Henipavirus evidence gaps: a Rapid Research Needs Appraisal.

INTRODUCTION: Henipaviruses have been identified as priority pathogens by the WHO for research and development. We applied a rapid research needs appraisal (RRNA) review methodology to systematically identify existing henipavirus evidence on clinical characteristics, immune response, transmission, risk factors, medical countermeasures and associated social and behavioural factors, to inform research prioritisation, and strategies to strengthen our preparedness and response for henipaviruses to protect populations and improve outcomes. METHOD: We searched PubMed, Ovid Embase, Cochrane Library,…

Preventing equestrian injuries. Locking the stable door.

The medical and sports literature databases were searched for equestrian sports-related injury published in English since 1980, together with conference abstracts and discussions with equestrian sporting bodies. This literature was critically reviewed, with emphasis on measures to prevent or control injury i.e. countermeasures. While there is considerable literature available on the epidemiology of injury incurred in most equestrian sports, there is little on the prevention of these injuries. Case-control or other studies evaluating the effectiveness of the countermeasures suggested by authors

Ebola virus disease candidate vaccines under evaluation in clinical trials.

Filoviruses are the etiological agents of two human illnesses: Ebola virus disease and Marburg virus disease. Until 2013, medical countermeasure development against these afflictions was limited to only a few research institutes worldwide as both infections were considered exotic due to very low case numbers. Together with the high case-fatality rate of both diseases, evaluation of any candidate countermeasure in properly controlled clinical trials seemed impossible. However, in 2013, Ebola virus was identified as the etiological agent of a large disease outbreak in Western Africa including al

Mismatch of Supply and Demand: Marburg Virus Disease Outbreaks Need Countermeasures But Will Not Provide Opportunity for Clinical Trials.

In many ways, Marburg virus disease resembles the more well-known Ebola virus disease: The clinical syndrome is similar, management of outbreaks is similar, and the fear engendered in the population experiencing the outbreak is similar. However, diagnostics, therapeutics, and vaccines to manage patients and outbreaks are not similarly available. These have been developed but not yet approved, as outbreaks have not provided the opportunity to establish an evidence base for regulators to evaluate their use in humans. The history of outbreaks of Marburg virus disease suggests that this opportunit

Rat Models of Partial-body Irradiation with Bone Marrow-sparing (Leg-out PBI) Designed for FDA Approval of Countermeasures for Mitigation of Acute and Delayed Injuries by Radiation.

The goal of this study was to develop rat models of partial body irradiation with bone-marrow sparing (leg-out PBI) to test medical countermeasures (MCM) of both acute radiation syndrome (ARS) and delayed effects of acute radiation exposure (DEARE) under the FDA animal rule. The leg-out PBI models were developed in female and male WAG/RijCmcr rats at doses of 12.5-14.5 Gy. Rats received supportive care consisting of fluids and antibiotics. Gastrointestinal ARS (GI-ARS) was assessed by lethality to d 7 and diarrhea scoring to d 10. Differential blood counts were analyzed between d 1-42 for the …

On-Demand Patient-Specific Phenotype-to-Genotype Ebola Virus Characterization.

Biosafety, biosecurity, logistical, political, and technical considerations can delay or prevent the wide dissemination of source material containing viable virus from the geographic origin of an outbreak to laboratories involved in developing medical countermeasures (MCMs). However, once virus genome sequence information is available from clinical samples, reverse-genetics systems can be used to generate virus stocks de novo to initiate MCM development. In this study, we developed a reverse-genetics system for natural isolates of Ebola virus (EBOV) variants Makona, Tumba, and Ituri, which hav…

Meta-analysis of functional genomics studies reveals conserved cellular pathways required by viruses of pandemic concern.

The coronavirus disease 2019 pandemic illustrated the need to develop medical countermeasures against emerging infectious diseases. As viruses rely on cellular machinery for replication, host-directed antivirals (HDAs) may complement conventional antiviral strategies in a manner that offers broad-spectrum efficacy, including against novel viruses, and a potentially higher barrier to viral escape. Despite their potential, HDAs are under-represented as therapeutics, partly due to a lack of consensus on druggable host targets. To address this, we have performed a meta-analysis of 62 functional ge…

Development of an inhalational Bacillus anthracis exposure therapeutic model in cynomolgus macaques.

