[Injuries of the extension apparatus of the knee joint].

The article analyses problems of the diagnosis, the choice of the therapeutic tactics, and the results of operative treatment in 478 patients. Operation undertaken as early as possible after the injury is indicated in fresh fractures of the patella with displacement of the fragments, ruptures of the tendon of the quadriceps femoris muscle and the patellar ligament. The authors prefer stable fixation of patellar fractures by means of a "tightening" wire loop and a "blocking" wire loop in injuries of the patellar ligament. Additional external immobilization is not employed. Study of the late-ter

Non-surgical management of overuse knee injuries in runners.

Seventy-three injured runners (45 men, 28 women) who presented with 83 running-induced overuse knee injuries were managed conservatively. The regimens utilized focused on quadriceps and hamstring muscle retraining and the control of functional overpronation. Patello-femoral pain syndrome (42 cases) was by far the most common problem. The next most common problems were iliotibial band friction syndrome (10), patellar tendonitis (9) and popliteal tendonitis (5). The most frequent etiological factor was a sudden increase in training mileage. Following the initial visit and at regular intervals th…

Rating systems in the evaluation of knee ligament injuries.

Many different methods of evaluating disability after knee ligament injury exist. Most of them differ in design. Some are based on only patients' symptoms. Other include patients' symptoms, activity grading, performance in a test, and clinical findings. The rating in these evaluating systems can be either numerical, as in a score, or binary, with yes/no answers. Comparison between a symptom-related score and a score of more complex design showed that the symptom-related score gave a more differentiated picture of the disability. It was also shown that the binary rating system gave less detaile

Occult knee ligament injuries associated with femoral shaft fractures.

Recognition of ligamentous injuries of the knee in the acute phase is essential for optimum treatment and prognosis. A multitraumatized patient with a fractured femur and occult knee ligament damage presents a difficult diagnostic challenge. This paper retrospectively reviews 52 patients with 54 midshaft femoral fractures. The high incidence (48%) of ipsilateral knee ligament damage, particularly of a severe nature (30%) evident at followup (mean 24.5 months) is alarming. In the 26 cases of knee ligament damage, the mean time from injury to documentation of instability is 12.8 months. Motor ve

[Ligamentous knee injuries in children: Focus on cruciate ligaments].

In cruciate ligament injuries in childhood and adolescence, bony avulsion injuries are common in addition to purely ligamentous ruptures. The injury patterns and mechanisms, continued longitudinal growth and psychosocial factors must be taken into account in the diagnostics and treatment. This article describes the most important techniques of surgical and nonoperative management, the associated rehabilitation protocols, as well as options for primary and secondary prevention.

Review of Extensor Mechanism Injuries in the Dislocated Knee.

Knee dislocations, aka multiligamentous injuries, are uncommon but devastating injuries often following high-energy trauma. Tears of the major knee stabilizers are well documented; however, injuries of the knee extensor mechanism are less commonly reported. The extensor mechanism is comprised of the patella, patellar tendon, and quadriceps tendons. Magnetic resonance imaging (MRI) is the preferred imaging modality of internal derangements of the knee due to its excellent soft tissue contrast. In this article, we will discuss the normal imaging findings of the extensor mechanism and review abno

Medial-Sided Injuries in the Multiple Ligament Knee Injury.

Multiligament knee injuries (MLKI) are complex and challenging to treat. The posteromedial corner (PMC) structures are commonly torn in MLKI. A thorough and systematic evaluation is imperative to avoid a missed diagnosis and for planning treatment. With several structures injured, the treatment method (operative vs. nonoperative, repair vs. reconstruction), availability of allografts, timing of surgery, and rehabilitation are some of the factors that have to be considered in the decision-making. Persistent valgus instability because of untreated or not healed medial collateral ligament (MCL) t…

Knee Anterior Cruciate Ligament Injuries: Common Problems and Solutions.

Anterior cruciate ligament (ACL) reconstruction is one of the most commonly performed procedures in the United States. Although complications are rare in ACL surgery, failure to appreciate them can lead to significant patient morbidity in the short and long terms. More common complications in ACL reconstruction include tunnel malposition, infection, tunnel osteolysis, fixation failure, fracture, arthrofibrosis, graft site morbidity, and deep vein thrombosis or pulmonary embolism. Tunnel malposition is the most common technical error in ACL reconstruction leading to failure. Proper planning dur

Apophyseal and epiphyseal knee injuries in the adolescent athlete.

