and a similar number of their heterozygous relatives along with a few normal control subjects. A significant increase in
