Cutaneous sebaceous carcinoma.

Cutaneous sebaceous carcinoma occurs almost exclusively on the head and neck and has a significant propensity for recurrence and metastasis. It is easily mistaken for benign conditions, resulting in inappropriate management. Thus, it is important to maintain a high index of suspicion. Despite previous reports, sebaceous carcinoma may occur with similar frequency in Asians and whites. Recent genetic data suggest there are multiple mutational groups of sebaceous carcinoma, paving the way for targeted treatment. After a diagnosis, investigations for staging and for Muir-Torre syndrome should be c

An Updated Review of the Therapeutic Management of Keratoacanthomas.

OBJECTIVE: Keratoacanthomas are fast-growing cutaneous neoplasms that can be difficult to distinguish from squamous cell carcinoma, both clinically and histologically. The uncertain behavior of these neoplasms creates a challenge in management, and treatment choice often varies significantly between cases. The objective of this review is to discuss the most common and up-to-date treatment modalities used in the management of keratoacanthomas. METHODS: A literature search was performed using PubMed to access and review relevant keratoacanthoma treatment modalities published within the last 40

Sebaceous carcinoma of scalp with proliferating trichilemmal cyst.

Sebaceous carcinoma is a rare neoplasm of the sebaceous gland. It is diagnosed mainly on histopathology and, clinically, it may mimic other neoplasms like squamous or basal cell carcinoma. We came across a patient presenting with a non-healing ulcer over the left temporo-parietal region of the scalp since 4 years and a single asymptomatic nodule over the occipital region since 3 years. Histopathology from an ulcer was pathognomonic of sebaceous carcinoma and that from a nodule was suggestive of proliferating trichilemmal cyst. The patient was screened for Muir Torre syndrome. We referred the p

Cutaneous Neoplasms with Sebaceous Differentiation: A Comprehensive Update on Clinical, Histopathological and Molecular Classification.

Cutaneous sebaceous neoplasms are rare adnexal tumours differentiated towards sebaceous glands, spanning a biological spectrum from benign (sebaceous adenoma, sebaceoma, nevus sebaceus) to outright malignant (sebaceous carcinoma). Most tumours are sporadic and solitary, with a predilection for the head and neck of elderly adults, but a clinically important subset is observed in the setting of Lynch syndrome/Muir-Torre syndrome. Recent large-scale genomic and transcriptomic studies have provided insights into the pathogenesis of these unique neoplasms, revealing a genetically heterogeneous land

The cutaneous manifestations of gastrointestinal malignancy.

The skin is often the herald of an underlying systemic illness, and gastrointestinal malignancies can present in numerous ways in the skin. Paraneoplastic phenomenon, such as acanthosis nigricans and tripe palm, may be the first indicator of a gastrointestinal malignancy. In addition, gastrointestinal cancers can metastasize to the skin, as described in the well-known Sister Mary Joseph's nodule. Inflammatory systemic conditions such as dermatomyositis and multicentric reticulohistiocytosis can be associated with underlying malignancy. Finally, in numerous genetic syndromes with underlying mal

Sebaceous carcinoma presenting as chronic chalazion.

We present a case of sebaceous carcinoma presenting as treatment-resistant chalazion. Diagnosis was made by excisional biopsy after failed treatment with warm compresses, eyelid hygiene and erythromycin ointment. Conjunctival map biopsies excluded local spread. MRI of the orbits and whole-body positron emission tomography demonstrated no metastatic disease. Definitive treatment was accomplished by wide local excision with no recurrence at 1-year follow-up. Sebaceous carcinoma often masquerades as benign diagnoses, such as blepharitis and chalazion, yet it is an aggressive cancer with high rate

[Sebaceous gland carcinoma of the eyelid co-occurred with other tumors: a report of four cases].

