Outcomes of 72-Hour Hospital Treatment Responsibility Post Discharge to Municipal Nursing Care.

BACKGROUND: The increasing prevalence of individuals living with chronic conditions and multi-morbidity presents challenges to healthcare systems and coordination of coherent patient pathways. In Denmark, transitions between healthcare sectors are susceptible to disruption due to inadequate cross-sectoral collaboration. To improve continuity of care and cross-sectoral collaboration, an agreement of 72-hour hospital treatment responsibility post-discharge was implemented. METHODS: We used data from a questionnaire survey for healthcare professionals, data from electronic patient records, and d

Critical care readiness in conflict and disaster-affected Somalia: mass-casualty response, emergency referral, and continuity of life-saving care special collection: critical care in war and disaster areas.

Critical care readiness during war and disasters is often judged by intensive care unit capacity, yet preventable mortality in fragile health systems may occur before, during, or after ICU-level care. In Somalia, recurrent mass-casualty events, emergency referral constraints, limited oxygen and blood readiness, constrained operative access, and weak post-resuscitation monitoring expose a critical gap between first contact and sustained life-saving care. This Comment addresses that gap by reframing critical care readiness in conflict and disaster-affected Somalia as a pathway problem rather tha

Outbreak of listeriosis associated with consumption of deli meats in a hospital, Germany, February to March 2023.

Listeria monocytogenes can cause severe illness in individuals with weakened immune systems. In March 2023, L. monocytogenes was isolated from blood (n = 2) or pleural fluid (n = 1) of three febrile patients receiving synthetic glucocorticoids in a tertiary hospital in Germany. Food supply records suggested sliced parboiled sausage as the likely source, and L. monocytogenes was isolated from four samples of sealed packaged sliced sausages and ham from one manufacturer. The patient and food isolates clustered within 0-4 allelic differences. Counts of L. monocytogenes in all four food samples we

Safety Culture and Perioperative Quality at the Volta River Authority Hospital in Akosombo, Ghana.

BACKGROUND: The Volta River Authority Hospital (VRAH) is a district hospital associated with a large public works project in Akosombo, Ghana, that has developed a reputation for high-quality care. We hypothesized that this stems from a culture of safety and standardized processes typical of high-risk engineering environments. To investigate this, we evaluated staff and patient perceptions of safety and quality, as well as perioperative process variability. MATERIALS AND METHODS: The Safety Attitudes Questionnaire (SAQ) and Hospital Consumer Assessment of Healthcare Providers and Systems (HCAH

Comparison of influenza surveillance systems in Australia, China, Malaysia and expert recommendations for influenza control.

BACKGROUND: The Western Pacific Region (WPR) is exposed each year to seasonal influenza and is often the source of new influenza virus variants and novel pathogen emergence. National influenza surveillance systems play a critical role in detecting emerging viruses, monitoring influenza epidemics, improving public disease awareness and promoting pandemic preparedness, but vary widely across WPR countries. The aim of this study is to improve existing influenza surveillance systems by systematically comparing selected WPR influenza surveillance systems. METHODS: Three national influenza surveill

Barriers and facilitators to implementation of enhanced recovery after surgery protocols in resource-constrained hospitals: A mixed-methods study from a semi-urban government teaching hospital.

INTRODUCTION: Enhanced recovery after surgery pathways improve postoperative outcomes, yet consistent implementation remains challenging, particularly in resource-constrained settings. Previous observations identified variability in adherence to key enhanced recovery after surgery components, including delayed mobilisation and prolonged postoperative device use. This study aimed to identify barriers and facilitators influencing enhanced recovery after surgery implementation from the perspective of multidisciplinary perioperative staff. METHODS: A sequential explanatory mixed-methods study was

Improving hospital-based care for patients with injection-drug related infections: provider perspectives.

BACKGROUND: Injection-related severe bacterial infections (SBIs), including skin and soft tissue infections, endocarditis, and osteomyelitis, are rising in prevalence in the United States and disproportionately affect people who inject drugs (PWID). Hospitalization for SBI presents a critical opportunity to engage patients in addiction and infectious disease care, yet healthcare systems often fail to capitalize on this moment. METHODS: This qualitative study explores the perspectives of 22 clinical stakeholders, including physicians, surgeons, nurses, and social workers, on barriers and strate

Reporting for duty. One year after terrorist attacks shook the nation, hospitals confront a changed landscape--and seek to do their part to defend the homeland.

As we observe the first anniversary of the Sept. 11 terrorist attacks that changed our nation, health care providers prepare to deal with potential disasters that in better times were merely the realm of science fiction. In communities across the country, readiness is the new goal, bioterrorism the new threat.

Management of post-surgical wounds in general practice.

BACKGROUND: The availability of complex wound dressings following progressive innovations, increasing demand for hospital beds and the early discharge of post-operative patients have shifted the sharing of care of such patients from hospital specialists to general practitioners (GPs). Although several published guidelines on the management of chronic wounds exist, there is a lack of similar material addressing the assessment and management of post-surgical wounds. OBJECTIVE: The aim of this article is to provide a practical guideline for identifying early complications of post-surgical wounds

Assessing disease burden from pathogen release through untreated hospital wastewater to neighboring communities in Dhaka city.

Hospitals in low-resource settings discharge untreated wastewater containing fecal pathogens, posing substantial health risks to surrounding communities. This study aims to model fecal pathogen flows from a government hospital and neighboring communities in Dhaka, Bangladesh, and to quantify associated disease burdens across neighboring communities with varying sanitation conditions. Using a systems modeling approach integrating Quantitative Microbial Risk Assessment (QMRA) and Disability-Adjusted Life Years (DALYs), we estimated health risks from five key pathogens (Vibrio cholerae, Norovirus

Clinical AI in Canadian Community Hospitals: A Framework for Implementation Readiness.

Clinical Artificial Intelligence (AI) tools are frequently developed in academic centres yet deployed in community hospitals with different workflows, staffing, and oversight. In Canada, where community hospitals constitute approximately 90% of hospitals, this creates a validation-to-implementation gap. Health Canada's 2026 pre-market guidance for machine learning-enabled medical devices addresses manufacturer responsibilities but leaves hospital implementation responsibilities under-specified. This article proposes a framework for community hospital AI implementation readiness organized aroun

An electronic strategy to identify hospitalized heart failure patients.

BACKGROUND: A common challenge in improving performance measures regarding heart failure (HF) is identifying patients early in the course of their hospitalization so that multidisciplinary education and clinical interventions can be implemented. We describe the accuracy of using an electronic pharmacy-based strategy to identify hospitalized patients likely to have a principal diagnosis of HF at discharge. METHODS: We evaluated 2 strategies. The first used the receipt of an intravenous loop diuretic as a single predictor; the second incorporated additional lab, pharmacy, and demographic inform

Evaluation of children with severe neurological impairment admitted to hospital with pain and irritability.

BACKGROUND: Pain is the most common symptom reported by caregivers of children with severe neurological impairment (SNI), a descriptive term for children with disorders affecting the neurological system across multiple domains. In SNI, cognition, communication, and motor skills are impaired and other organ systems are impacted. Pain is difficult to identify and treat in children with SNI because of communication impairment. When a clear cause of pain is not determined, the term "Pain and Irritability of Unknown Origin (PIUO)" is used to describe pain-like behaviours. This study explores the cl
