Latent Non-typhoidal Salmonella in Macrophages as a Potential Cause of Persistent Appendicitis.

Non-typhoidal Salmonella (NTS) infection typically causes gastroenteritis but can sometimes lead to persistent or recurrent abdominal pain due to mesenteric lymphadenitis. Here, we present a rare case of a 10-year-old Japanese boy who experienced persistent abdominal pain caused by appendicitis in addition to mesenteric lymphadenitis due to NTS. Initially, he exhibited symptoms of gastroenteritis, including transient fever, persistent lower abdominal pain, and loose stools. Stool cultures revealed Salmonella O7, leading to treatment with fosfomycin. However, his abdominal pain persisted and wa

Abdominal Kikuchi-Fujimoto lymphadenopathy: an uncommon presentation of a rare disease.

A 34-year-old man presented to our hospital with a 5-day history of progressive abdominal pain and fever. A CT scan identified extensive mesenteric lymphadenopathy. Initial diagnostic tests were inconclusive. Abdominal lymph node biopsy showed histiocytic necrotising lymphadenitis, compatible with Kikuchi-Fujimoto disease (KFD). This benign and self-limiting disease generally resolves following supportive treatment. In this case, remission occurred within 3 weeks of initial presentation. KFD is a very uncommon cause of lymphadenopathy, and selective mesenteric involvement is rare. Definitive d

Abdominal complications of infectious mononucleosis.

Infectious mononucleosis, a systemic illness caused by the Epstein-Barr virus, is seen frequently by primary care physicians. Mononucleosis affects several organ systems, and, within the abdomen, there can be splenic involvement, hepatitis, mesenteric lymphadenopathy, hyperplasia of gut-associated lymphoid tissue, pancreatitis, and transient malabsorption. Life-threatening abdominal complications require prompt recognition and intervention. Other abdominal complications, though worrisome, are usually short-lived and resolve without sequelae.

An outbreak of infection by Yersinia pseudotuberculosis in nonhuman primates.

Between October 1970 and June 1971, at the National Center for Primate Biology, Yersinia pseudotuberculosis of serotypes I-B and III was isolated from 9 monkeys (one during life and 8 at necropsy) of the following species: Macaca cynomolgus, Macaca nemestrina, Macaca radiata and Cercocebus fulliginosus. All these animals had characteristic gastrointestinal lesions consisting of superficial erosions or ulcerations with masses of gram-negative coccobacilli and an acute inflammatory exudate. Involvement of mesenteric nodes, livers and spleens by similar lesions was common. A more granulomatous re

Unilateral pulmonary miliary tuberculosis with abdominal tubercular lymphadenitis in an apparently immunocompetent adult: A case report.

Atypical forms of tuberculosis (TB) are gaining recognition, challenging traditional diagnostic criteria classically linked to chronic cough, lethargy, anorexia, evening pyrexia, and weight loss in immunocompromised patients. We present the case of a 23-year-old immunocompetent man from rural central India who developed unilateral pulmonary miliary TB with concurrent abdominal TB lymphadenitis. The patient presented with a two-month history of chronic cough and diffuse abdominal pain. Initial chest radiograph revealed unilateral, right-sided miliary opacities, while abdominal ultrasonography i

[Acute abdominal pain in a 8-year-old girl with Epstein-Barr infection].

Abdominal pain is uncommon in patient with Epstein-Barr infection and is usually attributed to an enlargement of the liver or spleen. We report on an 8-year-old girl with a pseudoperitonitis due to a mesenteric lymphadenitis associated with Epstein-Barr infection. Outcome was favourable without surgery. We review the different causes of abdominal pain occurring during Epstein-Barr infection.

Hypereosinophilic paraneoplastic syndrome in a cat with intestinal T cell lymphosarcoma.

A 10-year-old, neutered female, domestic shorthair cat was presented with a recent history of weight loss, polydipsia, diarrhoea and vomiting. On physical examination, intestinal thickening and mesenteric lymph node enlargement were apparent. Clinical investigations revealed peripheral blood eosinophilia, eosinophilic abdominal effusion and eosinophilic mesenteric lymphadenitis. There was a temporary response to treatment with glucocorticoids but signs progressed and the cat was euthanased. On histology, there was eosinophilic infiltration and fibroplasia of intestine and mesenteric lymph node

Intestinal and pulmonary mycotic lymphadenitis in cattle.

Among 4877 slaughtered cattle with tuberculosis-like lesions in lymph nodes, 94 cases (1.9 per cent) revealed fungal hyphae on histopathological examination. The survey period was 12 years and most of the affected animals (greater than 77 per cent) were beef cattle. Affected nodes were mesenteric in 84 cases (89.4 per cent), mediastinal and/or bronchial in seven cases (7.4 per cent), and in three cases (3.2 per cent), both mesenteric and mediastinal nodes were affected. The incidence of mycotic lymphadenitis was unrelated to year of study or season. Eighty-two of the cases were re-examined his

Avian mycobacteriosis in three horses.

