The fiberoptic device is a relatively new type of intracranial pressure (ICP) monitor which appears to offer certain advantages over conventional monitoring systems, particularly its ability to measure brain parenchymal pressures. This study was undertaken to analyze the accuracy and drift characteristics of the fiberoptic device and to compare pressures in the subdural, intraparenchymal, and intraventricular compartments. The device was accurate to +/- 3 mm Hg over a 0- to 30-mm Hg range in vitro. The maximum daily drift was +/- 2.5 mm Hg, with an average daily drift of +/- 0.6 mm Hg and an a…

The mechanism of cerebral homeostasis is reviewed, paying particular attention to the way blood-brain barrier, cerebrospinal fluid and cerebral blood flow contribute to the maintenance of normal intracranial pressure. The pathophysiology of raised intracranial pressure is outlined delineating the different types of cerebral oedema. Guidelines for the management of patients with raised intracranial pressure are presented as well as the techniques of intracranial pressure monitoring.

The major changes in clinical condition during and after encephalography may be explained by intracranial pressure variations. If neither intracranial hypertension nor CSF dynamic perturbations exist, minimal complications may be predicted, but in intracranial hypertension immediate deterioration may occur. Despite an apparently good level of tolerance, complications may develop if the compensation phenomena cannot occur: normal pressure hydrocephalus, craniosynostosis. In the cases with good clinical tolerance, minimal changes occur when air injection is administered at a slow rate and with a…

Neurosurgical patients, whether injured or not, have benefited in recent years from a new technique of supervision, measurement of thr intracranial pressure (ICP). Thus, measurement of a pressure figure and its course, gives precious information, both diagnostic and therapeutic. Various techniques of measurement are now proposed. We chose, for special reasons, measurement of the ICP within the ventricle. A catheter is introduced into the ventricle through a trephine hole. It is linked by a liquid column to a recording system (cathode oscilloscope, ink writer or magnetic tape). A slow inscriber…

Real-time monitoring of intracranial pressure (ICP) is a routine part of neurocritical care in the management of brain injury. While mainly used to detect episodes of intracranial hypertension, the ICP signal is also indicative of the volume-pressure relationship within the cerebrospinal system, often referred to as intracranial compliance (ICC). Several ICP signal descriptors have been proposed in the literature as surrogates of ICC, but the possibilities of combining these are still unexplored. In the present study, a rapid ICC assessment consisting of a 30-degree postural shift was performe…

Changes in intracranial pressure (ICP) during sleep were investigated in 37 patients with chronic intracranial hypertension or chronic hydrocephalus, in whom episodic pressure waves characterized by A-waves or episodic B-waves were seen in Part I of this paper. The patients were conscious, and sleep stages including REM sleep were observed in all of them. During non-REM sleep, ICP significantly rose in Stage II on many occasions, and was always lower in Stage IV than in other sleep stages. A marked elevation of ICP was seen in REM sleep. ICP changes during REM sleep were characterized by frequ…

The current evidence associating intracranial pressure (ICP) with glaucoma, the translaminar pressure gradient hypothesis, and anatomic factors likely affecting the relationship between ICP and retrolaminar tissue pressure including the size of the optic canal and lamina cribrosa thickness are reviewed. In addition, the evidence of diurnal and positional variation on the translaminar pressure gradient, effects of glaucoma medications, evidence of ICP helping to maintain cerebrospinal fluid flow in the optic nerve to prevent glaucomatous damage, and the effect of intraocular pressure variation …

BACKGROUND: Intracranial pressure (ICP) monitoring is critical for managing traumatic brain injury, yet invasive methods carry risks and require specialized settings. Noninvasive transcranial Doppler (TCD)-based methods have been proposed to estimate ICP and cerebral perfusion pressure (CPP), although validation studies predominantly assess static agreement. Clinical monitoring depends on accurate reproduction of dynamic changes. This study assesses the ability of noninvasive methods to reproduce temporal variations in ICP, CPP, and autoregulatory indices.
METHODS: Nonoverlapping 5-minute seg…

This work presents a prospective morphological and quantitative analysis of 43 intracranial pressure recordings performed on normal pressure hydrocephalic patients. This analysis led us to separate Lundberg's B waves into different subtypes and to refine the definition of the 'Plateau' wave. Two B wave subtypes named Great Symmetrical wave and Intermediate wave appeared correlated with the surgical improvement. In addition, the degree of post-operative improvement was correlated with the frequency of Intermediate wave. An extended quantitative classification of intracranial pressure waves is p…

OBJECTIVES: This study aimed to investigate the impact of Twin Block appliances on intracranial pressure (ICP) in adolescents by measuring the optic nerve sheath diameter (ONSD) using ultrasonography.
METHODS: We conducted a prospective study involving 20 adolescents (8 girls and 12 boys) with skeletal mandibular retrognathia undergoing treatment with Twin Block appliances. ONSD measurements were taken at six different time points: before appliance placement (T0), 1 min after placement (T1), 10 min after placement (T2), one month after the start of treatment with twin block (T3), immediately …

Monitoring intracranial pressure (ICP) is important in a variety of neurologic conditions, including aneurysmal subarachnoid hemorrhage and traumatic brain injury. Monitoring and controlling ICP can mitigate secondary injury of the brain. Of the invasive methods of monitoring ICP, the external ventricular drain is still considered the gold standard. However, microtransducers have been shown to be a reliable option with significantly lower risks of complications. Due to their reproducibility, and their limitations, they are not ready to replace invasive ICP monitoring techniques. This article r…

Intracranial pressure (ICP) is the pressure within the cranium. ICP rise compresses brain vessels and reduces cerebral blood delivery. Massive ICP rise leads to cerebral ischemia, but it is also known to produce hypertension, bradycardia and respiratory irregularities due to a sympatho-adrenal mechanism termed Cushing response. One still unresolved question is whether the Cushing response is a non-synaptic acute brainstem ischemic mechanism or part of a larger physiological reflex for arterial blood pressure control and homeostasis regulation. We hypothesize that changes in ICP modulates sympa…

Intracranial pressure (ICP) is governed by volumes of intracranial blood, cerebrospinal fluid, and brain tissue. Expansion of any of these volumes will trigger compensatory changes in the other compartments, resulting in initially limited change in ICP. Due to the rigid skull, once compensatory mechanisms are exhausted, ICP rises very rapidly. Intracranial hypertension is associated with unfavorable outcome in brain-injured patients. This chapter discusses the pathophysiology of raised ICP, as well as typical waveforms, monitoring techniques, and clinical management. The dynamics of ICP are mo…

PURPOSE OF REVIEW: : Persistent elevation of intracranial pressure (ICP) can lead to cerebral ischemia, brain herniation, and possibly death. Understanding the fundamental mechanism that contributes to the rise in ICP and recognizing the specific intracranial compartment involved (brain, CSF, or blood) can lead to early diagnosis and effective treatment. This article reviews the conceptual approach to a patient with elevated ICP.
RECENT FINDINGS: : The overall goal for patients with intracranial hypertension is to lower ICP below 20 mm Hg and to maintain cerebral perfusion pressure (CPP) betw…

Tuberculous meningitis (TBM) remains a common serious neurological emergency especially in the developing world. Elevated intracranial pressure (ICP) is often a feature of severe TBM and is associated with high morbidity and mortality. The pathology associated with TBM, such as cerebral edema, hydrocephalus, tuberculoma(s), and infarcts related to arthritis, contribute to increase in intracranial volume and, therefore, elevated ICP. The three types of edema (vasogenic, cytotoxic, and interstitial) may contribute to cerebral edema. The molecular mechanisms underlying the events that ultimately …
