Is the minimal clinically important difference (MCID) in acute pain a good measure of analgesic efficacy in regional anesthesia?

In the field of acute pain medicine research, we believe there is an unmet need to incorporate patient related outcome measures that move beyond reporting pain scores and opioid consumption. The term "minimal clinically important difference" (MCID) defines the clinical benefit of an intervention as perceived by the patient, as opposed to a mathematically determined statistically significant difference that may not necessarily be clinically significant. The present article reviews the concept of MCID in acute postoperative pain research, addresses potential pitfalls in MCID determination and qu…

Measuring acute postoperative pain using the visual analog scale: the minimal clinically important difference and patient acceptable symptom state.

BACKGROUND: The 100 mm visual analog scale (VAS) score is widely used to measure pain intensity after surgery. Despite this widespread use, it is unclear what constitutes the minimal clinically important difference (MCID); that is, what minimal change in score would indicate a meaningful change in a patient's pain status. METHODS: We enrolled a sequential, unselected cohort of patients recovering from surgery and used a VAS to quantify pain intensity. We compared changes in the VAS with a global rating-of-change questionnaire using an anchor-based method and three distribution-based methods (…

Minimal Clinically Important Difference of the Modified Barium Swallow Impairment Profile (MBSImP) Pharyngeal Total Score in Post-Stroke Dysphagia.

The Modified Barium Swallow Impairment Profile (MBSImP) is widely used to quantify swallowing physiology during videofluoroscopic swallowing assessment, but the minimal clinically important difference (MCID) for its pharyngeal total score in post-stroke dysphagia remains unclear. This study estimated the MCID for improvement in the MBSImP pharyngeal total score during inpatient rehabilitation. This prospective multicenter observational study included stroke survivors with dysphagia admitted for inpatient rehabilitation 2 weeks to 3 months after stroke onset. Baseline and 10-day follow-up asses…

Dyspnoea-referenced minimal clinically important difference in the resting inspiratory capacity response to short-acting bronchodilators in COPD patients.

BACKGROUND: In COPD, the clinical significance of bronchodilator-induced changes in inspiratory capacity (IC) at rest remains uncertain. Unlike forced expiratory volume in one second (FEV1), IC may better reflect symptom improvement, but no minimal clinically important difference (MCID) has been formally established. RESEARCH QUESTION: To relate short-acting bronchodilator (SABD)-induced dyspnoea relief in stable COPD patients at rest with the concomitant IC changes and determine the MCID of the latter. STUDY DESIGN AND METHODS: In 106 stable COPD outpatients undergoing routine lung function…

Radiographic predictors of functional and pain outcomes in Scheuermann's kyphosis: an ROC-based minimal clinically important difference (MCID) analysis.

STUDY DESIGN: Retrospective study. OBJECTIVES: This study uses receiver operating characteristic (ROC)-based minimal clinically important difference (MCID) analysis to quantify which radiographic changes best predict meaningful functional and pain improvements following posterior spinal fusion for Scheuermann's kyphosis (SK). SK is a rigid spinal deformity resulting in significant back pain and functional impairment, commonly affecting adolescents and young adults. Surgical correction aims to restore sagittal balance, but the relationship between radiographic correction and patient-reported o…

Editorial Commentary: Large Heterogeneity of Minimal Clinically Important Difference Utilization in Hip Arthroscopy Studies: Where Do We Go From Here?

The minimal clinically important difference is a relatively new method to evaluate outcomes after surgery, defined as the lowest value of change in the outcome score that results in a perceived clinical improvement. There is no clear delineation of use for this metric, resulting in a heterogeneous application in hip arthroscopy research, making comparisons with other studies difficult. Cohort-specific values calculated using an anchor-based method are best.

Minimal Clinically Important Difference in Patient-Reported Outcome Measures with the Transforaminal Endoscopic Decompression for Lateral Recess and Foraminal Stenosis.

BACKGROUND: Patient-reported outcome measures (PROMs) have become widely used to better measure patients' judgment of treatment benefits from surgical spine care. The concept of determining the minimal clinically important differences (MCIDs) of PROMs is aimed at assessing the benefits of lumbar spine care that are meaningful to the patient. The goal of this study was to validate the utility of MCIDs of the visual analog score (VAS) and Oswestry Disability Index (ODI) in patients with sciatica-type low back and leg pain due to lateral recess and foraminal stenosis who were treated with directl…

Defining and validating the minimal clinically important difference for the cough severity visual analogue scale in adults with subacute cough.

BACKGROUND: Subacute cough imposes a significant burden, yet determining meaningful symptom relief remains challenging. While the cough severity visual analogue scale (VAS) is widely used, its minimal clinically important difference (MCID) is undefined for this population, limiting its utility in symptom management and clinical decision-making. OBJECTIVES: To establish the VAS MCID in adults with subacute cough, confirm its psychometric validity, and validate its utility in predicting clinical outcomes. METHODS: Individual patient data were analyzed from two randomized trials (N = 335) invol…
