[Damage control laparotomy in blunt abdominal injury].

PURPOSE OF THE STUDY: Damage control surgery is one of the major advances in surgical techniques used in polytrauma patients in the past 25 years. It is based on a foreshortened pre-operative examination during ongoing resuscitation which includes surgical intervention in an injured patient with severe hypotension due to haemorrhage. In this seven-year retrospective study the authors evaluate the results of damage control laparotomy in patients with injury to the abdominal organs, retroperitoneum and pelvis. MATERIAL AND METHODS: The evaluated group included 21 patients with blunt abdominal t…

[Bloodless emergent laparotomy for acute abdominal syndrome].

Bloodless laparotomy (BL) is defined as an opened exploration of the abdominal cavity that yields negative results, i.e., "provides no information as to the cause of the clinical and paraclinical symptoms responsible for prompting the surgical investigation". The authors report a retrospective study spanning January 1975 to December 1989, on the incidence of and mortality associated with emergent BL in patients with acute abdominal syndrome, with the intent of reducing its frequency. Over this period, 24 BL occurred in 3480 emergent laparotomies, i.e., 0.63%. These involved 7 men, 5 women, 5 b…

Ovarian carcinoma follow-up: US versus laparotomy.

Ultrasound (US) was compared with surgical findings in 98 patients with carcinoma of the ovary undergoing follow-up laparotomy after chemotherapy. US had an overall accuracy of 94% in the pelvis, with only small and sheetlike lesions escaping detection. It was more sensitive than clinical examination. Overall accuracy for the liver was 91% with very few false-positive results. For the peritoneal cavity in general, however, accuracy was low, with even large masses escaping detection. Ascites is reliably detected (accuracy of 97%) but is a poor indicator of peritoneal involvement. US is a useful…

Suprapubic paramedian laparotomy for equine abdominal cryptorchidism.

The history of techniques for abdominal equine cryptorchidectomy is briefly reviewed. The technique of suprapubic paramedian laparotomy as used since 1955 at the University of Liverpool veterinary field station is described in detail and an account given of the authors' experience with the technique in nearly 200 cases. It is concluded that the approach to the abdominal cavity is easy, that the testis itself is generally readily located and removed and that controlled repair of the incision is possible. Serious post-operative complications are rare and the danger of post-operative prolapse of …

The Emergency Laparotomy Protocol: A Retrospective Cohort Study of Morbidity and Mortality After the Introduction of a Perioperative Protocol for Emergency Laparotomies.

BACKGROUND: Emergency laparotomy and its perioperative care is associated with high morbidity and mortality. At our institution, 30-day mortality was approximately 12% in 2013-2017. We introduced a perioperative protocol for emergency laparotomies in 2019. METHOD: Retrospective cohort of emergency laparotomies 2019-2022. Reoperations, trauma and converted laparoscopies (appendectomy and cholecystectomy) were excluded. Primary outcome was mortality at 30, 90, and 180 days. Secondary outcomes were postoperative complications (Clavien-Dindo; CD) and hospital-based outcomes. RESULTS: Six hundred…

Standard of Care and Outcomes of Primary Laparotomy Versus Laparotomy in Patients with Prior Open Abdominal Surgery (ReLap Study; DRKS00013001).

BACKGROUND: Patients undergoing relaparotomy are generally underrepresented in trials, despite how common the procedure is in clinical practice. The aim of this trial was to determine standard of care and gain evidence of intra- and postoperative outcomes for patients undergoing relaparotomy compared to primary laparotomy. METHODS: In this single-center controlled clinical trial, adult patients scheduled for elective abdominal surgery via relaparotomy or primary laparotomy were consecutively screened for eligibility. The perioperative course was monitored prospectively in five study visits du…

ECLAPTE: Effective Closure of LAParoTomy in Emergency-2023 World Society of Emergency Surgery guidelines for the closure of laparotomy in emergency settings.

