Liposarcoma is the most common histologic subtype of soft tissue sarcoma in the retroperitoneum. The distinction of primary cord liposarcomas, which arise in and are confined to the inguinal canal, from inguinoscrotal extension of a retroperitoneal tumor is mandatory. Four years after treatment of colon cancer an 82-year-old man presented with an irreducible, recurrent inguinal hernia with mesh-plug repair. Delayed surgical exploration revealed metastatic cancer with small bowel adhesion to the mesh-plug and perforation. The mesh-plug with adherent tissue was removed through the inguinal incision, and the ruptured small bowel was resected. Histopathology revealed moderately differentiated mucinous adenocarcinoma of the tissue adhering to the prosthetic material corresponding to the original colon cancer.

Original device from nikelid-titanium for reconstruction of posterior wall of inguinal canal was created. Clinical application of this device permitted to prevent the hernia recurrences in long-term period after surgery and reduce the number of complications associated with compression of spermatic cord. Doppler examination of testicular vessels demonstrates that this device improves blood flow compared with other methods of hernioplasty.
