[Intraoperative ultrasonography in Itsenko-Cushing's disease and syndrome].

Ultrasonic study conducted during an operation is finding wider use lately. Surgery is the main method of treatment of most diseases of the adrenals, therefore problems of topical intraoperative diagnosis are very urgent. Twenty-three patients were operated on for Itsenko-Cushing's disease and syndrome and tumors of the retroperitoneal space at the Faculty Surgical Clinic of the Moscow Medical Academy within a period of 12 months. All patients were subjected to complex ultrasonic examination, including pre-intra- and postoperative study. Intraoperative ultrasonic study was conducted in two sta

Patient and operative factors associated with unanticipated intensive care admission and outcomes following posterior fossa decompressions in children: A retrospective study.

INTRODUCTION: Posterior fossa decompression for Chiari I Malformation is a common pediatric neurosurgical procedure. We sought to identify the impact of anesthesia-related intraoperative complications on unanticipated admission to the intensive care unit and outcomes following posterior fossa decompression. METHODS: Medical records of all patients <18 years who underwent surgery for Chiari I malformation between 1/1/09 and 1/31/21 at the Ann & Robert H. Lurie Children's Hospital of Chicago were included. Records were reviewed for patient characteristics, anesthesia-related intraoperative comp

Spilled gall stones during laparoscopic cholecystectomy: a review of the literature.

Laparoscopic cholecystectomy is associated with spillage of gall stones in 5%-40% of procedures, but complications occur very rarely. There are, however, isolated case reports describing a range of complications occurring both at a distance from and near to the subhepatic area. This review looks into the various modes of presentation, ways to minimise spillage, treating the complications, and the legal implications.

Management of diabetes mellitus during surgery.

Patients with diabetes mellitus are subjected to major operations more frequently than those without diabetes. Although many of these operations are done on an elective basis, the perioperative control of blood glucose levels--ranging from 6.7 to 13.3 mmol per liter (120 to 240 mg per dl)--remains a therapeutic challenge. In planning the management, the type of diabetes, current treatment, the degree of recent control, the presence of complications, and the type of surgical procedure must all be considered. All insulin-dependent patients and many non-insulin-dependent ones need insulin therapy

Palpatory method used to identify the recurrent laryngeal nerve during thyroidectomy.

The major complication of thyroid surgery, occurring in 1% to 6% of cases, is injury to the recurrent laryngeal nerve (RLN). A simple method to identify the RLN during thyroid surgery is described by the authors. It consists in palpation of the nerve caudally to the inferior pole of the thyroid, after the nerve has been made taut by the upward and medial traction of the thyroid gland. This method was used on 47 human cadavers and 45 patients with benign thyroid diseases. It made it possible to identify the RLN in all of the cadavers and 52 of the 55 identifications during 45 thyroidectomies (i

Intraoperative Irrigation: Fluid Administration and Management Amidst Conflicting Evidence.

Surgeons use irrigation during open cavity procedures to improve their view of the patient's anatomy and to reduce the patient's risk of infection. However, there are no standard guidelines that recommend a specific type of fluid, additive, or volume of irrigation to use during open procedures. Intraoperative hypothermia can occur if irrigation fluids have not been warmed or have cooled before use, causing adverse patient outcomes. In addition, failing to manage (eg, measure and document) fluid volume accurately may affect clinical decision making and cause other complications. Perioperative p

Intraoperative management of accelerated idioventricular rhythm triggered by tourniquet deflation: a case report and mechanistic insights.

BACKGROUND: Accelerated idioventricular rhythm (AIVR) is a common reperfusion arrhythmia, but reports of AIVR triggered by tourniquet deflation in orthopedic trauma surgery are rare. CASE PRESENTATION: A 59-year-old male with left tibiofibular open fracture underwent emergency open reduction and internal fixation. Recurrent AIVR and severe hemodynamic instability occurred twice immediately after tourniquet deflation, with wide QRS complexes, atrioventricular dissociation, and transient ST-segment depression, which was initially misdiagnosed as acute myocardial ischemia. The arrhythmia resolve

Intraoperative Plasma Exchange for Sensitized Lung Transplantation Candidate Leads to Significant Perioperative Coagulopathy.