Appropriate animal models are required to test medical countermeasures to bioterrorist threats. To that end, we characterized a nonhuman primate (NHP) inhalational anthrax therapeutic model for use in testing anthrax therapeutic medical countermeasures according to the U.S. Food and Drug Administration Animal Rule. A clinical profile was recorded for each NHP exposed to a lethal dose of Bacillus anthracis Ames spores. Specific diagnostic parameters were detected relatively early in disease progression, i.e., by blood culture (∼37 h postchallenge) and the presence of circulating protective anti…

Differential pathogenesis of closely related 2018 Nigerian outbreak clade III Lassa virus isolates.

Nigeria continues to experience ever increasing annual outbreaks of Lassa fever (LF). The World Health Organization has recently declared Lassa virus (LASV) as a priority pathogen for accelerated research leading to a renewed international effort to develop relevant animal models of disease and effective countermeasures to reduce LF morbidity and mortality in endemic West African countries. A limiting factor in evaluating medical countermeasures against LF is a lack of well characterized animal models outside of those based on infection with LASV strain Josiah originating form Sierra Leone, ci…

The Special Pathogens Research Network: Enabling Research Readiness.

The 2013-2016 epidemic of Ebola virus disease (EVD) that originated in West Africa underscored many of the challenges to conducting clinical research during an ongoing infectious disease epidemic, both in the most affected countries of Guinea, Liberia, and Sierra Leone, as well as in the United States and Europe, where a total of 27 patients with EVD received care in biocontainment units. The Special Pathogens Research Network (SPRN) was established in the United States in November 2016 to provide an organizational structure to leverage the expertise of the 10 Regional Ebola and Other Special

The Arenaviridae Family: Knowledge Gaps, Animal Models, Countermeasures, and Prototype Pathogens.

Lassa virus (LASV), Junin virus (JUNV), and several other members of the Arenaviridae family are capable of zoonotic transfer to humans and induction of severe viral hemorrhagic fevers. Despite the importance of arenaviruses as potential pandemic pathogens, numerous gaps exist in scientific knowledge pertaining to this diverse family, including gaps in understanding replication, immunosuppression, receptor usage, and elicitation of neutralizing antibody responses, that in turn complicates development of medical countermeasures. A further challenge to the development of medical countermeasures …

Hemorrhagic Fever Disease in STAT-1 Knockout Mice Infected with Lujo Virus.

Lujo virus (LUJV) is an arenavirus that causes Lujo Hemorrhagic Fever (LHF), a viral hemorrhagic fever that emerged in a 2007 outbreak in Zambia and South Africa with an 80% case fatality rate and evidence of human-to-human nosocomial transmission. There are no approved medical countermeasures for LHF, although several screens have identified lead antiviral compounds. The lack of accessible animal models limits the development of lead compounds and characterization of broadly protective anti-arenavirus compounds such as ribavirin for the treatment of LHF. Here, we present preliminary data char…

Specimen and data sharing to advance research and development on Zika virus.

For diseases with epidemic potential, specimen and data sharing is crucial for sustained research and development of medical countermeasures such as diagnostics, therapeutics, and vaccines. In the case of Zika virus, although a global framework for specimen and data sharing to advance research and development is highly desirable, challenges related to legal, ethical, and intellectual property issues persist. Since the 2015-16 Zika virus outbreak, regional laboratory networks and research partnerships have made some progress in specimen and data sharing among some Zika virus-endemic countries. …

A Longitudinal Analysis of Memory Immune Responses in Convalescent Crimean-Congo Hemorrhagic Fever Survivors in Uganda.

Evaluating the adaptive immune responses to natural infection with Crimean-Congo hemorrhagic fever (CCHF) virus (CCHFV) in human survivors is critical to the development of medical countermeasures. However, the correlates of protection are unknown. As the most prevalent tick-borne human hemorrhagic fever virus with case fatality rates of 5%-30% and worldwide distribution, there is an urgent need to fill these knowledge gaps. Here, we describe adaptive immune responses in a cohort of Ugandan CCHF survivors via serial sampling over 6 years. We demonstrate persistent antibodies after infection an…

Manned interplanetary missions: prospective medical problems.

The present review aimed to suggest approaches to prospective medical problems related to the health maintenance of space crews during future manned interplanetary, particularly Martian, missions up to 2-3 years with a possible stay on a planet with gravity different from that on Earth. The approaches are based on knowledge so far obtained from our analysis of the medical support of long-term orbital flights up to one year, as well as on the consideration of specific conditions of interplanetary missions. These specific conditions include not only long-term exposure to microgravity, but also a
