Nine patients had magnetic resonance imaging studies to aid in the diagnosis of wrist pain in the midcarpal region. Six had positive magnetic resonance imaging studies with a decrease in signal intensity from the area of the lunate, one had an occult ganglion, and two had negative studies. Among the six patients with a diagnosis of Kienböck's disease by magnetic resonance imaging, four patients had collapse of the lunate on plain radiographs, and the other two patients had normal wrist radiographs. Magnetic resonance imaging studies showed a substantial decrease in the signal intensity on T1-

Lunate and perilunate dislocations, although somewhat uncommon, are serious injuries and can be easily missed. The mechanism of injury is hyperextension of the wrist, often associated with a fall on the outstretched hand (FOOSH) injury, falls from height, or motor vehicle crash with a high-energy traumatic injury to the wrist. Perilunate dislocations typically present with pain and swelling over both the dorsal and volar aspects of the wrist and limited range of motion of the wrist. Perilunate dislocations result in disruption of the relationship between the lunate and the capitate, whereas lu

We report a case of avascular necrosis of the scaphoid, lunate and triquetrum in a young 21-year-old patient, after a purely ligamentous peri-lunate dislocation of the wrist. He presented with a Mayfield III peri-lunate dislocation after a road traffic accident and underwent an open reduction and internal fixation. Post-operatively, the scapho-lunate gap widened after removal of the temporary K-wires, and he gradually developed avascular necrosis of the scaphoid, lunate and triquetrum, and osteoarthritis of his wrist. We present this unusual case of simultaneous avascular necrosis of multiple

PURPOSE: To determine if the absence (type I lunate) or presence (type II lunate) of a medial hamate facet on the lunate affects the radiographic characteristics of patients presenting with Kienböck disease.
METHODS: A retrospective review was performed on all patients evaluated at our institution from 2002 to 2010 with a diagnosis of Kienböck disease confirmed on plain radiographs in concert with magnetic resonance imaging (MRI) and/or bone scan. Study groups consisted of patients with type I versus type II lunates, as determined by radiographs, MRI, and/or computed tomography. Measured vari

PURPOSE: To describe quantitatively the protrusion of the volar part of the lunate facet of the distal radius articular surface anterior to the volar metaphyseal cortex, which is susceptible to fracture and can be difficult to control with plates and screws alone.
METHODS: Quantitative anatomic measurements of 48 3-dimensional computed tomography scans of the distal radius were analyzed to quantify the anatomy of the volar part of the lunate facet of the distal radius. The measurements were scaled to account for overall bone size. Male and female anatomies were compared.
RESULTS: The height

The main accepted principle to treat Kienböck disease is to decompress the lunate. Radius shortening is the most used technique. Three transverse osteotomies of the radius are described: neutral shortening osteotomy, lateral closing wedge osteotomy, and medial closing wedge osteotomy. Shortening the radius decompress the lunate and more or less the scaphoid. This deviates axial constraints toward ulna and triangular fibrocartilage complex. But the ulnar wrist is not able to support important axial constraints. The authors propose a solution to decompress only the lunate and not the scaphoid. T

Aseptic osteonecrosis of the lunate bone is a rare disease of unknown aetiology. The mechanical theory of shearing of the lunate underneath the radius is generally accepted, especially in the presence of a short ulna, even if it is insufficient. The diagnosis is exclusively radiographic and requires, in the early stages, the use of MRI which is more sensitive and specific than bone scan. Plain X-rays allow the disease to be classified into various stages, which determine the therapeutic indications. Many treatments have been proposed, but the follow-up is often inadequate. Simple immobilizatio

INTRODUCTION: Lunate dislocation is a rare and serious injury, frequently due to high-energy trauma such as a fall from height or a motor vehicle accident. Lunate dislocation is usually in the volar direction; however, dorsal dislocation of the lunate is extremely rare.
PRESENTATION OF CASE: We report a case of 55-year-old male who sustained a high-energy trauma; he was hit by a car while crossing the street. The patient was referred to department of Plastic Surgery for left wrist dorsal dislocation of the lunate and bilateral multiple carpal bone fractures. He was managed by open reduction a

BACKGROUND: Kienböck's disease is idiopathic lunate avascular necrosis, which may lead to lunate collapse, abnormal carpal motion and wrist arthritis. The current study aimed to assess the outcomes of treating stage IIIA Kienböck's disease by a novel technique of limited carpal fusion via partial lunate excision with preservation of the proximal lunate surface and scapho-luno-capitate (SLC) fusion.
MATERIALS AND METHODS: We conducted a prospective study of patients with grade IIIA Kienböck's disease managed with a novel technique of limited carpal fusion comprising SLC fusion with preservatio

