Rush rod fixation of supracondylar fractures above total knee arthroplasties.

The authors report on 22 displaced supracondylar femur fractures and total knee arthroplasties treated with Rush rod (Berivon, Meridian, MI) fixation. The average age at the time of total knee arthroplasty was 67 years. Fractures occurred at an average of 5 years after surgery. In only five cases was notch in the anterior cortex noted. The follow-up period averaged 7 years. There were no nonunions. All knees healed between 3 and 4 months after surgery. Postoperative flexion averaged 108 degrees (preoperative average, 102 degrees). Only two knees demonstrated a flexion contracture before surger

Management Strategies for Tibial Periprosthetic Fractures After Total Knee Arthroplasty.

Tibial periprosthetic fractures following total knee arthroplasty are relatively uncommon, but represent a complex and increasingly encountered complication given the growing volume of primary and revision arthroplasty procedures. These injuries present unique challenges due to compromised bone stock, implant-related constraints, and patient-specific factors such as advanced age and osteoporosis. The location of the fracture, implant stability, bone quality, and the functional needs of the patient all influence management tactics. While operative intervention is frequently necessary for displa

Bilateral Simultaneous Patellae Fractures Following Knee Trauma: A Case Report.

Bilateral simultaneous fractures of the patellae were first documented in the literature in the early 19th century, and they remain uncommon today. The patella is the largest sesamoid bone in the body and plays an important role in the knee extensor mechanism. Isolated bilateral patellar fractures following trauma are exceedingly rare (2-3% of all patellar fractures) and most commonly occur with dashboard injuries or various pathological disorders. We report a case of a 43-year-old healthy African male who presented with bilateral simultaneous extensor mechanism disruption following knee traum

Periprosthetic fractures after medial unicompartmental knee arthroplasty: a narrative review.

INTRODUCTION: On rare occasions, fractures of the tibial plateau may occur after uni-compartmental knee arthroplasty (UKA) and account for 2% of total UKA failures. The purpose of this narrative review is to identify and discuss potential risk factors that might lead to prevention of this invalidating complication. MATERIALS AND METHODS: Electronic database of Pubmed, Scopus, Cochrane and Google Scholar were searched. A total of 457 articles related to the topic were found. Of those, 86 references were included in this narrative review. RESULTS: UKA implantation acts as a stress riser in the

Intraoperative Proximal Tibia Periprosthetic Fractures in Primary Total Knee Arthroplasty.

Intraoperative fracture of the proximal tibia is a rare complication of total knee arthroplasty (TKA) with few studies available reporting risk factors or prognosis. A review of our prospective joint registry was performed to determine the incidence and associated risk factors of intraoperative tibia fractures during primary TKA; 14,966 TKAs of all manufacturers were performed with 9 intraoperative tibia fractures. All fractures occurred in a single TKA design. There were 8,155 TKAs of this design performed with a fracture incidence of 0.110%. All but one fracture occurred on the medial tibial

Primary arthroplasty for management of osteoporotic fractures about the knee.

Primary cemented arthroplasty of the knee is a viable alternative to open reduction and internal fixation (ORIF) for treatment of osteoporotic fractures about the knee. This permits early return of knee function and weight bearing activity. Stemmed revision total knee arthroplasty implants and techniques are needed, which can be associated with complications of late loosening and periprosthetic fracture. However, for elderly sedentary patients who would not be expected to outlive the durability of the arthroplasty and with fracture patterns in which ORIF may be associated with poor outcomes, p

Knee osteochondral fractures in skeletally immature patients: French multicenter study.

INTRODUCTION: Femoral or patellar osteochondral fractures complicating patellar dislocation are more frequently observed in adolescents than in adults. These incidental lesions of vulnerable joint cartilage are often neglected in the initial phase, which is regrettable given their good capacity for healing after repositioning. The objective of this study was to investigate the characteristics and analyze the results of repositioning these fractures in skeletally immature patients. MATERIAL AND METHODS: This retrospective multicenter study grouped 14 patients and knees (seven females). The mea

A functional below-the-knee cast for tibial fractures. 1967.

An experimental method of treating tibial fractures with use of the below-the-knee total-contact weight-bearing cast is discussed. The success of this method appears to depend on the careful application and molding of plaster to the fractured leg after the swelling has been reduced to a minimum and a closed reduction has been obtained. Good alignment is essential, but slight shortening or overriding of fragments can be accepted with the expectation that little or no increase in the amount of shortening will occur. The results obtained indicate that this type of cast not only holds reduction we

"Floating knee" injuries: ipsilateral fractures of the femur and tibia.

