Variations of plasma levels of insulin, glucagon, amylase and glucose and histopathological lesions of pancreatic tissue after complete elimination of exocrine pancreatic function with surgical ligation versus prolamine occlusion.

OBJECTIVE: Pancreaticoduodenectomy has been established as the treatment of choice for the management of benign and malignant lesions of the … usion with prolamine than after ligation. Ligation of pancreatic duct lead to significantly greater hyperamylasemia than prolamine occlusion every day until the 15th. Mild hyperglycemia presented from the first to the fourth day in both groups, which was associated with a significant drop in insulin. Glucagon remained within the normal values at all times during …

Diagnosis of macroamylasemia in a pregnant patient.

BACKGROUND: Macroamylasemia occurs with the formation of macroamylase complexes that cannot be cleared via renal excretion. In patients with persistent serum amylase elevation or hyperamylasemia with normal urine amylase and lipase levels, the diagnosis of macromylasemia is suggested. MAIN FINDINGS: The patient is a 31-year-old G2P1 who presented to the ob/gyn triage with the complaint of recurrent right-sided abdominal pain. Serum amylase values were elevated. The patient had been admitted previously at GA 16 weeks for recurrent acute cholecystitis and gallstone pancreatitis and had undergon…

Afferent loop obstruction presenting as acute pancreatitis and pseudocyst: case reports and review of the literature.

Afferent loop obstruction after gastrectomy and Billroth II reconstruction is an uncommon problem. Complete acute obstruction requires emergent laparotomy. However, chronic obstruction may begin insidiously and its symptoms may reflect other gastrointestinal diseases. Two patients are described who developed acute abdominal pain, marked hyperamylasemia, and palpable abdominal masses 5 and 15 years after Billroth II gastrectomy. The masses were initially interpreted as pancreatic pseudocysts. Both patients were found to have chronically obstructed afferent limbs, and in one the obstruction was …

[Acute pancreatitis and malabsorption with tetany secondary to Crohn's disease with duodenal involvement].

Involvement of the duodenum in Crohn's disease is uncommon, and the pathomechanism of the associated acute pancreatitis remains controversial. We describe a case of Crohn's disease with duodenal involvement associated with hyperamylasemia and malabsorption showing a favorable response to steroid treatment.

Twenty-four-hour serum amylase predicting pancreatic reaction after endoscopic sphincterotomy.

BACKGROUND AND STUDY AIMS: Acute pancreatitis is still the most common complication after endoscopic sphincterotomy (ES) and cholangiopancreatography (ERCP). The aim of this study was to detect the time when the peak of serum amylase was predictive for postprocedure pancreatitis or long-lasting severe hyperamylasemia, in order to plan the follow-up of patients. METHODS: Serum amylase activity was measured in a prospective series of 409 consecutive patients after ES, immediately before ES and two, four, eight and 24 hours thereafter; the two, four and eight-hour data were compared with those a…

Fluctuations in serum amylase in patients with macroamylasemia.

OBJECTIVE: To report wide fluctuation of serum amylase in patients with macroamylasemia. It has generally been considered to remain constant … fluctuation of serum amylase in three out of four patients. In the fourth patient, more persistent hyperamylasemia was noted during the shorter observation period. CONCLUSION: Marked fluctuation in serum amylase, ranging from 115 to 1160% in this study, may occur in patients with macroamylasemia. The reasons for these fluctuations are not clear but may be due t …

Amylase inhibitor from wheat: its action and clinical application.

The electrophoretic and column chromatographic characteristics of an amylase inhibitor of wheat origin were investigated. Further, the clini … atients with acute pancreatitis was over 8.0, whereas that in sera from patients with salivary-type hyperamylasemia such as mumps, pulmonary diseases and following surgery was less than 0.1. However, hyperamylasemia due to macroamylase or renal failure could not be identified by the inhibitor method.

Fictitious pancreatitis in choledochal cyst.

The classical presentation of choledocal cyst has been regarded as a triad of abdominal pain, jaundice and a palpable abdominal mass; unusua … cal cyst. Our 3 cases were treated by cyst excision and have remained asymptomatic. The presence of hyperamylasemia should not delay appropriate surgical management. The treatment of choice is cyst excision, since it will eliminate factors contributing to the development of cholangitis and hyperamylasemia.

Biliary disease in pregnancy: strategy for surgical management.

Acute biliary tract disease complicated intrauterine pregnancy in 26 patients seen during a 5 year period. Biliary symptoms were distinct and occurred during the first trimester in 7 patients, the second trimester in 5 patients, the third trimester in 12 patients, and in two early postpartum patients. Nine patients had marked hyperamylasemia which resolved with medical management, and no severe cases of pancreatitis occurred. Ultrasonography was used to confirm the presence of gallstones in 18 patients and demonstrated dilated intrahepatic ducts in one of two patients with surgically proved ch…

Chronic intussusception due to antral myoepithelioma.

Intussusception was seen on abdominal sonography and computed tomography in a 15-year-old boy who presented with a 6-week history of weight loss, vomiting, abdominal pain, abdominal mass, and hyperamylasemia. Laparotomy revealed a chronic gastroduodenal intussusception, the lead point of which was an antral myoepithelioma, a rare entity in this age group.

Simultaneous macroamylasemia and macrolipasemia in a patient with mucosa-associated lymphoid tissue lymphoma.

We report a case of simultaneous macroamylasemia and macrolipasemia complicated with mucosa-associated lymphoid tissue (MALT) lymphoma. A 78-year-old man presented with hyperamylasemia and hyperlipasemia for 2 years and was misdiagnosed with chronic pancreatitis at another hospital. However, his other pancreatic enzymes were normal, his amylase-creatinine clearance ratio was low, and no definite findings of pancreatic disease were evident. Immunological analyses revealed that both amylase and lipase were bound to immunoglobulin (Ig) A-κ, and that serum IgA was high (827.1 mg/dL). He was diagno…

[Sialoadenitis following ablative therapy with high doses of radioiodine for treatment of differentiated thyroid cancer].

We analyzed eight-three patients who received 3.7 GBq (100 mCi) or 7.4 GBq (200 mCi) after total thyroidectomy for thyroid cancer. This study consisted in clinical and lab analysis (amylase serum level), followed by iodine whole body scan. The sialoadenitis was defined by hyperamylasemia (> 200 U/L). Eleven (13.25%) patients referred spontaneous local pain or after mastication post I 131 ablative therapy. Hyperamylasemia was detected in 31 (37.3%) patients in the second day after treatment. After seven days, serum amylase levels were in normal values in all patients. The symptomatic sialoadeni…

Pancreatic involvement in fatal human leptospirosis: clinical and histopathological features.

Hyperamylasemia has been reported in more than 65% of patients with severe leptospirosis, and the true diagnosis of acute pancreatitis is complicated by the fact that renal failure can increase serum amylase levels. Based on these data we retrospectively analyzed the clinical and histopathological features of pancreas involvement in 13 cases of fatal human leptospirosis. The most common signs and symptoms presented at admission were fever, chills, vomiting, myalgia, dehydratation, abdominal pain and diarrhea. Trombocytopenia was evident in 11 patients. Mild increased of AST and ALT levels was …

Celiac sprue and macroamylasemia: potential clinical and pathophysiological implications. Case study.

In a patient with celiac sprue, abdominal complaints were ascribed to acute pancreatitis in the setting of hyperamylasemia and choledocholithiasis. The problem turned out to be macroamylasemia associated with elevated serum immunoglobulin A. The patient remained stable for 4 years with no specific intervention except gluten restriction. An association between celiac sprue or other malabsorptive states and macroamylasemia requires more study.