This article reported four cases of sebaceous gland carcinoma of the eyelid with other tumors. Among them, three cases were co-occurred with other ocular tumors, two cases with visceral malignancies, and one case with chronic lymphocytic leukemia/small lymphocytic lymphoma. Immunohistochemistry and/or whole-exome sequencing confirmed that the two cases with visceral malignancies did not meet the criteria for Muir-Torre syndrome. Additionally, whole-exome sequencing was performed on two cases of sebaceous gland carcinoma of the eyelid, both revealing mutations in the ZNF750 gene. Surgical inter

Corneal limbal sebaceous adenoma.

PURPOSE: The aim of this study was to describe a case of a corneal limbal sebaceous adenoma. METHODS: An 83-year-old male patient presented with a white solid nodular lesion covering most of the cornea of his right eye. The corneal lesion was removed totally. The corneal surface was reconstructed by amniotic membrane transplantation. RESULTS: The histopathological examination of the lesion revealed the presence of a sebaceous adenoma. A systemic examination revealed no other abnormalities. CONCLUSIONS: Sebaceous adenomas on the ocular surface are known to be very rare. To the best of our kn

Family with MSH2 mutation presenting with keratoacanthoma and precancerous skin lesions.

Muir-Torre syndrome (MTS) is a familial cancer syndrome characterized by a predisposition to keratoacanthoma (KA) and sebaceous tumors. Although MTS and hereditary non-polyposis colorectal cancer (HNPCC) share the same genetic alterations in mismatch repair (MMR) genes, the other skin lesions in MTS or HNPCC have been only rarely reported. We report a family with an MSH2 mutation c.1126_1127delTT (p.Leu376Thrfs*12). A 46-year-old male proband developed KA with sebaceous differentiation, colon cancer and gastric cancer, and fulfilled the diagnostic criteria for MTS. His 80-year-old mother, diag

Reticulated acanthoma with sebaceous differentiation: A case report.

RATIONALE: Reticulated acanthoma with sebaceous differentiation (RASD) is a rare and benign cutaneous neoplasm that is characterized by reticulated epidermal hyperplasia and differentiation of mature sebaceous cells. Due to the scarcity of this condition, the potential association with Muir-Torre syndrome (MTS) remains the subject of debate. PATIENT CONCERNS: A 52-year-old Chinese woman presented with a 3-year history of a solitary papule on the right buttock. DIAGNOSES: Histopathology provided a confirmed diagnosis of RASD. Immunohistochemical analysis demonstrated uniform nuclear positivit

Skin Signals: Exploring the Intersection of Cancer Predisposition Syndromes and Dermatological Manifestations.

Cutaneous manifestations can serve as early and sometimes the first clinical indicators in various hereditary cancer predisposition syndromes. This review provides a comprehensive overview of the dermatological signs associated with these syndromes, aiming to facilitate their recognition in clinical practice. Hereditary Breast and Ovarian Cancer syndrome is notably linked to an increased risk of melanoma. BAP1 tumor predisposition syndrome is characterized by BAP1-inactivated melanocytic tumors. Muir-Torre syndrome, a variant of Lynch syndrome, presents with distinctive cutaneous neoplasms suc

Overview of natural history, pathology, molecular genetics and management of HNPCC (Lynch Syndrome).

Hereditary non-polyposis colorectal cancer (HNPCC) predisposes to cancers of the colon, endometrium and several other extra-colonic sites in the absence of premonitory physical stigmata (Muir-Torre syndrome excepted). Discovery of the several DNA mismatch repair genes (hMSH2, hMLH1, hPMS1, hPMS2) holds the potential for determining the cancer destiny of patients, theoretically in utero. Pre-symptomatic DNA testing is now possible in patients from HNPCC families and will be clinically available once inexpensive and simple tests for these germ-line mutations have been effected. Surgical resection and full-thickness skin grafting for sebaceoma extending into the external auditory canal: a case report.

Sebaceoma is a rare benign cutaneous adnexal neoplasm with sebaceous differentiation that typically occurs in the head and neck region of elderly individuals. Sebaceoma extending into the external auditory canal is extremely uncommon. We report the case of an 85-year-old woman who presented with an enlarging mass involving the left tragus and extending into the external auditory canal. Histopathological examination demonstrated dermal nests of basaloid cells with sebaceous differentiation, and immunohistochemical analysis showed focal positivity for epithelial membrane antigen and Ber-EP4, lea

Genetic skin diseases predisposing to basal cell carcinoma.