A wide spectrum of conditions can trigger signal alterations in the lunate bone. From vascular complications such as Kienböck's disease to axial microtrauma in ulnocarpal impaction syndrome, the lunate signal is an essential indicator of carpal health. Many of the pathologies described in this review rely on magnetic resonance imaging for analysis; however combined assessment of patient history, clinical symptoms, and imaging is often the only way to find the correct diagnosis. Standard radiography serves as the first-line modality for diagnostic assessment in most patients, but morphological

BACKGROUND: The lunate bone plays a crucial role in wrist stability and movement; therefore, any disruption to its integrity could significantly impact the function of the hand and subsequently, the quality of life. The variation in lunate morphology has been associated with several wrist pathologies. Although limited studies have assessed the association between lunate morphology and wrist pathology, none has been done on the Saudi population.
MATERIALS AND METHODS: A cross-sectional study was conducted at a hospital, in Saudi Arabia. A total of 284 wrist radiological reports were included i

Among 70 patients treated by hemodialysis for renal failure and operated for carpal tunnel syndromes, 17 presented with bone and joint lesions. Seven of them had destructive arthropathies of the distal interphalangeal joint, bilateral in 6 cases. Two radiological stages are described. Radiolucent lesions of the carpal bones were present in all the patients ("carpal amyloidosis"). The most frequent sites were around the radius-scaphoid-lunate and scaphoid-lunate joint spaces. Scaphoid-trapezoid-trapezium arthropathy was frequent with dynamic instability in one case. One patient had a pathologic

PURPOSE: Various skeletal and soft tissue abnormalities have been identified in Madelung deformity and have been hypothesized to play a causal role in its progressive symptomatology; however, our pathological understanding of these changes remains limited. In this study, we biomechanically assessed the Madelung deformity wrist, using 4-dimensional computed tomography imaging.
METHODS: Nine Madelung deformity wrists (5 patients; age, 24 ± 5 y) and 18 healthy wrists (9 volunteers; age, 28 ± 3 y) underwent 4-dimensional imaging during flexion-extension motion and radioulnar deviation. Carpal kin

The pattern of subchondral bone density has been considered to reflect the stress distribution that occurs under physiologic loading conditions. To determine the force distribution through the wrist joint with Kienböck's disease in living subjects, we applied a computed tomography osteoabsorptiometry and investigated the subchondral bone density pattern across the radio-carpal joint of 6 normal subjects and 10 patients suffering from Kienböck's disease (Lichtman's stage IIIA, 5 patients; stage IIIB, 5 patients). A single density maximum was found in each scaphoid and lunate fossa in all normal

Scapholunate dissociation is a rotatory subluxation of the scaphoid with widening of the gap between the scaphoid and lunate. It is caused by rupture of the ligaments between radius, scaphoid, lunate and capitate bones. The scaphoid has lost its stabilizing function as a bridge between the proximal and distal carpal row, so that the wrist shows the typical "concertina" deformity of dorsal instability. The scapholunate angle measures about 100 degrees (more than 70 degrees). The subluxation of the scaphoid and lunate with the wide gap between these bones is best treated by primary repair of the

Lunate coronal fracture is a rare injury, associated with impact along the capitate and lunate axis; neutralization of this axial force is important for satisfactory patient outcomes. This report presents a case of a 51-year-old man with a lunate coronal fracture associated with coronal fractures in the proximal carpal bones and volar and proximal dislocation of the distal carpal row. Open reduction and internal fixation with a dorsal distraction bridge plate were performed. Solid bony union was achieved at the fracture site and the patient resumed full activities without difficulty. Use of di

The aim of this study was to assess the outcome of arthroscopic lunate fossa resection arthroplasty for patients with Kienböck's disease, previously managed without operation, and a non-functional lunate fossa. For adult patients with relapse or exacerbation of pain from Kienböck's disease 2 years after the onset of symptoms, arthroscopic removal of lunate fossa cartilage was performed to reach bleeding bone, if the cartilage in the lunate fossa was non-functional and the scaphoid fossa was intact. Sixteen patients (mean age 35 years; range 27-56) were included. At a mean follow-up of 39 month

UNLABELLED: A case of palmar dislocation of the scaphoid and lunate which where dissociated from each other is reported with a 3.5 years follow up. A 32 years old man fell while reading his motorcycle. He landed on his left hand dorsiflexed. There was no skin dilaceration and neuro-vascular status of his hand was intact. X-rays showed a palmar dislocation of the scaphoid and lunate with a large gap between the two bones. The lunate was also completely dissociated from the triquetrum and the capitate. Distal pole of the scaphoid remained in contact with the trapezium. The patient was taken to t