The clinical, bacteriologic and pathologic findings of three adult horses suffering from avian tuberculosis are presented. Chronic weight loss and hypoproteinemia were pertinent clinical abnormalities in all three horses. Gross pathologic lesions were characterized by chronic enterocolitis with mesenteric lymphadenopathy in two horses and hepatic granulomas in the third horse. The microscopic diagnoses were chronic, non-caseating granulomatous enterocolitis, and necrotizing, non-mineralizing granulomatous hepatitis, respectively. All three horses had granulomatous lymphadenitis of mesenteric l

Recurrent peritonitis with ascites as the predominant manifestation of systemic lupus erythematosus.

We describe a patient with abdominal pain and ascites, mesenteric lymphadenitis and peritoneal panniculitis. Initially her ANA was negative. The abdominal pain recurred again three years later and in between the two episodes she had had skin rash, alopecia, arthralgia, and positive Coombs' test-haemolytic anaemia. Her ANA became positive a few years after the initial episode.

Mycobacterium bovis infection in a horse with granulomatous enterocolitis.

A 2-year-old dappled Percheron horse had a wasting condition that did not respond to antibiotic treatments and ultimately resulted in death. Thickening of the wall of the large colon and enlargement of the mesenteric lymph nodes were observed at postmortem examination, along with the presence of pinpoint whitish foci in the liver. Microscopic examination of affected tissues revealed diffuse chronic granulomatous enterocolitis, granulomatous mesenteric lymphadenitis, and multifocal granulomatous hepatitis. The DNA extracted from paraffin-embedded intestinal and lymph node samples was analyzed u

Yersiniosis as a surgical disease.

An infection with Yersinia strains may develop with the picture of acute appendicitis. Laparotomy will then not reveal an acutely inflamed appendix, but will often show mesenteric lymphadenitis or acute terminal ileitis. These conditions have been observed in four patients. The differential diagnostic possibilities are discussed. It is pointed out that acute terminal ileitis is practically never transformed into chronic terminal ileitis (Crohn's disease).

Septicemia and peritonitis in a colony of common marmosets (Callithrix jacchus) secondary to Klebsiella pneumoniae infection.

Six common marmosets from a colony of 50 died over a period of 3 weeks, with the predominant finding of gram-negative bacterial septicemia. Four of these animals died peracutely; the other two were found when they were moribund, and they subsequently died despite clinical intervention. Gram-negative bacterial rods were present in the blood vessels of stained tissues from five of the six marmosets. Three marmosets also had severe fibrinopurulent peritonitis. In addition, one of the marmosets with peritonitis also had purulent mesenteric lymphadenitis with large colonies of gram-negative bacteri

Ultrasonography in suspected acute appendicitis in childhood-report of 1285 cases.

OBJECTIVE: For the assessment of the diagnostic value of real-time ultrasonography (US) in children with suspected acute appendicitis (a.A.) the results of graded compression US are compared with clinical and histological final diagnoses. METHODS: In a prospective study over nearly 9 years we examined 1285 children aged 1-15 years (m=514, f=771). Using a 5-MHz curved array transducer the right lower quadrant was examined in a graded compression technique. RESULTS: Prevalence of histologically proven a.A. was remarkably low (9%). In diagnosis of acute appendicitis in childhood US achieves a s

Perforation of the Small Intestine with Acute Peritonitis Caused by Enterobius Vermicularis.

Perianal and perineal pruritus is often associated with Enterobius vermicularis in children. Although this roundworm is common in pediatric practice, most doctors are unaware that it can cause appendicular colic with/or appendicitis, severe urogenital complications, bowel perforation, and peritonitis. We report a case of a young male who presented with signs and symptoms of acute peritonitis. Dur-ing the operation, perforation of the jejunum with a purulent exudate under the transverse colon, a left lateral canal, a Douglas cavity were found as well as single enlarged mesenteric lymph nodes. Kikuchi-Fujimoto Disease Masquerading as Acute Appendicitis.

Kikuchi-Fujimoto Disease (KFD) is a self-limiting necrotizing lymphadenitis that usually presents with fever and cervical lymphadenopathy. Recognition of this condition is crucial, because it can be mistaken for tuberculosis, lymphoma and connective tissue disorders. When present at an unusual location the diagnosis can be challenging. We present an unusual case of Kikuchi-Fujimoto disease involving mesenteric lymph node masquerading as acute appendicitis along with its differential diagnosis. A 30-year-old female presented with complaints of acute abdominal pain, vomiting and fever. Physical

Childhood Mesenteric Adenitis -The Spectrum of Findings.

Background Mesenteric adenitis in children (≤ 18 years) can present with varied findings often causing a diagnostic confusion. In children, considering the inadequacies in history and physical examination, an accurate clinic-radiological diagnosis backed up with laboratory data as indicated becomes important to avoid misdiagnosis and futile interventions. Objective To study the profile of children evaluated for acute abdominal pain and diagnosed as mesenteric adenitis. Method A retrospective review of children with abdominal symptoms, diagnosed to have mesenteric adenitis between January 2018