Basal cell carcinoma (BCC) is the commonest cancer in humans. Predisposing factors reflect common genetic variations and environmental influences in most cases. However, an underlying Mendelian disorder should be suspected in a specific subset of patients, namely those with multiple, early onset lesions. Some specific conditions, including Gorlin, Bazex-Dupré-Christol and Rombo syndromes, and Xeroderma Pigmentosum, show BCC as a prominent feature. In addition, BCC may represent a relatively common, although less specific, finding in many other genodermatoses. These include disorders of DNA rep

An inherited form of large bowel cancer: Muir's syndrome.

Three inherited types of large bowel cancer not associated with polyposis were previously proposed. A fourth should also be added to the list, namely, Torre's or Muir's syndrome. This report summarizes the clinical and genetic features of that syndrome in one family. The principle features of the syndrome comprise multiple skin tumors, i.e., sebaceous adenomas, keratoacanthomas, and basal and squamous cell carcinomas occurring with polyps and adenocarcinomas, mainly of the large bowel and also of the small intestine and stomach. Other malignancies occurring with the multiple skin tumors, with

Oral mucosal stigmata in hereditary-cancer syndromes: From germline mutations to distinctive clinical phenotypes and tailored therapies.

Numerous familial tumor syndromes are associated with distinctive oral mucosal findings, which may make possible an early diagnosis as an efficacious marker for the risk of developing visceral malignancies. In detail, Familial Adenomatous Polyposis (FAP), Gardner syndrome, Peutz-Jeghers syndrome, Cowden Syndrome, Gorlin Syndrome, Lynch/Muir-Torre Syndrome and Multiple Endocrine Neoplasia show specific lesions of the oral mucosa and other distinct clinical and molecular features. The common genetic background of the above mentioned syndromes involve germline mutations in tumor suppressor genes,

Sebaceous carcinoma in a 54-year-old Black African man after cancer chemotherapy: a case report.

BACKGROUND: Sebaceous carcinoma is a very rare malignant skin adnexal tumor that is occasionally aggressive. We have not seen a case of sebaceous carcinoma in our center in the last 10 years. It is extremely rare in Black Africans. CASE PRESENTATION: We described the case of a 55-year-old man African man who presented to our ophthalmologist with complaints of growth on the right upper eyelid for 8 months. He had surgery and chemotherapy for rectal carcinoma 6 years prior to presentation and received his last dose of chemotherapy 5 years before seeing our ophthalmologist. There was a history o

Review of the Lynch syndrome: history, molecular genetics, screening, differential diagnosis, and medicolegal ramifications.

More than one million patients will manifest colorectal cancer (CRC) this year of which, conservatively, approximately 3% (approximately 30,700 cases) will have Lynch syndrome (LS), the most common hereditary CRC predisposing syndrome. Each case belongs to a family with clinical needs that require genetic counseling, DNA testing for mismatch repair genes (most frequently MLH1 or MSH2) and screening for CRC. Colonoscopy is mandated, given CRC's proximal occurrence (70-80% proximal to the splenic flexure). Due to its early age of onset (average 45 years of age), colonoscopy needs to start by age

Molecular pathology of skin adnexal tumours.

AIMS: Tumours of the cutaneous adnexa arise from, or differentiate towards, structures in normal skin such as hair follicles, sweat ducts/glands, sebaceous glands or a combination of these elements. This class of neoplasms includes benign tumours and highly aggressive carcinomas. Adnexal tumours often present as solitary sporadic lesions, but can herald the presence of an inherited tumour syndrome such as Muir-Torre syndrome, Cowden syndrome or CYLD cutaneous syndrome. In contrast to squamous cell carcinoma and basal cell carcinoma, molecular changes in adnexal neoplasia have been poorly chara
