PURPOSE: Keratoconus is a degenerating disease of the eye that results in an irregularly-shaped cornea. The etiology of the disease is unknown and the prognosis is difficult due to the variability in outcome. Keratoconus has been associated with eye rubbing, atopy, contact lens wear, as well as genetic conditions, such as Down's syndrome, Ehlers-Danlos syndrome, and Marfan's syndrome. Thinning of the cornea in keratoconus has been well studied and is documented to occur as a result of degradation of corneal collagen. The reason for this tissue degradation is unknown but has been hypothesized t…

A 35-year-old man with best corrected visual acuities of -18.00/+10.00×180 (6/60) OD and -10.00/+8.00×5 (6/36) OS. Bilateral steep central corneal thinning, paracentral ectasia and Vogts striae were present; normal fundi. Corneal topography disclosed 7.4 dioptres of irregular astigmatism in the central 3 mm with thinning (335 μm). Electroretinography (ERG) showed no response. There were no medical or environmental influences for his keratoconus. Occurrence of keratoconus and CSNB in the patient may represent a chance association, but keratoconus has not been previously linked with CSNB1 either…

PURPOSE: To report the case of a patient with high hyperopia and bilateral keratoconus.
METHODS: A high hyperopic patient was evaluated using Orbscan corneal topography and slitlamp biomicroscopy to confirm suspected keratoconus.
RESULTS: A male patient (32 years old) with a long history (10 years) of daily hydrogel contact lens wear with severe hyperopia was examined (RE +8.25-5.00 x 070 degrees , visual acuity [VA] 0.7 and LE +8.00-3.00 x 100 degrees , VA 0.9). The patient's Orbscan topography was compatible with bilateral asymmetric keratoconus (asymmetry of central dioptric power, irregu…

PURPOSE: This study aimed to investigate intraocular pressure (IOP) measurements taken using the dynamic contour tonometer (DCT) and the non-contact tonometer (NCT) in subjects with keratoconus.
METHODS: Intraocular pressure was measured in 20 keratoconus subjects and 20 age-matched control subjects using the DCT and NCT instruments. Central and off-centre measures were taken with the DCT in order to highlight any systematic errors associated with corneal biomechanical factors. Measures of anterior and posterior corneal topography and thickness were also taken in each subject.
RESULTS: No si…

Superior keratoconus is a rare form of keratoconus, in which the corneal apex is atypically superior. We report a 32-year-old man who presented with corneal hydrops in his right eye. Ophthalmic examination revealed corneal edema with Descemet's break involving the superior part of his right cornea. Superior steepening in the left cornea demonstrated by corneal topography was consistent with subclinical keratoconus.

We report a rare case of severe keratoconus and acute corneal hydrops in a 6-year-old girl. We describe the management of this challenging case and the outcome.

PURPOSE: Keratoconus is a chronic, noninflammatory disease of the cornea with onset in early adulthood. As these years are important to financial and social health, keratoconus may have more severe impact on quality of life than would be expected given its clinical severity. We examined the vision-related quality of life of patients in the Collaborative Longitudinal Evaluation of Keratoconus (CLEK) Study.
DESIGN: Cross-sectional study.
METHODS: The National Eye Institute-Visual Function Questionnaire (NEI-VFQ) was administered to 1166 CLEK Study patients at their first annual follow-up exami…

Keratoconus, a non-inflammatory thinning of the cornea, is a leading indication for corneal transplantation. For its causation, we propose a "Cascade Hypothesis" stating that keratoconus corneas have abnormal or defective enzymes in the lipid peroxidation and/or nitric oxide pathways leading to oxidative damage. The accumulation of oxidative, cytotoxic by-products causes an alteration of various corneal proteins, triggering a cascade of events, (i.e. apoptosis, altered signaling pathways, increased enzyme activities, fibrosis). This hypothesis is supported by biochemical, immunohistochemical a…

PURPOSE: To prospectively study the effects of the use of Intacs microthin prescription inserts for the management of keratoconus.
DESIGN: Prospective nonrandomized clinical trial.
METHODS: Thirty-three eyes of 26 keratoconus patients (17 males and 9 females) ages 21 to 51 years (mean age, 32 +/- 9.7 years) were included in the current study. All patients had clear central corneas and contact lens intolerance. Patients were excluded if any of the following criteria applied after the preoperative examination: previous intraocular or corneal surgery; history of herpes keratitis; diagnosed auto…

PURPOSE: Keratoconus is a noninflammatory disease characterized by thining and ectasia of the central cornea. The etiology and pathogenesis of keratoconus remain unclear. We studied the composition of basement membranes (BM) in normal, scarred and keratconus corneas to find out possible changes specific for keratoconus.
METHODS: Paraffin-embeded sections of normal, scarred and keratoconus corneas were immunostained with antibodys against type IV collagen, Fibronectin (Fn) and Laminin (Ln).
RESULTS: Keratoconus had increased staining of the epithelial BM for alpha 1-alpha 2 chain of type IV c…

PURPOSE: To assess the clinical feasibility of refractive live-epikeratophakia (L-EPI) for early keratoconus to cap, rather than reverse, the ectatic cornea.
SETTING: Eye Department, Martin-Luther-Hospital, Bochum, Germany.
METHODS: Twenty-seven consecutive cases with keratoconus stage I-II had L-EPI. Lenticules were obtained from eye-bank eyes. The lenticules were cut on the artificial anterior chamber bench of the Barraquer-Krumeich-Swinger set. Optical power was generated over refractive dies to achieve postoperative emmetropia. The lenticule was sutured into a peripherally undermined 7.0…

At a minimum, the refractive surgeon should use the patient's history, refraction, keratometry, and slit-lamp examination to determine the status of a patient's cornea. A diagnosis of keratoconus easily can explain a patient's symptoms or greatly alter a plan of action for refractive surgery. If the surgeon finds automated topography useful as a screening device for keratoconus, for patient education, and for the documentation of corneal status, it also should be included in the patient's examination. Indeed, most cases of keratoconus will be demonstrated by automated topography. However, the …