Laparotomy incisions provide easy and rapid access to the peritoneal cavity in case of emergency surgery. Incisional hernia (IH) is a late manifestation of the failure of abdominal wall closure and represents frequent complication of any abdominal incision: IHs can cause pain and discomfort to the patients but also clinical serious sequelae like bowel obstruction, incarceration, strangulation, and necessity of reoperation. Previous guidelines and indications in the literature consider elective settings and evidence about laparotomy closure in emergency settings is lacking. This paper aims to p…

[Prediction and prevention of fascial dehiscence after laparotomy].

OBJECTIVE: To evaluate the efficacy of fascial dehiscence prevention suture in patients with perioperative risk factors regarding the incide … cial dehiscence in postoperative period. In the third group (n=22), original preventive suturing of laparotomy was applied. The validity of differences in continuous variables was assessed using the Kruskal-Wallis test. Categorical variables were analyzed using chi-square test, as well as Dunn's and Fisher's post-hoc tests. Differences were significant at p< …

Preliminary Experience with Mini-Laparotomy Cholecystectomy in Jos.

BACKGROUND: Different techniques have been described for removing a diseased gall bladder; however, cholecystectomy via the laparoscopic approach is currently regarded as the gold standard. Laparoscopic surgery services are not widely available in low- and middle-income countries and mini-laparotomy cholecystectomy may be a suitable alternative in such circumstances. This technique achieves cholecystectomy with a smaller incision and affords the advantages of the laparoscopic approach. OBJECTIVE: We report our experience over a 2-year period of 24 consecutive patients from two hospitals who u…

[Mortality rate after acute laparotomy can be reduced].

Acute abdomen is a common cause of admission to hospital. Emergency laparotomy is associated with a significant morbidity and mortality due to deranged physiology and surgery-induced stress. Damage control laparotomy is on the rise as an operative strategy for the septic abdomen as well as for trauma laparotomy but lacks definition in the non-trauma setting. Principles of perioperative care in elective surgery are currently applied to the emergency abdominal surgery patients and should be further studied in the future to reduce morbidity and mortality.

Laparotomy for blunt abdominal trauma-some uncommon indications.

Trauma laparotomy after blunt abdominal trauma is conventionally indicated for patients with features of hemodynamic instability and peritonitis to achieve control of hemorrhage and control of spillage. In addition, surgery is clearly indicated for the repair of posttraumatic diaphragmatic injury with herniation. Some other indications for laparotomy have been presented and discussed. Five patients with blunt abdominal injury who underwent laparotomy for nonroutine indications have been presented. These patients were hemodynamically stable and had no overt signs of peritonitis. Three patients …

A mass seen under gastroscopy "disappeared" during laparotomy.

We describe here a case of 36-year-old man with a hepatic cavernous hemangioma that was misdiagnosed as a gastric submucosal tumor (SMT) with endoscopic ultrasound (EUS) and CT scan. On gastroscopy, a submucosal tumor was found on the cardia of the stomach. Based on EUS and abdominal CT scan, the lesion was diagnosed as a gastric duplication cyst. The patient underwent gastroscopy after receiving laparotomy at the median point of the abdomen. A spherical mass arising from the left triangular ligament of the liver region was found. There was no abnormal finding at the cardia of the stomach.

Laparoscopy versus laparotomy in management of abdominal trauma.

BACKGROUND: A majority of abdominal injuries (AIs) are associated with shock, hence most of the patients are hemodynamically unstable, which … onsisting of 1,363 patients, had conventional routine investigations following which they underwent laparotomy for confirmatory diagnosis and definitive management. The second group, consisting of 1,332 patients, underwent diagnostic laparoscopy in addition to the conventional investigations in the first group; 411 patients of this group had therapeutic lapa …

Mini-laparotomy cholecystectomy: technique, outcomes: a prospective study.

BACKGROUND: The last decades have been characterized by a rapid growth in minimally invasive techniques for acute and chronic cholecystitis. The aim of our study was to analyze 10 years of experience with the mini-laparotomy cholecystectomy. METHODS: From 1994 to 2004, we performed 2295 mini-laparotomy cholecystectomies, including 1028 patients with acute and 1267 patients with chronic cholecystitis. There were 1780 women and 515 men. We utilized a special surgical tool kit with a system of circular and small hook-retractors incorporating an illuminator and long surgical instruments. Our surg…