Sensitized lung transplantation recipients are at an increased risk for complications. Therapies such as plasma exchange (PLEX) exist to lessen the antibody burden in hopes of mitigating acute and chronic complications. Although the use of PLEX has been shown to be an effective immunomodulation for many diseases, its benefit in lung transplantation has been debated, and the fear of PLEX-induced coagulopathy has curtailed its use more broadly. Although the type of fluid used for exchange can determine the severity of coagulopathy, even the use of fresh frozen plasma (FFP) can disrupt the coagul

[Perioperative management of traumatic spinal cord injured patients after the acute phase].

OBJECTIVE: To understand the physiopathology and the consequences of a traumatic spinal cord injury after the acute phase. The aim is to describe the preoperative management of these patients and the various anaesthetic techniques available. DATA SOURCE: The data were retrieved from the PubMed data base, papers from the major French anaesthesiology and intensive care conventions, publications from the French medico-surgical encyclopaedia, combined with reference analysis of major articles in this field. RESULTS: In France, there are 40,000 patients with traumatic spinal cord injury. These le

Anesthetic considerations in porphyrias.

Four hereditary types of porphyria are now classified as acute porphyrias. Enzymatic defects result in accumulation of porphyrin precursors (usually ALA and PGB). The quantity of these precursors may be normal or slightly increased in latent periods but increase to toxic levels during a porphyric crisis. Iatrogenic induction of ALA synthetase by administration of certain triggers (classically barbiturates) is only one of several factors which contribute to porphyric crisis. Signs and symptoms of acute porphyric attack consist primarily of neurologic dysfunction, which occurs secondary to neuro

Anesthesia considerations for anorexia nervosa and bulimia nervosa.

Anorexia nervosa and bulimia are appearing with increasing frequency in preadolescents and adolescents. Eating disorders lead to significant physical changes related to starvation and/or purging. These patients should be specifically questioned about taking laxatives, diuretics, or amphetamines to lose weight. Subclinical abnormalities, coupled with the patient's denial, emphasize the need for careful physical examination and laboratory assessment. Preexisting electrolyte, cardiovascular, and thermoregulatory alterations must be recognized so that an appropriate anesthesia plan can follow.

Intraoperative Margin Assessment Methods in Oral Cavity Squamous Cell Carcinoma: A Systematic Review and Meta-Analysis.

OBJECTIVE: Currently, there is a lack of consensus regarding the superiority of specimen-based versus tumor bed-based intraoperative margin analysis in oral cavity squamous cell carcinoma (OCSCC). The purpose of this study is to perform the first meta-analysis examining intraoperative margin assessment methods in OCSCC with the goal of determining if there is a significant difference in patient outcomes between specimen-driven and tumor bed-driven margin analysis. DATA SOURCES: A comprehensive search of the literature was performed using PubMed, Embase, and Web of Science. REVIEW METHODS: St

Intraoperative Management of Hypoxemia with Anticoagulant-Free Venovenous Extracorporeal Membrane Oxygenation Amid Diagnostic Uncertainty.

Clinically relevant fat embolism syndrome (FES) is a potentially fatal intraoperative event, particularly in trauma patients. Rapid diagnosis is challenging in the perioperative setting, but treatment requires swift intervention and multidisciplinary coordination. This case report presents a polytrauma patient undergoing pelvic ring fixation who developed refractory hypoxia secondary to suspected FES. Successful initiation of anticoagulant-free venovenous extracorporeal membrane oxygenation (ECMO) intraoperatively, without definitive diagnosis, resulted in patient survival. Given the paucity o