OBJECTIVES: In the context of pediatric sports injuries, the epiphyseal and apophyseal knee fractures represent rather peculiar lesions. The most frequently involved anatomical area is the knee. The peculiar function of the physis and the need to preserve their integrity, makes choosing what treatment methods to employ very important. Objective of this study is to assess the kind and the effectiveness of the most suitable treatment in the apophyseal and epiphyseal knee lesions occurring in the adolescents. MATERIALS AND METHODS: From 2006 to 2011, were treated 41 patients (34 M-7 F) between t

[Management of extensor mechanism injuries following total knee arthroplasty].

Extensor mechanism injuries in total knee arthroplasty include disruption of the quadriceps tendon, disruption of the patellar tendon, and/or patellar fractures. While these injuries are rare, they are a devastating complication to manage. This review summarizes the anatomy of the extensor mechanism, risk factors for extensor mechanism injuries, and the prevalence and diagnosis of extensor mechanism injuries. In addition, this review outlines non-operative and operative management options. A new surgical approach for the reconstruction and augmentation of the extensor mechanism with the use of

Sports-related knee injuries in female athletes: what gives?

Knee injuries occur commonly in sports, limiting field and practice time and performance level. Although injury etiology relates primarily to sports specific activity, female athletes are at higher risk of knee injury than their male counterparts in jumping and cutting sports. Particular pain syndromes such as anterior knee pain and injuries such as noncontact anterior cruciate ligament (ACL) injuries occur at a higher rate in female than male athletes at a similar level of competition. Anterior cruciate ligament injuries can be season or career ending, at times requiring costly surgery and re…

Management of peroneal nerve injuries associated with knee dislocations.

Peroneal nerve palsy after knee dislocation is a serious problem, and neurolysis at the time of knee reconstruction does not always result in return of peroneal nerve function. In light of the current state of microneural surgery, and the potential to reconstruct lengthy nerve defects, this review addresses the appropriate evaluation of peroneal nerve palsy and provides recommendations regarding surgical treatment.

[Sport injuries of the extensor mechanism of the knee].

Injuries of the extensor mechanism of the knee occur frequently during sport activity. For a successful treatment they must be diagnosed early. Besides osseous structures the patellar tendon, the patella, the quadriceps muscle and tendon, retinacula and bursae can be affected. After initial clinical examination there are different noninvasive imaging modalities available for assessment of bone, cartilage, ligaments, tendons and soft tissue. Conventional radiographs are still the basic imaging tool for the clinician. Additional information about the osseous status is provided by computed tomogr

Activity-related knee injuries and pain in athletic adolescents.

By collecting data from 45 students at a ski high school, we found that a total of 73% of the students reported activity-related pain/injuries of the knee. Sixty-one percent had overuse injuries, 27% malalignment, and 12% had indistinct knee pain. Females suffered more knee pain/injuries (88%) than males (57%). Significantly higher Q-angle degrees were recorded for females (16) than for males (10). "Jumper's knee" was found in all competitive students with a KT manual maximum difference (MMD) of 3 mm or more (mean 4 mm), with a hard endpoint, whereas this was less common among the other compet

Low-velocity knee dislocation with sports injuries. Treatment principles.

Knowing that the injured MCL and PCL can heal and that the injured ACL and lateral side, predictably, will not heal are the bases for the treatment approach to these injuries. (1) Allow MCL healing nonoperatively. (2) Allow PCL healing to occur as long as PCL laxity is 2+ or less (reconstruct the PCL acutely if posterior drawer is > 2+ initially). (3) Initially delay ACL treatment and reconstruct later, if needed posterior drawer. (4) Perform acute lateral side repair to reattach structures to their distally torn site.

Significance of arthroscopy in children with knee joint injuries.

Clinical and arthroscopic findings after knee joint injury were evaluated in 35 children aged 4-15 years. Arthroscopy revealed a rupture of the anterior cruciate ligament in 9 patients; two lesions were only partial. Eight children suffered from patellar dislocation. Eleven patients showed minimal synovial and cartilaginous lesions as the only cause of hemarthrosis, hydrarthrosis and clinical symptoms. Three had chondral lesions due to a dysplastic patella. The clinical diagnosis was confirmed in only 12 of 35 cases. Arthroscopic therapy was performed in 15 and open surgical intervention in 16

Knee-ligament injuries associated with leg fractures. Prospective study.

It has been proved in 25-35% of the cases that knee-ligament injuries are associated with fracture of the femoral diaphysis. No such association has been confirmed between leg fractures and knee ligaments. In order to find out if this is a coincidence, a prospective study was conducted on 229 patients who had undergone operations for leg fractures at various locations and of variable intensity. It was established in 41 cases (17.34%) that the leg fracture was associated with knee-ligament injuries, resulting in joint instability. A significantly higher percentage of associated ligament lesions
