[Hiccups in adults: Pathophysiology, diagnostic approach, and management].

Chronic hiccups, defined as lasting more than 48hours, represent an underestimated clinical entity with potentially severe impacts on patients' quality of life. The pathophysiology involves a reflex arc comprising the phrenic and vagus nerves, integrated at the pontomedullary level. Recent advances highlight the role of a central pattern generator and neurotransmitter systems (GABA, dopamine, serotonin) in the genesis and perpetuation of hiccups. Epidemiological data reveal a striking male predominance (82-92% of cases), with an organic cause identified in over 90% of men. Etiologies are domin

Tuberculous Meningitis With Persistent Hiccups: A Case Report.

A 69-year-old Japanese male presented with atypical symptoms of tuberculous meningitis (TBM), including intermittent left temporal headache and persistent hiccups that were unresponsive to initial therapies. Despite comprehensive imaging, no definitive intracranial lesions were identified. His persistent hiccups, along with typical meningitis symptoms, highlighted the diagnostic challenges of TBM, particularly in the context of negative microbiological tests. Early empirical treatment, initiated based on clinical suspicion and elevated cerebrospinal fluid (CSF) adenosine deaminase levels, led

Be Wary of Hiccups: An Unusual Case of COVID-19.

Hiccups, involuntary contraction of the diaphragm and intercostal muscle followed by an abrupt closure of the glottis, are a bothersome symptom that can be caused by a variety of illnesses or medications. Hiccups that persist for more than 48 hours should raise the suspicion of an underlying cause. Pneumonias, especially caused by the novel coronavirus, have rarely been reported to trigger hiccups. To the best of our knowledge, we present the first case in sub-Saharan Africa of a patient presenting to our institution with persistent hiccups and no other objective signs suggestive of underlying

Tramadol-Induced Hiccups: A Report of Two Cases.

Hiccups can be idiopathic, psychogenic and organic, with drugs being one of the most important causes of hiccups. Although the exact pathophysiological processes involved are still poorly understood, the neurotransmitters dopamine, serotonin, and γ-aminobutyric acid (GABA) have been documented to play a significant role in the generation of hiccups. We report on two patients with cellulitis who developed hiccups with the use of tramadol as an analgesic. The possible mechanisms and clinical implications of this rare adverse event are discussed. Both patients recovered from the hiccups with the

Donepezil-related intractable hiccups: a case report.

This case report describes a man with intractable hiccups probably caused by donepezil. The patient's symptoms were not responsive to commonly used medications for hiccups, but they were improved and completely relieved upon donepezil dose deescalation and discontinuation. We report two occasions in which the discontinuation of donepezil resulted in hiccup resolution and three occasions in which initiation of donepezil was associated with the onset of hiccups. This report contributes to the growing body of literature that describes an association between centrally acting medications and intrac

Management of intractable hiccups: an illustrative case and review.

Often thought of as a benign and self-limited condition, hiccups can become persistent or intractable, and thus be associated with substantial morbidity and distress. In such cases, an underlying etiology is often present, and may be overlooked. Debilitating hiccups can present a major challenge to optimal symptom management. Various causes of protracted hiccups have been identified including metabolic abnormalities, central nervous system pathology, malignancy, medications, and disorders attributed to cardiac, pulmonary and gastrointestinal etiologies. We present a case of intractable hiccups

A solitary fibrous tumor of the pleura revealed by hiccups.

Solitary fibrous tumors of the pleura are rare and benign primary localized tumors; they possess a malignant potential and thus should be excised. We report a case of a 43-year-old woman, who had suffered for 5 years from right basithoracic pain associated with progressive dyspnea and persistent hiccups during the last 6 months. We have not found any similar case in the literature. Further testing after excision by thoracotomy revealed a solitary fibrous pleural tumor. A brief discussion of the clinical presentation and incidence of these tumors is included.

Diagnosis and management of hiccups in the patient with advanced cancer.

Benign, self-limited hiccups are more of a nuisance, but persistent and intractable hiccups lasting more than 48 hours and 1 month, respectively, are a source of significant morbidity in the patient with advanced malignancy. The hiccup reflex is complex, but stimulation of vagal afferents followed by activation of efferent phrenic and intercostal nerve pathways results in contraction of the diaphragm and intercostal muscles, respectively. The etiology of hiccups in the cancer and palliative care population may include chemotherapy, electrolyte derangements, esophagitis, and neoplastic involvemen

Hiccups: an unusual manifestation of an abdominal aortic aneurysm.

A patient with hiccups was found to have an abdominal aortic aneurysm that subsequently ruptured. We believe that a leaking abdominal aortic aneurysm led to an ileus-induced distention of the splenic flexure of the colon with consequent diaphragmatic irritation and phrenic nerve stimulation. This led to persistent hiccups as a result of repetitive stimulation of the reflex arc mediating hiccups. Persistent hiccups require investigation for an underlying organic etiology, and a leaking abdominal aortic aneurysm should be included in the differential diagnosis.

Hiccup-like Contractions in Mechanically Ventilated Patients: Individualized Treatment Guided by Transpulmonary Pressure.

Hiccups-like contractions, including hiccups, respiratory myoclonus, and diaphragmatic tremor, refer to involuntary, spasmodic, and inspiratory muscle contractions. They have been repeatedly described in mechanically ventilated patients, especially those with central nervous damage. Nevertheless, their effects on patient-ventilator interaction are largely unknown, and even more overlooked is their contribution to lung and diaphragm injury. We describe, for the first time, how the management of hiccup-like contractions was individualized based on esophageal and transpulmonary pressure measureme

A case of sinus arrest and post-hiccup cough syncope in medullary infarction.

We describe asymptomatic sinus arrest and post-hiccup cough syncope in a patient with medullary infarction. A 78-year-old woman developed arrhythmia, hiccup, and cough syncope attacks. Neurological examination was not remarkable. Cough syncope occurs after hiccup attacks. Bradycardia and decreased blood pressure were also present after the beginning cough. Holter 24-hour electrocardiography monitor exhibited 65 episodes of asymptomatic sinus arrest more than 3 seconds. Magnetic resonance imaging disclosed acute infarction in the bilateral medial regions and the right tegmentum of the upper and

Intractable hiccups as initial manifestation of achalasia: a case report.

BACKGROUND: Intractable hiccups pose a clinical challenge due to their diverse etiologies and lack of standard treatment. While associated with esophageal conditions, hiccups rarely occur in achalasia. CASE REPORT: A 65-year-old man presented with intractable hiccups, heartburn, and regurgitation for 7 years and progressive dysphagia and weight loss for 2 years. He was unrelieved by Nissen fundoplication for suspected gastroesophageal reflux disease (GERD), performed with no previous assessment of esophageal motility. Two years later, high-resolution impedance manometry (HRIM) revealed type I
