A familial cerebral malformation combined with chronic hypernatremia is reported. We found three children of a Moroccan family with microcephaly and hypoplasia of the frontal brain. The frontal parts of the lateral ventricles were absent. Two of the children had a defect of the midline structures with fusion of the lateral ventricles similar to holoprosencephaly. Further clinical symptoms were generalized spasticity, delay of mental development and hypodipsia.

We report the case of a 57-year-old chronic schizophrenic patient who developed an acute catatonic stupor after postoperative withdrawal of neuroleptics with excessive and firstly therapy-resistant hypernatremia. Only after re-treatment with neuroleptics the hypernatremia could be compensated. A relationship between the catatonic schizophrenia and the electrolyte-disturbances can be supposed; probably in both diencephalic structures are involved. Therefore the syndrome of "acute life threatening catatonia" should be included into the differential diagnosis of uncertain metabolic disorders.

Rhabdomyolysis, a rare condition in the paediatric population, is characterised by the release of intracellular components due to muscle damage. Although various aetiologies have been described, severe hypernatraemia is rarely recognised as a cause. We present a paediatric case of rhabdomyolysis associated with severe hypernatraemia secondary to traumatic brain injury (TBI). A male in early adolescence with severe TBI following a motorcycle accident developed severe hypernatraemia (168 mEq/L) during management in the intensive care unit with hypertonic solution. Creatine kinase levels were ele

Adipsic hypernatremia is a rare hypothalamic disorder characterized by impaired thirst despite preserved renal concentrating ability and may coexist with multisystem neuroendocrine and sleep disturbances. We report a 20-year-old male with beta-thalassemia minor presenting with severe hypernatremia with serum sodium of 170 mEq/L (SI: 170 mmol/L) (reference range 133-146 mEq/L [SI: 133-146 mmol/L]), absent thirst, weight loss, fatigue, hypogonadotropic hypogonadism, and hyperprolactinemia, along with excessive daytime sleepiness, hypnagogic hallucinations, and emotion-triggered cataplexy. Sodium-glucose cotransporter 2 (SGLT2) inhibitors have been widely used. They inhibit proximal tubular glucose reabsorption, resulting in glycosuria. Herein, we report the case of a 65-year-old woman who presented with hypernatremia during the perioperative period of a subarachnoid hemorrhage. The patient continued to take dapagliflozin postoperatively and subsequently developed severe hypernatremia. Based on the urinalysis findings, we diagnosed osmotic diuresis due to glycosuria as contributing to hypernatremia. Hypernatremia improved with the discontinuation of dapagliflozin and the adminis…

OBJECTIVE: The literature indicates a single universal cut-off point for weight loss after birth for the risk of hypernatremia, without considering other factors. The aim of this study was to construct and internally validate cut-off points for the percentage weight loss associated with the risk of hypernatremia, taking into account risk factors.
METHODS: A prospective study with a three-day follow-up was conducted in 165 neonates with a gestational age ≥35 weeks. The main outcome variable was mild or moderate hypernatremia (serum sodium≥145mmol/L). Secondary variables (risk factors) were mat…

INTRODUCTION: Neonatal hypernatermic dehydration (NHD) is a potentially very serious condition, which has been reported to occur in infants who have breast feeding problems in the first week of the life. This study looked at the incidence, risk factors, clinical symptoms and complications of NHD in healthy breastfed term neonates.
METHODS: A prospective case-control study was conducted on 53 neonates with serum sodium concentrations of >or=150 mmol/L (as the case group) who were recruited between June 2006 and June 2007 from the Ghaem hospital (Mashhad, Iran) to investigate the relationship b

UNLABELLED: The association of diabetes insipidus and adipsia after craniopharyngioma surgery has high morbidity. Hypernatremia can be caused by adipsia and be aggravated by diabetes insipidus. Rhabdomyolysis rarely occurs.
CASE REPORT: This is the first report of a diabetic patient with craniopharyngioma who developed diabetes insipidus and adipsia after surgery, evolving with severe hypernatremia that caused considerable rhabdomyolysis.
CONCLUSION: The importance of the evaluation of muscle integrity when under hypernatremic states is pointed out. Although adipsia may have a simple solutio…

OBJECTIVE: The aim of the study was to determine the incidence of significant weight loss, dehydration, hypernatremia and hyperbilirubinemia in exclusively breast-fed term healthy neonates and compare the incidence of these problems in the warm and cool months.
METHODS: During the study period 496 neonates were recruited.
RESULTS: 157 neonates (31.6%) had significant weight loss (> 10 % cumulative weight loss or per day weight loss > 5%). Clinical dehydration was present in 2.2% of neonates. Of these 157 neonates, 31.8% had hypernatremia and 28 % had hyperbilirubinemia.
CONCLUSION: The inci…

We report a case of postobstructive hypernatremia, and illustrate its pathogenesis and treatment. Physicians should be aware of this condition, given its high mortality rate (up to 70%), the high prevalence of potentially obstructive prostatic disease in elderly people and the peculiar sensitivity of this age group to disorders of osmotic regulation. Knowledge of the processes involved in osmoregulation has provided insights into the pathogenesis of this condition, which includes at least three factors: (i) decreased efficacy of the thirst mechanism in elderly patients, (ii) water loss in exce…

Locally recurrent, poorly differentiated carcinoma of the prostate was associated with hypokalemic alkalosis, marked hypernatremia, diabetes mellitus of recent onset, and hyperosmolar syndrome. These findings, with mild hypertension, in the absence of clinical features of Cushing's syndrome, suggested an ectopic ACTH syndrome. Plasma ACTH and cortisol levels were markedly elevated, and failed to suppress in response to either low or high-dose dexamethazone administration. The patient's condition deteriorated rapidly. Autopsy findings included carcinoma extensively infiltrating the prostate wit…

A 14-year-old boy presented 6 weeks after out-of-country sleeve gastrectomy with profound hypernatremic dehydration (serum sodium 195 mmol/L), stage 3 acute kidney injury, and severe rhabdomyolysis. The patient had no structured postoperative follow-up and no primary care pediatrician and relied on sports drinks and intermittent intravenous fluids at a medical spa for hydration. Despite extreme hypernatremia, neurologic findings were relatively mild, consistent with gradual onset. Judicious rehydration and correction of serum sodium in the pediatric intensive care unit resulted in full recover…

STUDY OBJECTIVE: Hypernatremia is a treatable biochemical disorder associated with significant morbidity and mortality in patients undergoing surgery. However, its impact on patients who undergo elective craniotomy is not well understood. This study aimed to investigate the prognostic implications of postoperative hypernatremia on the 30-day mortality of patients undergoing elective craniotomy.
DESIGN: Retrospective cohort study.
SETTING: The Department of Neurosurgery of a high-volume center.
PATIENTS: Adult patients undergoing elective craniotomy except those with pituitary tumors, intrac…
