We treated three patients with herpes simplex dendritic keratitis that occurred between three and 11 months after keratoplasty. The patients had no history of herpetic infection. The eyes of two of the patients were grafted for corneal scarring of undetermined origin. The eye of the third patient was grafted for pseudophakic bullous keratopathy. From 170 pregnant women, cervico-vaginal secretion, serum and amnion fluid were collected to determine concentrations of herpes simplex virus (HSV) IgA in different risk groups. We found a higher concentration of HSV IgA in cervicovaginal secretions in women with positive cultures, compared with women with negative cultures. Herpes simplex oesophagitis is usually associated with debilitated, traumatized or immunologically-compromised hosts. We report here two cases of severe self-limiting oesophagitis in immunologically competent young children that were caused by herpes simplex type 1. This diagnosis should be considered in the differential diagnosis of acute severe pain on swallowing in children for whom oral and pharyngeal pathology have been excluded.

The management of a patient with ocular HSV is a challenge for the ophthalmologist. Attacks may vary in clinical presentation with each episode. Patients should be instructed to contact their ophthalmologist at the first sign of a problem. Because a recurrent attack may be painless, patients may be reluctant to seek medical attention. Therefore, I give patients three easy warning signs to remember. They are instructed to contact me if the eye becomes red or painful or the vision decreases. Those patients with a history of ocular HSV who will undergo immuno-suppression (e.g., renal transplant)

We have reported a case of transplacentally transmitted herpes simplex virus (HSV) infection in association with both congenital malformations and other serious abnormalities, including facial abnormalities, microcephaly, cerebral atrophy, and microscopic cranial calcifications. Before death, the infant showed marked neurologic deficits, seizures, and respiratory distress. Serum IgM and complement fixing antibodies to HSV were elevated at birth. Light and electron microscopy and immunofluorescence studies confirmed the presence of the virus.

A 37-year-old man developed an acute follicular conjunctivitis with preauricular lymphadenopathy believed to be epidemic keratoconjunctivitis. We conducted a retrospective longitudinal cohort study of 82 penetrating keratoplasties performed to treat herpes simplex keratitis. In 53 cases (65%) there was extensive corneal neovascularization or stromal ulceration and active intraocular inflammation, making the prognosis in these cases poor. Superficial epidermal surgical removal of a recurrent herpes simplex lesion within 36 hours of onset nearly always prevented recurrence in the site of excision. It is postulated that the surgery results in epidermal denervation, which prevents reinoculation of that epidermal site by the virus-laden sensory neuron. The procedure did not prevent further clinical expression of the disease at other epidermal sites innervated by the same or other infected neurons.

A case of herpes simplex virus encephalitis is described in a patient known to suffer from chronic atrial fibrillation and whose clinical symptoms suggested cerebral embolism of cardiac origin.
