[Sporotrichosis as the cause of a leg ulcer].

A young man presented at Lugala Lutheran Hospital (Tanzania) with an ulcer on his lower leg which had developed over the past 9 weeks. Subcutaneous nodules and plaques were found all the way up to his groin; this observation prompted a strong suspicion that the patient had the lymphocutaneous form of sporotrichosis which had not been seen at this hospital before. The diagnosis was confirmed histopathologically. The patient was then treated with saturated solution of potassium iodide.

["Difficult" venous ulcers of the legs].

The aim of the study was to analyze the diagnostic and surgical criteria in cases of "critical" venous leg ulcers. This report describes the management of 200 patients with trophic lesions of the lower limbs. Patients with venous leg ulcers were identified with the aid of ultrasonography and echocolor Doppler. Among these patients, according to the chronic evolution of the lesions, we identified all those with critical venous leg ulcers. The management of the lesions involved both medical and fairly aggressive surgical treatment with the use of general anaesthesia and deep surgical toilette of…

In-patient management of leg ulcers.

OBJECTIVE: To review in-patient management of leg ulcers and to compare our experiences with those from other similar centres. DESIGN: A retrospective study. SETTING: University of Benin Teaching Hospital, Nigeria. PATIENTS: Forty five patients hospitalised with leg ulcers. RESULTS: There were 27 males and 18 females (MF 3:2). The age range was 8-90 years (mean 32.5 years). The commonest sites were around the ankle(55.6%) and the distal third of the leg (40%). Trauma, diabetes mellitus and sickle-cell disease accounted for the commonest aetiological factors(35.6%, 22.2% and 17.8% respectiv…

[Contact sensitization in leg ulcers].

BACKGROUND: The leading complication of leg ulcers is the development of peri-lesional eczema. The etiological diagnosis is often difficult due to the large number of topical agents used. PATIENTS AND METHODS: Fifty patients, 35 women and 15 men, mean age 67.8 years, range 37-91 years, with leg ulcers of variable duration (15 days to 32 years, median 2 years) underwent explorations using the standard EECDRG battery in search of contact allergies. Eighteen potential allergens and the topical agents employed were tested. Readings were made at 48, 72, 96 or 120 hours. RESULTS: Sensitization rat…

Malignancy in chronic leg ulcers.

OBJECTIVE: To evaluate the frequency of malignant ulcers in patients presenting with leg ulcers. DESIGN: A descriptive study from data collected between July 1988 and June 1995 from 981 patients (2448 ulcers) attending a leg ulcer clinic. SETTING: A specialised leg ulcer clinic at a tertiary teaching hospital. SUBJECTS: 43 patients with 55 malignant skin lesions. OUTCOME MEASURES: Tissue biopsies in ulcerated lesions that suggested malignancy or were not responding to appropriate treatment. RESULTS: Forty-three patients were found to have malignant lesions on the legs, giving a frequency …

[Leg ulcers in systemic diseases].

The collagen vascular diseases and vasculitis, in particular, are occasionally associated with chronic, relapsing lower extremity ulcerations. Different mechanisms can induce such ulcerations, and an understanding of the type of ulcerations is important in the differential diagnosis of patients with leg ulcerations in general, and management of these patients in particular. In this review, the authors analyze the various mechanisms of the leg ulcerations in these patients and their treatments: vasculitis, thrombosis, traumatisms, calcinosis, panniculitis, pyoderma gangrenosum, infections, and …

Leg ulceration in rheumatoid arthritis.

Chronic leg ulcers occur in 1% of the adult population with considerable associated morbidity, and community costs estimated at up to 600 million pounds p.a. in the UK. Leg ulcers appear to be relatively common in patients with RA and are widely believed to be resistant to treatment. Such ulcers are often attributed to vasculitis, but little is known of the occurrence of other potential aetiological factors. This article reviews the epidemiology, natural history and aetiology of chronic leg ulcers in RA as well as the therapeutic options available.

Hydroxyurea and lower leg ulcers.

We report four cases of cutaneous lower leg ulcers associated with hydroxyurea treatment for myeloproliferative disorders. This association has been reported in one other series of patients who had chronic myelogenous leukemia and were treated with hydroxyurea. A review of the literature and survey of possible pathogenetic mechanisms is presented.

Bacterial studies of leg ulcers.

--Bacteriologic studies of 30 chronic leg ulcers were studied in 18 adult patients before and after treatment with a local cleansing agent, Debrisan.--Results show that all ulcers, whether clinically inflamed or not, are secondarily infected.--The majority of ulcers grew more than 1 type of organism, up to 6 different types in 1 single ulcer.--The use of Debrisan reduces the types of organisms and colony counts in ulcers, allowing healthy granulation and epithelialization to proceed normally.--Control of infection reduces surrounding local edema and erythema.--By reducing types of organisms an…

Arterial Leg Ulcers in the Octogenarian.

Arterial leg ulcers are a debilitating sequela of chronic ischemia, and their management, particularly in the octogenarian, is an immense challenge. ALUs are frequently a manifestation of end-stage peripheral arterial disease, and their presence portends a high morbidity and mortality. Management primarily relies on restoration of flow, but in the geriatric population, interventions may carry undue risk and pathologies may not be amenable. Adjunctive therapies that improve quality of life and decrease morbidity and mortality are therefore essential, and understanding their benefits and limitat…
