The authors investigated clinical manifestations of hyperandrogenism in infertile women with hirsutism. Assessment of accurate quantitative indicators (hirsutic number, clitoral index, body mass growth coefficient and morphogram) made it possible to characterize the manifestations of hyperandrogenism in infertile women (n = 112). Correlation analysis revealed that the state of hirsutism and the body mass growth relations are the most sensitive indicators of the action of androgens (testosterone) in fertile age.

Several antiandrogens compounds are available in the treatment of acne and hirsutism in women. Cyproterone acetate is the most effective antiandrogen. Its use must prevent the occurrence of amenorrhea with the prescription of an appropriate estrogen. There are diagnostic tests that allow to differentiate feminine hyperandrogenism of tumorous, non-tumorous, ovarian, adrenal and peripheral origin. We made a review of the current available tests accordingly to a case of amenorrhea and hyperandrogenism. By means of the low-dose dexamethasone suppression and human chorionic gonadotropin hormone (hCG) stimulation test we confirmed a suprarenal participation. Five females with idiopathic hirsutism, ten with adrenogenital syndrome, four with acne vulgaris, and three with Stein-Leventhal syndrome were treated for nine months with large or small doses of cyproterone acetate (100 or 2 mg daily, respectively) in combination with oestrogen. The drug had a good effect on the acne vulgaris and moderated the hirsutism. In the various patient groups the serum testosterone level was decreased to between 58.6% (+/- 28.7% S.E.) and 10.8% (+/- 8.2% S.E.) of the basal value. No essential difference in effectiveness was observed between the large and small doses.
