During the 1989-1991 year period, leprosy patients with various ulcers, attending the surgical O.P.D. at CJIL, Agra were seen. Of these, 64 cases were found to be infested with maggots. Live maggots were collected in all cases from different sites viz. nasal cavity, hand, great toe and second toe. It was possible to rear the maggots into flies in 53 out of 64 cases. In 11 cases the maggots did not survive and died in the early part of their life cycle. Four different types of flies were identified viz. Sarcophaga haemorrhoidalis, Chrysomya bezziana, Callitroga americana and Musca domestica.

Ten out of the twenty-five lepromatous leprosy patients studied showed clinical evidence of involvement of the tongue, and they presented with various symptoms like loss of taste, stiffness of tongue, bleeding, pain etc. Various types of lesions ranging from small nodule to granuloma formation, ulceration, macroglossia and fissured cracked tongue were noted. The tongue lesions were found to be related to the severity of leprosy.

Studies including close contacts of leprosy patients were performed; an inverse relationship between degree and length of exposure to Mycobacterium leprae (MI) and the magnitude of the in vivo specific immune response (Mitsuda reaction) was observed. Serum immunoglobulins were estimated by the single radial immunodiffusion technique and the results were compared among various groups of leprosy patients. Most of the serum immunoglobulin classes are increased in lepromatous leprosy patients. Controls from different regions in Thailand and even from two populations in the same region show different results. This can lead to different interpretations of results in leprosy patients and may explain apparent discrepancies in earlier studies of serum immunoglobulins in leprosy.

Eighty-one placentae from women with leprosy and 17 placentae from healthy controls were subjected to a detailed macroscopic, light microscopic, ultrastructural, immunopathological, microbiological and biochemical study. Iritis in leprosy is still a significant cause of blindness and visual impairment in this disabling disease. Evidence is accumulating from clinical observations and from pathological and pharmacological studies that the chronic iritis of lepromatous leprosy may have a neuroparalytic origin. A hypothesis derived from this evidence is presented in order to stimulate further research into the pathogenesis of this condition with a view to developing new methods of management and prevention.

Serum immunoglobulins IgG, IgA and IgM were estimated in 22 lepromatous (LL) patients, 28 tuberculoid (TT), 9 borderline tuberculoid (BT), and 8 borderline lepromatous (BL), and compared with 50 normal healthy adult males belonging to a low socio-economic class. Occurrence of cold lymphocytotoxins has been observed in 59 leprosy sera. In 46% of the patients, cold lymphocytotoxins were present whereas in helathy controls only 13% showed such antibodies. The highest incidence of alloantibodies was detected in lepromatous leprosy. Levels of autoantibodies, immunoglobulins and C3 were tested in parallel without finding any significnat correlations.
