Candida parapsilosis keratitis.

PURPOSE: To present clinical, microbiologic, and histopathologic features of keratitis due to Candida parapsilosis. METHODS: Clinicomicrobiologic evaluation of four patients (four eyes) with culture-proven C. parapsilosis keratitis. The patients were evaluated for symptoms, visual acuity, clinical observations, microbiologic examination of corneal scrapings, and pathologic examination of corneal buttons. RESULTS: Three cases were observed after penetrating keratoplasty, and one case occurred after inhalation of corticosteroids. Clinical presentation of C. parapsilosis keratitis showed a grea…

[Syphilitic parenchymatous keratitis].

It shows a clinical observation about luetic bilateral keratitis at a young man 17 years old. The high-positive VDRL, the presence of the AT antigen, the bilateral low hearing perception and the left low hearing transmission associated with minimal dental malformations suggest the luetic congenital etiology of the disease. The AT positive-test at the patient's mother (which VDRL is negative) shows a tardive luetic infection during the pregnancy, that has determined the apparition at the fetus of congenital lately lues. The specific antiluetic treatment with Benzylpenicillinum kalicum and Predn…

[Bilateral acanthamoeba keratitis].

BACKGROUND: Given the protracted clinical course, diagnostic difficulties, frequent treatment failures and the increasing incidence, keratitis caused by free-living acanthamoeba represents a clinical challenge. PATIENT AND METHODS: A 37-year-old healthy female who wore gas-permeable contact lenses for ten years developed bilateral keratouveitis, pseudodentritic subepithelial infiltrates and corneal ring ulcers. Cultures were obtained from corneal scrapings, contact lenses and storage container. Medical treatment during the clinical course included propamidine isethionate, miconazole, ketocona…

Atypical mycobacterium keratitis.

We present two cases of Mycobacterium chelonae keratitis, both of which followed minor corneal trauma. One case initially showed improvement with medical therapy alone but eventually required penetrating keratoplasty. The second case required surgical intervention to provide tectonic support, but the infection resolved with antibiotic therapy.

Equine ulcerative keratitis.

Ulcerative keratitis is one of the most common vision-threatening diseases of the horse. Successful therapy, however, can be achieved in the majority of cases provided the diagnosis is made early and the treatment is specific for the disease. This article reviews the pathophysiology, clinical signs, and contemporary medical and surgical approaches to bacterial and fungal ulcerative keratitis in the horse.

Pseudallescheria boydii keratitis.

We treated a case of post-traumatic keratitis caused by the soil saprophyte, Pseudallescheria boydii. The injury was caused by a wood splinter which produced a perforating corneal laceration that was primarily repaired. Signs of corneal infection were not evident until the fourth postoperative week. The organism was eradicated by topical miconazole and natamycin. Subsequent penetrating keratoplasty combined with cataract extraction and intraocular lens implantation has achieved a good visual outcome.

Pseudallescheria Boydii keratitis.

Pseudallescheria Boydii is one of the rarer organisms known to cause mycotic keratitis, only 13 previous cases having been reported in the literature. What is believed to be the first case of Pseudallescheria Boydii keratitis successfully treated with topical Miconazole is reported here.

Pseudomonas cepacia keratitis.

Pseudomonas cepacia has recently become recognized as a virulent pathogen responsible for nosocomial infections in hosts with altered immunity. It has been implicated in endophthalmitis and conjunctivitis, and is resistant to conventional antipseudomonal therapy. No cases of P. cepacia keratitis have been reported in the literature. We report such a case in association with topical steroid and contact lens use following penetrating keratoplasty. In addition, we developed an experimental model of P. cepacia keratitis in the rabbit. P. cepacia should be considered as a cause of infectious kerati…

Fusarium moniliforme keratitis.

We report a case of keratomycosis from which fusarium moniliforme was isolated. After initial therapy without improvement, antifungal testing showed susceptibility to cyclopiroxolamine. After changing to this treatment the infection improved and an eventual healing took place with moderate corneal scarring.

Discoid lupus keratitis.

Two patients with long-standing discoid lupus erythematosus developed acute, unilateral, corneal stromal infiltration and edema. No evidence of infection was found, and both responded rapidly to topical corticosteroid therapy. To our knowledge, only one case of stromal keratitis associated with discoid lupus erythematosus has been published previously. We describe the first cases, to our knowledge, in which a satisfactory response to corticosteroid therapy is demonstrated.

Mycobacterium fortuitum keratitis.

Two of four cases of Mycobacterium fortuitum keratitis occurred after corneal surgery with contact lens wear, one was associated with extended contact lens wear alone, and one occurred after a foreign body injury. All cases were characterized by pain, conjunctival hyperemia, stromal inflammation, and ulceration. Diagnosis was made by culture and acid-fast staining of corneal scrapings. On the basis of published experience with amikacin for the treatment of nonocular M. fortuitum infections, three patients were treated with topical amikacin. Two patients responded clinically, but histopathologi…

Nocardia asteroides keratitis.

Nocardia asteroides has been reported as the cause of keratitis in only 7 cases and of other ocular disease in another 12 cases. We report a case of N. asteroides keratitis that presented 3 weeks after rural trauma and progressed despite trials of appropriate antibiotics. Seven weeks after the origianl injury a successful conjunctival flap was placed over the cornea. The morphology and the sensitivity testing of N. asteroides to antibiotics appears necessary before reliable information can be obtained for clinical use. Moreover, our case did not show the relatively benign course of other repor…
