T- cell prolymphocytic leukemia - a rare case.

T- cell Prolymhocytic leukemia (T-PLL) is a rare mature post-thymic T-cell malignancy that is usually reported in the elderly and follows an aggressive course. A 68 year old male presented with a history of weakness and weight loss of two months duration. Clinical examination revealed pallor, enlarged cervical and axillary lymph nodes and splenomegaly. He also had a maculo- papular skin rash. There was marked leucocytosis, anemia and thrombocytopenia (WBC 445 x 103 sub/ml, Hb 8.5 gm/dl, Platelet 25 x 103 sub/microl) with 60% prolymphocytes in the peripheral blood. Bone marrow was hypercellular

PTCL: lessons from adult T-cell leukemia.

Peripheral T-cell lymphoma (PTCL) is a neoplastc disease of peripheral T-lymphocytes/NK cells, including PTCL unspecified, anaplastic large T-cell lymphoma (ALCL), IBL-like T-cell lymphoma (AILD), intestinal T-cell lymphoma (ITCL) and adult T-cell leukemia/lymphoma (ATL). The incidence of PTCL is relatively uncommon although its incidence shows significant variations in the geographical regions and racial populations. In Asia, its incidence is high due to HTLV-I infection in Japan. Molecular mechanisms of oncogenesis of PTCLs remain unknown. Therefore, analyses of ATL will give us a clue to cl

[Adult T-cell leukemia/lymphoma. Therapeutic aspects].

UNLABELLED: CONVENTIONAL CHEMOTHERAPY: Complete remission of aggressive ATL (acute or lymphomatous forms) can be achieved in about 40% of the patients with conventional chemotherapy, but early relapse and infectious complications is the rule. For painless and chronic ATL, chemotherapy does not appear to be useful and can aggravate the immunodepression. NEW THERAPEUTIC APPROACHES: Encouraging results have been obtained with a combination regimen using an antiretroviral agent (AZT) and interferon alpha. Response rate has been high with good tolerance. In responders, the survival rate is better

Allelotype analysis of adult T-cell leukemia.

Allelotype analysis of adult T-cell leukemia (ATL) was undertaken for the first time to identify chromosomal loci relevant to the development of acute/lymphomatous ATL. Loss of heterozygosity (LOH) was screened using 94 highly polymorphic microsatellite markers, distributed among all nonacrocentric, autosomal chromosomes. In each of the 22 cases, DNA obtained from their leukemic cells in acute/lymphomatous phase was compared with their constitutional DNA from mononuclear cells in chronic or remission phase. Allelic losses of at least on one chromosome arm occurred in 91% of the cases (20 indiv

CD44 expression in T-cell lymphoblastic leukemia.

1. CD44 is an adhesion molecule expressed by B and T lymphocytes that mediates cell attachment to extracellular matrix components and specific cell surface ligands. In normal process of T-cell development, CD44 can be implicated in homing of bone marrow-derived T-cell precursors into the thymus. In hematopoietic malignancies, CD44 and other adhesion molecules can mediate the behavior of neoplastic cells such as metastatic migration. In the leukemic process, CD44 expression is correlated with increased numbers of circulating blasts and it is present at other sites such as the central nervous sy

Lupus nephritis with adult T cell leukemia.

A 64-year-old Japanese male patient with lupus nephritis associated with adult T cell leukemia (ATL) is described. Percutaneous renal biopsy demonstrated findings consistent with membranous lupus nephritis. To our knowledge, this is the first case of lupus nephritis complicated by ATL, suggesting that human T cell leukemia virus type I may be correlated not only to outbreaks of ATL but also to lupus nephritis/systemic lupus erythematosus.

Cancer superimposed on adult T-cell leukemia.

Forty-three patients with adult T-cell leukemia (ATL), admitted to the hospital from 1982 to 1987, were studied. Five of those were found to have additional cancer; two of the five had two additional kinds of cancers besides ATL. During the same period, in contrast, the authors encountered only three patients with some superimposed cancer among 155 cases of hematologic malignancies which had no association with human T-cell lymphotropic virus type I (HTLV-I). These facts strongly suggest that HTLV-I infection is not only associated with ATL, but it is likely to increase the incidence of other

Adult T-cell leukemia/lymphoma in Brooklyn.

Fifteen patients with adult T-cell leukemia/lymphoma (ATLL) were identified in less than a two-year period in the Crown Heights-Bedford Stuyvesant section of Brooklyn, NY. All patients were black; nine patients originated from the Caribbean islands and six from the southern United States. Two of the patients were father and daughter, the first evidence of familial occurrence in the United States. Their clinical course was similar to that of previously described patients with this disorder. To our knowledge, these 15 patients represent the largest series of ATLL reported in the United States. Monoclonal gammopathies in adult T-cell leukemia.

Age and sex distributions of monoclonal gammopathy were studied for 12,196 healthy controls and 2056 patients with various malignancies. The frequency of monoclonal component in adult T-cell leukemia (ATL) patients was found to be much higher than that in patients with other malignancies and in healthy controls. This suggested that the occurrence of ATL and monoclonal gammopathy is not coincidental. Three cases of ATL with monoclonal gammopathy are also reported.

Human T-cell leukemia virus specific antigens.

Type I and type II human T-cell leukemia viruses (HTLV) contain in their genomes three structural genes, gag, pol, and env and a putative transforming gene, lor. Using a living cell membrane immunofluorescence assay, antibodies to surface-expressed env gene products of HTLV-MA have been detected in healthy carriers living in the HTLV-I endemic areas, and in patients with adult T-cell leukemia/lymphoma (ATLL). Similar antibody reactivities were detected in patient MO, from whom HTLV-II was first isolated, and in a proportion of patients with acquired immune deficiency syndrome (AIDS). Amino aci

Progress in adult T cell leukemia research.

The clustering incidence of adult T cell leukemia (ATL) is found in adult populations who were born in southwest Japan and showed impairment of cell-mediated immunity; all patients with ATL contain antibody for specific ATL associated antigen in their sera. This ATLA positive T cell leukemia is morphologically characterized by leukemic cells in various sizes showing unique nuclear polymorphism. The neoplastic cells bear common immunocytologic markers with inducer T cell subset among the peripheral T2 cell group and functionally show the suppressor inducer activity. Histologic features of lymph

Chromosome 14q+ in adult T-cell leukemia.

Cytogenetic studies were performed on leukemic cells from two patients with adult T-cell leukemia. A 14q+ marker chromosome was found in the peripheral blood leukocytes from patient No. 1 and in a leukemic T-cell line (MT-1) derived from the peripheral blood of patient No. 2. The 14q+ resulted from a t(12;14) in patient No. 1 and from a t(Y;14) in patient No. 2 with a break point at 14q32 in each case. In addition, the leukemic cells from patient No. 1 showed a t(1;7) and a 9q-, while the MT-1 line had numerous structural abnormalities. Thus, it is clear that a 14q+ translocation is not restri

Pseudoclitoromegaly from acute T-cell lymphoblastic leukemia.

A 7-year-old girl presented with painful genital enlargement, which was first believed to be clitoromegaly of hormonal origin. However, on the physical exam the clitoris was not visible and the prepuce and labia minora were enlarged and tender. Magnetic resonance imaging demonstrated an infiltrative abnormal signal with restricted diffusion involving the enlarged clitoris and adjacent soft tissues of the prepuce and labia minora, confirming a nonhormonal infiltrative malignancy. The same abnormal signal was present in enlarged inguinal lymph nodes, the kidneys, and an anterior mediastinal mass
