OBJECTIVE: To study the characteristics of cataract extraction and surgical techniques in cases following vitrectomy.
METHODS: Extra-capsular extraction/phacoemulsification of cataract and intra-ocular lens implantation were performed under vitreous irrigation.
RESULTS: Of 37 cases (40 eyes), 31 eyes (77.5%) had obtained post-operative visual acuity > or = 0.1, 12 eyes (30.0%) > or = 0.3. The complications included nuclear dislocation into vitreous cavity (1 eye) and vitreous hemorrhage (1 eye) in the operation, and rubeosis iridis (1 eye) and pupillary capture (1 eye) after the operation.

BACKGROUND: Placement of a secondary intraocular lens (IOL) in a child may be considered in children with congenital monocular cataracts who have had complete opacities removed early in life and who later become contact lens intolerant, in eyes that have received trauma which precluded placement of a primary IOL, and in young adults who have bilateral aphakia who become resistant to use of contact lenses or spectacles.
METHODS: Clinical records of all children in our practice who received a secondary IOL between January 1988 and December 1994 were reviewed. Indications, biometry, type of proc

6 months after senile extra-capsular cataract extraction and posterior chamber intraocular lens (IOL) implantation, Goldmann gonioscopy was performed on 30 eyes having undertaken the surgery. Peripheral anterior synechiae (PAS) were found in 17 eyes (56%). PAS overlying the positions of 20 lens haptics were observed in 15 eyes. It is easier to develop PAS in eyes with vertically fixed haptics than in eyes with horizontally fixed ones. The causes and the consequences of PAS were discussed. It is suggested that during the follow-up, routine gonioscopic observation after posterior chamber IOL imp

The results and the postoperative complications of 107 secondary intraocular lens implantations were reviewed over a two-year period. In 84.1% of the cases, postoperative visual acuity was the same or better than preoperative visual acuity with aphakic glasses or contract lenses. The most frequent postoperative complications were secondary glaucoma, six cases (5.6%); cystoid macular edema, five cases (4.6%); and bullous keratopathy, three cases (2.8%).

Intraocular lenses were implanted in 16 eyes of 13 patients with congenital cataract, and visual progress was plotted using a preferential-looking technique. Initial surgery was by lens aspiration with preservation of the posterior capsule, and subsequent posterior capsulotomy without anterior vitrectomy. Poly-HEMA posterior chamber lenses were used, usually as a primary procedure but in four cases as a secondary procedure after contact lens failure. No serious complications were encountered. Most eyes achieved a very significant visual improvement, and none were worse than preoperatively. Res

We used 21 cadaver eyes to study transsclerally sutured, ciliary sulcus-fixated intraocular lens implantation. Results showed that transscleral sutures should exit the sclera less than 1 mm posterior to the corneoscleral limbus for true ciliary sulcus fixation. The relationship of the ciliary sulcus to the overlying posterior surgical limbus differed in the vertical and horizontal meridians; needles that pierced the ciliary sulcus after being passed perpendicularly through the sclera entered the sclera 0.83 +/- 0.1 mm posterior to the posterior surgical limbus in the vertical meridians and 0.4

Due to continued improvement in lens design, lens manufacturing, and surgical technique, the incidence of complications of cataract surgery with intraocular lens (IOL) implantation have decreased in recent years. Complications may be categorized into operative and postoperative, both early and late onset. Operative complications include posterior capsule rupture with or without vitreous loss, loss of all or part of the lens nucleus, iris damage, and stripping of Descemet's membrane. Early postoperative complications include pupillary block, hyphema, elevation of the intraocular pressure associ

This paper reports a study of postoperative refraction and eikonometry of 50 patients who had unilateral cataract extraction with implantation of a pupil-supported intraocular lens. The mean postoperative aniseikonia (+/- SD) was 1.97 (+/- 1.82). A statistically significant relationship was shown between anisometropia and aniseikonia. Despite aniseikonia of up to 7.8 per cent there were no diplopia problems as the visual system exhibits a high degree of tolerance. It is concluded that aniseikonia can be controlled to within clinically acceptable limits by the simple calculation of intraocular

A microsurgical modification of Binkhorst's "open sky" technique of extracapsular cataract extraction was developed for intraocular lens implantation. Clear visualization of both anterior and posterior capsule is stressed and risky aspiration is minimized. Thirteen cases operated upon in the first year are described. Synechiae to the haptics occurred when mydriasis was not prompt. All but one obtained useful naked acuity. Binocularity depended chiefly upon the acuity of the opposite eye. Pre-calculation of lens power reduced the spread of postoperative refractive errors although deviation was

Specular microscopy was used to study the corneal endothelium in a series of patients undergoing phacoemulsification with and without intraocular lens implantation by one experienced surgeon. Cell loss after phacoemulsification alone was variable, but the mean was 18%, with a median of 11%. The patients undergoing phacoemulsification plus lens implantation showed a mean cell loss of 26%, with a median of 17%. This is considerably lower than has been reported with standard intracapsular extraction plus lens implantation. The decreased endothelial damage is attributed to the technique of inserti

PURPOSE: To report a new intraocular lens (IOL) and an IOL implantation concept, the bag-in-the-lens implantation technique, designed to prevent posterior capsule opacification (PCO).
SETTING: The University of Antwerp, Department of Ophthalmology, Edegem, Belgium, and the Netherlands Ophthalmic Research Institute, Department of Morphology, Amsterdam, The Netherlands.
METHODS: After identical curvilinear capsulorhexes are created in both the anterior and posterior capsules, the capsules are inserted in a flange of the IOL, thus the term bag-in-the-lensas opposed to the currently used lens-in

A 51-year-old man with ankylosing spondylitis had intracapsular cataract extraction and anterior chamber lens implantation in the left eye in 1985. He had localized sclerokeratitis around the corneal section and a protracted anterior uveitis in the immediate postoperative period. He developed a spontaneous extrusion of the implant in 1991 (six years later). The extrusion was through a site 1.5 mm posterior to the previous surgical section. There was no corneal decompensation or corneal melt and no history of trauma. Signs of localized scleritis were present. It is suggested that anterior chamb

The literature has been investigated on the prevalence of preexisting and complicating glaucoma in intraocular lens surgery. Few exact data are available. The problem of the management of preexisting primary or postoperative secondary glaucoma in pseudophakia is discussed, in particular the inevitable restrictions the intraocular lens imposes on medical therapy and eventual glaucoma surgery. Preexisting glaucoma should therefore be a relative contraindication to intraocular lens surgery.

40 cases of intraocular lens implants are reported. 33% had an optimal post-operative vision of greater than 1.0; 56% and larger than or equal to 0.9 and 84% 0.5 or better. After a postoperative follow-up of 1 month to 32 months, mean 15.1 months, the patients were all satisfied with the pseudophakic eye, and no endothelial corneal dystrophy, chronic uveitis or glaucoma was detected. Several minor post-operative complications were seen, most without consequence. One case had recurrent iridial haemorrhages.

Intraocular lens implants have been around for more than 25 years. On the verge of a major breakthrough to ever widening popularity, there is, however, a general lack of understanding of the pertinent optical principles. A critical comparison is made between this new method and conventional methods of correcting aphakia. From this it appears that the visual needs of the patient are a decisive factor in the selection of the method of correction. Moreover, the full potential of intraocular lens implants can only be realized by appropriate selection of the power of the implant in each situation.