Ipsilateral fractures of the femur and tibia have been called "floating knee" injuries and may include combinations of diaphyseal, metaphyseal, and intra-articular fractures. These are often high-energy injuries and most frequently occur in the polytrauma patient. Many of these fractures are open, with associated vascular injuries. Surgical stabilization of both fractures and early mobilization of the patient and the extremity produce the best clinical outcomes. The use of a radiolucent operating room table and the introduction of retrograde intramedullary fixation of femoral fractures have fa

Internal fixation of supracondylar fractures after condylar total knee arthroplasty.

Condylar buttress plates were applied in six patients with total knee arthroplasties and supracondylar fractures. According to the criteria of Cain, 50% of the patients had a satisfactory result with an average follow-up period of 17 months. Time to union (full weight bearing) averaged 14 weeks. The final coronal femorotibial alignment averaged 5 degrees of valgus, whereas preoperative valgus had averaged 8 degrees. Four patients were pain free and the two others had minimal pain that did not interfere with daily activity. Knee motion averaged 97 degrees. All patients were ambulating; five of

Treatment of supracondylar fractures following prosthetic arthroplasty of the knee.

Fifteen supracondylar fractures of the femur in patients with ipsilateral total knee arthroplasties were treated between 1975 and 1982. Three groups were identified for analyzation of treatment and end result. The average follow-up period after fracture was 18 months, with a range of ten to 48 months. Group I had four patients treated by closed reduction, cast immobilization, and early weight-bearing. At follow-up evaluation, three had a decrease in knee rating score, and one required a corrective osteotomy. Group II had eight patients treated by traction followed by cast or cast-brace immobil

Surgical Treatment for Avulsion Fractures of the Anterolateral Ligament Associated with Periarticular Fractures of the Knee.

The existence and anatomy of the anterolateral ligament (ALL) of the knee are a somewhat controversial topic in orthopaedic surgery. The fixation of the avulsion fracture of the ALL (Segond fracture), associated with periarticular knee fractures, is rarely given much consideration while the major fracture fragments are reconstructed. This study aims to confirm the existence of ALL and evaluate the clinical outcomes of surgical management for avulsion fractures, involving its insertion, when associated with periarticular knee fractures. Twenty-three patients (16 males and 7 females) with avulsi…

Clinical instability of the knee and functional differences following tibial plateau fractures versus distal femoral fractures.

BACKGROUND: Fractures of the knee account for about 6% of all trauma admissions. While its management is mostly focused on fracture treatment, it is not the only factor that defines the final outcome. OBJECTIVES: This study aimed to study objective and subjective outcomes after proximal tibial versus distal femoral fractures in terms of knee instability and health-related quality of life. PATIENTS AND METHODS: This retrospective, cross-sectional, cohort study was carried out on 80 patients with either isolated proximal tibial (n = 42) or distal femoral (n = 38) fractures, who underwent open

[Fractures of the lower extremities in childhood : Part 1: fractures of the thigh and in the proximity of the knee].

Fractures in children are a major challenge in everyday clinical practice. The decision about the correct treatment of fractures follows the consideration of individual factors. The age of the child and the resulting residual growth play an essential role. This article aims to facilitate a structured approach with respect to examination, diagnostics and treatment in clinical practice. This first part of this series deals with fractures of the thigh and in the proximity of the knee joint in children. The peculiarity of pediatric bone lies in its growth, which results not only in a much faster h

Osteoarthritis knee with patellar fracture in the elderly: single-stage fixation and total knee arthroplasty.

Periarticular fractures that occur in a previously osteoarthritic knee, especially in an elderly patient, will be associated with other comorbidities and is a treatment challenge. Early mobilization is a key treatment principle in the geriatric population. We present a case report of an elderly diabetic patient with osteoarthritis of the knee joint who sustained a patellar fracture and was treated with single-stage primary total knee arthroplasty and fixation of the patellar fracture.

Robotic-Assisted Total Knee Arthroplasty for Distal Femur Fracture with Lateral Knee Osteoarthritis.

Introduction. Open reduction and internal fixation (ORIF) of compound fractures around the knee in elderly patient raise concerns about long-term postoperative external fixation and complications. Total knee arthroplasty (TKA) has been proposed as an alternative solution. We report a case where robotic-assisted (RA) TKA was used to treat lateral knee osteoarthritis (OA) with distal femur fracture. Case Presentation. A 90-year-old female visited our hospital with complications of sustained knee pain after a fall at home. Fracture line from the trochlea to the intercondylar notch was diagnosed o
