This paper is a critique of recent service-intensive shelter programs for homeless mothers and the policies that underlie these shelters. We first document the process by which mental health problems and family homelessness became so closely but mistakenly linked. We then demonstrate empirically that shelter programs for homeless families nonetheless presume that mental health problems are part of the causal nexus of family homelessness and indiscriminately deliver mental health services to homeless mothers. OBJECTIVE: To describe the clinical outcome of infants born to mothers with gestational diabetes mellitus (GDM) and preexisting insulin-dependent diabetes mellitus (IDDM).
SETTING: A tertiary care regional perinatal center with a specialized diabetes-in-pregnancy program.

Eighty infants born to HIV-infected mothers were studied prospectively at Children's Hospital, Bangkok, Thailand from February 1989 to June 1991. The risk factors for acquisition of HIV infection were analyzed in 33 mothers (41.25%) including a history of being sex workers and having husbands who had extramarital sexual contact (22.5% each), having other sexually transmitted diseases (20.0%) and being IV drug users (6.25%). Human breast milk samples from eight southeast Louisiana communities and one in Mississippi were analyzed by the purge and trap plus gas chromatography method for trihalomethane compounds (THMs). THM levels in drinking water samples from the same communities were compared but analysis of variance computations showed no significant correlation between the water and breast milk. THMs do not appear to be extracted from drinking water to concentrate in human breast milk.

A multicenter study was able to utilize 120 medical files of children born from mothers who presented an abnormal thyroid function, 67 euthyroid goiters, 29 hyperthyroidisms, and 24 hypothyroidisms. Large for gestational age (LGA) infants of insulin-dependent diabetic mothers (IDM), appropriate for gestational age (AGA) IDM, and infants of nondiabetic mothers were compared for the incidence of neonatal hyperbilirubinemia and related etiologic factors. At 60 hours of age, LGA IDM had significantly higher serum bilirubin concentrations (12.3 +/- 2.1 mg/100 ml) than AGA IDM (7.6 +/- 3.9 mg/100 ml) or control infants (7.8 +/- 2.8 mg/100 ml) (P < .001).
