Hyperprolactinemia in a patient with Sheehan's syndrome.

I have reported the case of a woman with Sheehan's syndrome who had hyperprolactinemia rather than prolactin deficiency. The patient showed no increase in serum thyrotropin in response to TRH challenge and failed to show a normal increase in serum GH and cortisol after insulin-induced hypoglycemia. A blunted FSH response to LH-RH stimulation was also found. A CT scan of the sella showed no adenoma. The patient responded to prednisolone and thyroxine replacement therapy. When, a few months later, the dose of thyroxine was reduced to a suboptimal level, the hyperprolactinemia recurred.

[Hyperprolactinemia. Study of a current topic].

This recapitulative study presents an integral synthesis of this important subject which during the last years has become more important, particularly regarding drugs for its control, since the studies of Chiari Fromel until now. The idea is to present a didactic contribution for the gyneco-obstetrician which will become more important and useful in the treatment of patients with this problem.

Reduction of distress in hyperprolactinemia with bromocriptine.

In a double-blind placebo-controlled crossover study of bromocriptine in eight hyperprolactinemic patients, self-rated distress decreased and well-being increased parallel with the fall in prolactin levels; for the majority of measures the differences were significant.

The natural history of idiopathic hyperprolactinemia.

Idiopathic hyperprolactinemia (IH) can be defined as the presence of elevated serum PRL levels in a patient in the absence of demonstrable pituitary or central nervous system disease and of any other recognized cause of increased PRL secretion. This study examined the long term clinical outcome of 41 patients (mean age, 26 yr) with IH followed for up to 11 yr (mean, 5.5 yr). Initial and final PRL levels were determined by RIA in the same laboratory. A correction factor was used to obviate periodic changes in the potency of the NIH standards used in the PRL assay, so that all results are expres…

[Hyperprolactinemia in children with celiac disease].

The serum prolactin level was determined serially in patients with coeliac disease during gluten uptake and on gluten free diet, and in one to 14 years old healthy children serving as controls. The prolactin levels in healthy controls and in coeliac patients on diet were within normal limits whereas in children with coeliac disease taking gluten in their meals significant hyperprolactinaema was found.

[Dopaminergic agents in the treatment of hyperprolactinemia].

The treatment of hyperprolactinemia with dopaminergic agents is directly correlated with the improvement of the knowledge about hypothalamic control of prolactin secretion. Since 1963 it is wellknown that the hypothalamus acts on this activity of the pituitary gland with a tonic inhibition. A prolactin inhibiting factor (PIF) was suspected for this control and the tubero-infundibular dopaminergic system was considered to modulate the secretion of PIF. In fact a lot of experience demonstrated a direct control of dopamine on pituitary gland itself (fig. 1). Ergot alkaloids are the most useful do…

Prevalence of hyperprolactinemia in anovulatory women.

To determine the prevalence of hyperprolactinemia in an ovulatory patients presenting to a general gynecology clinic, a 1-year prospective study (1978) was instituted. A total of 119 patients with at least 3 months of anovulation were screened with serum prolactin determinations. Those patients demonstrating hyperprolactinemia were further evaluated with a serum TSH level and hypocycloidal polytomography of the pituitary sella. In patients with anovulation with or without galactorrhea, a hyperprolactinemia prevalence rate of 15% was established. Anovulatory patients with galactorrhea had a hyp…

Hyperprolactinemia and hypogonadism in the human female.

Prolactin is a mammotropic hormone essential for the initiation of lactation. It also influences ovarian function; during hyperprolactinemia hypogonadism occurs. This is true for pathological forms of hyperprolactinemia but also for the early puerperium when there is physiological hyperprolactinemia. Amenorrhea is a better parameter of hyperprolactinemia than galactorrhea. The mechanisms by which prolactin disrupts ovarian function are not as yet fully understood; it probably alters hypothalamic neurotransmitter content through a direct feedback mechanism resulting in a decrease of Gn-RH. Howe…

[Hyperprolactinemia in the postmenopause: versions and contraversions].

The prevalence of hyperprolactinemia in postmenopausal women is unknown and has been estimated as infrequent by many studies. Prolactinomas found after menopause are usually macroadenomas and remain unrecognized for a long time due to atypical clinical signs or their absence. The growth potential of prolactinomas persists after menopause, most of them are invasive and accompanied by high prolactin levels. Treatment with dopamine agonists is usually long-term, the goals of which are to reduce tumor size, normalize prolactin levels and the negative effects of hyperprolactinemia. Treatment with c…

Hair loss and hyperprolactinemia in women.

In the literature of the past 30 years there are only some publications concerned with hair loss and hyperprolactinemia in women. Therefore, the relevance of hyperprolactinemia was evaluated in 40 women with diffuse alopecia.Hair loss was assessed by clinical appearance and the pluck trichogram. 82.5% of the female patients had diffuse hair loss and 17.5% had androgenetic alopecia. The highest prolactin values measured were 1390 ng/ml and 255 ng/ml. Six patients had values between 150-80.4 ng/ml and 10 between 79.1-51.7 ng/ml. All others had prolactin values below 50 ng/ml. Fifteen untreated pa…

[Macroprolactinemia identification in patients with hyperprolactinemia].

BACKGROUND: Macroprolactinemia is defined as hyperprolactinemia with predominance of the big-big prolactine isoform. Its frequency has not been clearly established due to technical difficulties to identify it. This method used to detect it is gel filtration chromatography, an expensive and complicated procedure that could not be used routinely. OBJECTIVE: To validate the PEG precipitation technique, to identify the presence of macroprolactinemia and to correlate it with the clinical characteristics in a group of pre-selected patients with elevated serum PRL levels from different causes. PATI…

[The diagnostic value of macroprolactin in hyperprolactinemia].

Macroprolactinemia (MP) in which there is a discrepancy between prolactin (P) levels and clinical symptoms has recently generated great interest among physicians of various specialties since not the whole of P is biologically active. The diagnosis of this condition is a topical problem. The purpose of the investigation was to detect high molecular-weight or big big P in patients with hyperprolactinemia (HP) of various genesis and to define its clinical importance. A hundred and one females with HP (P > 700 mU/l) were examined. In each patient, biologically active monomeric P was measured via t…

Diagnosis, misdiagnosis and management of hyperprolactinemia.

Hyperprolactinemia is a commonly encountered disorder that suppresses both male and female gonadal function. The etiology includes pituitary tumors, hypothalamic or pituitary stalk lesions, drugs and hypothyroidism. In women, the hyperprolactinemic syndrome is characterized by menstrual disorders with or without galactorrhea, while men present with hypogonadism and related symptoms. Occasionally, a pituitary macroadenoma may be associated with pressure symptoms and/or hypopituitarism. Clinically, the most important cause of hyperprolactinemia is a prolactin-secreting pituitary adenoma. The maj…
