Medicare-mandated shared decision making for left atrial appendage closure in clinical practice.

INTRODUCTION: The Centers for Medicare & Medicaid Services (CMS) required a shared decision-making (SDM) interaction, with an "independent" physician, before left atrial appendage closure (LAAC). The purpose of this study is to better understand how this requirement is implemented in clinical practice. METHODS: We surveyed LAAC-performing centers. The characteristics of respondent and nonrespondent hospitals were compared using the CMS Provider of Services File for 2017. RESULTS: We received 86 responses out of 269 surveys mailed (32%). Respondent and nonrespondent hospital affiliations were

The History of the Bellevue Hospital Chest Service (1903-2015).

For more than 100 years, the Bellevue Hospital Chest Service in New York City has contributed major advances in our understanding of pulmonary disease. Research from the cardiopulmonary laboratory of the Chest Service by Drs. Exploration and practice of data-driven high-quality development in smart hospitals.

BACKGROUND: In this study, we explored a data-driven approach to smart hospital construction, addressing challenges such as data silos, fragmented processes, and delayed decision-making within healthcare systems. The proposed framework supports the high-quality development of public hospitals in China and facilitates intelligent integration among healthcare delivery, service provision, and management functions, achieving synergistic advancement of the "trinity" of hospital operations. METHOD: A phased action research method was employed, comprising three stages: (1) Infrastructure constructio

Catastrophic health payments in Ghana post-National Health Insurance Scheme implementation: an analysis of service-specific health expenditures.

BACKGROUND: Ghana implemented several health reforms in the 1970s and 1990s. Still, several access barriers persist, including high out-of-pocket (OOP) spending, which led to the implementation of the National Health Insurance Scheme (NHIS) in 2003 to achieve Universal Health Coverage and lower OOP spending. This study evaluates the incidence and intensity of catastrophic health expenditure (CHE) among Ghanaian households post-NHIS, considering OOP health spending on different health services. METHODS: Data came from the Ghana Living Standards Surveys rounds 6 (2012/2013) and 7 (2016/2017) an

Inpatients' understanding of the hospitalist role and common medical terminology.

Many patients are unable to identify members of their hospital care team and experience confusion regarding some medical terminology used during hospitalization, including descriptions of the structure of their inpatient care team. This cross-sectional study sought to (1) examine inpatients' understanding of the role of a hospitalist and (2) assess inpatients' familiarity with other medical terminology commonly used in the hospital. We surveyed 172 patients admitted to the hospital medicine service at two academic medical centers. We found that almost half (47%) of respondents were unfamiliar

Is Electronic Information Exchange Associated With Lower 30-Day Readmission Charges Among Medicare Beneficiaries?

OBJECTIVE: Fragmented readmissions, when admission and readmission occur at different hospitals, are associated with increased charges compared with nonfragmented readmissions. We assessed if hospital participation in health information exchange (HIE) was associated with differences in total charges in fragmented readmissions. DATA SOURCE: Medicare Fee-for-Service Data, 2018. STUDY DESIGN: We used generalized linear models with hospital referral region and readmission month fixed effects to assess relationships between information sharing (same HIE, different HIEs, and no HIE available) and

Transplant Business Best Practices: An American Society of Transplant Surgeons Business Practice Services Committee Opinion on Reimbursement, Cost Accounting, and Professional Coding in Modern Transplant Practice.

Financial stewardship in transplantation requires integrated management of three distinct reimbursement domains: organ acquisition, transplant hospitalization, and physician professional services. Of these, organ acquisition is often the most complex and least intuitively understood, despite its major financial implications. This article outlines business best practices for transplant programs. Organ Acquisition Cost Centers should be managed as cost recovery structures rather than profit centers, with attention to Medicare Cost Report mechanics, living donor cost reporting, organ statistics,

A pilot study-genetic diversity and population structure of snow leopards of Gilgit-Baltistan, Pakistan, using molecular techniques.

CONTEXT: Hand therapy optimises functional use of the hand and arm after injury and is an expert area of practice for occupational therapists (OTs) and physiotherapists. In rural Australia, patients frequently travel to metropolitan or larger regional centres for initial hand surgery and rehabilitation. However, rural patients' access to follow-up hand therapy after the initial phase of care is impacted by several factors such as transport options, distance, staff shortages and availability of therapists skilled in hand therapy. To ensure service equity, these challenges require consideration

The Top Ten Research Priorities for Outpatient Service Delivery: A South West London Outpatient Transformation Group and James Lind Alliance Priority Setting Partnership.

BACKGROUND: Outpatient appointments account for 85% of hospital activity and are fundamental to the running of the National Health Service (NHS). The COVID-19 pandemic and government initiatives have prompted outpatient services transformation. We aimed to determine research priorities for outpatient transformation, which represent the shared priorities of patients, their families, carers and healthcare professionals. METHODS: We established a national (UK) priority setting partnership (PSP) with the James Lind Alliance (JLA) to capture the top 10 research priorities regarding outpatient serv

Patient Participation in Moral Case Deliberation: Evaluating the Dynamics of Shared Reflection in a Mental Healthcare Setting in Türkiye.

Clinical ethics support services (CESS) have traditionally centered on healthcare professionals (HCPs), leaving the patient participation (PP) as a subject of controversy. This paper evaluates the practical and dialogical dynamics of a patient-participatory moral case deliberation (MCD). We analyze a MCD session conducted in a Turkish psychiatric hospital using the Dilemma Method. The group-comprising the facilitator, the case presenter (a senior psychiatrist), seven mental health professionals including a nurse and a clinical psychologist, and the patient herself-deliberated on a complex mora

Orthopaedic injuries due to electric scooter trauma presenting to a level 1 trauma centre before and after introduction of a scooter-sharing scheme.

BACKGROUND: Epidemiological data from various jurisdictions has shown that electric scooters are associated with significant trauma. The Victorian state government introduced a trial scooter sharing scheme on February 1, 2022 in inner city Melbourne. This is a descriptive study from the largest trauma centre in Victoria, geographically at the heart of the government sharing scheme, investigating the "scope of the problem" before and after introduction of the ride sharing scheme. METHODS: Retrospective case series. Insitutional orthopaedic department database was searched from 1 Jan 2021 to 30

Acute Respiratory Infections in Children: A Narrative Perspective on Integrated Primary-Hospital Care in Italy.

Acute respiratory infections (ARIs) remain one of the leading causes of pediatric consultations, emergency department visits, and hospital admissions worldwide, imposing a substantial clinical, organizational, and economic burden on healthcare systems. Although most ARIs are self-limiting viral illnesses, their high incidence during childhood continues to challenge both primary care pediatricians and hospital services. In recent years, the epidemiology and prevention of pediatric respiratory infections have evolved considerably following the COVID-19 pandemic and the introduction of nirsevimab
