[Mycobacterium chelonae myositis].

INTRODUCTION: Mycobacteria are only exceptionally responsible for infection of the skin and soft tissues. A Mycobacterium chelonae myositis occurred in an immunodepressed patient. OBSERVATION: A 49 year-old man, treated for many years with corticosteroids for vasculitis of the lower limbs associated with rheumatoid polyarthritis, was hospitalized for invasive pulmonary aspergillosis. Ten days later he developed myositis of the right arm with multiple subcutaneous abscesses. Culture of the purulent substance isolated Mycobacterium chelonae. Treatment with ciprofloxacine and clarithromycine led…

[Myositis ossifications progressiva].

Myositis ossificans progressiva (MOP) is a rare hereditary connective tissue disorder. Skeletal abnormalities and progressive ectopic ossifications are features of this condition. Diagnosis is often delayed because of heterogenous picture of MOP. Treatment is unknown, so MOP leads to prolonged disability.

Myositis overlap syndromes.

Myositis-overlap syndromes are characterized by a heterogeneous group of clinical syndromes of which many are closely linked with specific autoantibodies. Clinical and laboratory-based research into this fascinating group of syndromes have given us clues in elucidating the role of immunogenetic, environmental influences on the etiopathogenesis of autoimmune disease. Patients positive for anti-snU1 RNP antibody often present with a constellation of "autoimmune-related" clinical features, often not fulfilling the diagnostic criteria for a single disease entity. Controversy remains as to whether …

Idiopathic orbital myositis.

Idiopathic orbital myositis is a subtype of nonspecific orbital inflammation primarily involving the extraocular muscles. It occurs most frequently in young to middle-aged adults with a 2 to 1 female predominance. The cardinal clinical feature is orbital pain exacerbated by eye movement. Other common findings include diplopia, proptosis (which is generally minimal), conjunctival injection and chemosis, and periorbital edema. Thyroid eye disease is commonly confused with orbital myositis, but the latter is characterized by a more acute onset, more severe pain, and a rapid response to systemic c…

Acute orbital myositis.

We examined 9 consecutive cases of unilateral orbital myositis (7 women and 2 men; age range 15-46 years) presenting to Casualty. Only 3 were correctly diagnosed on the first visit. Eight patients exhibited globe retraction in the acute stages and, after treatment with systemic steroids, all made a full recovery. None of the patients had associated systemic disease and all remain well over a 6-12 month follow-up period. Orbital myositis may be a more common condition than previously thought; it can present with a variety of clinical signs, and may be difficult to diagnose in the early stages. …

[Myositis ossificans progressiva].

BACKGROUND: Myositis ossificans progressiva is a rare progressive disease of connective tissue with a poor prognosis. CASE REPORT: A 16 year-old girl suffered from lameness of her right leg associated with inguinal swelling. Progressive aggravation of pain with extension of swelling to the posterior part of her thigh required an X-ray examination which showed hip dysplasia and calcifications around the hip. Angiography was normal; a diagnosis of hematoma was suggested by scannography and bone scintigraphy, but biopsy showed features of nodular fasciitis. The association of progressive ectopic…

Myositis ossificans traumatica.

OBJECTIVE: To present a case of myositis ossificans traumatica (MOT) in a hockey player. Serial X-ray studies allow the reader an opportunity to observe MOT in its earliest through fulminated stages. CLINICAL FEATURES: A 20-yr-old hockey player was subjected to an acute blow to the lateral thigh. Copious amounts of swelling soon developed. The patient was unable to skate. X-rays were initially performed 4 hr after the injury and demonstrated a hugh mass developing in the thigh at that time. INTERVENTION AND OUTCOME: The player was inappropriately treated with deep tissue massage and heat at …

Myositis ossificans traumatica.

Myositis ossificans is a benign condition of heterotopic bone formation. It is a generalized term for four separate clinical entities. Although not a rare disease process, diagnosis often remains difficult. This article reviews the various forms of myositis ossificans, as well as the pathology, diagnostic tests, and treatment options.

Fulminant streptococcal myositis.

A previously healthy 70-year-old woman was hospitalized for acute pain and edema of her right leg. Deep vein thrombosis was suspected, and she was put on anticoagulant therapy. Ten hours later, she developed a massive swelling of the leg with a well-demarcated violaceous discoloration of the skin and hemorrhagic bullae. She was in deep shock with signs of disseminated intravascular coagulation and adult respiratory distress syndrome. Gram stain of an aspirate from the bullae revealed short chains of Gram-positive cocci, and multiple blood cultures showed abundant growth of Streptococcus pyogen

[Myositis ossificans progressiva].

The case of a 19-year-old female patient with myositis ossificans progressiva is reported. This disease is a rare hereditary disorder with a dominant autosomal genotype. The patient had typical ossifications of the humeral and dorsal muscles, as well as of those of the left thigh and upper arm, and also an ankylosis of the left hip. There were typical deformations of the cervical vertebrae and of the skeleton of the hands and feet. Laboratory tests showed alkaline phosphatase to be greatly increased. ECG revealed a bifascicular bundle-branch block, and a high-grade restrictive ventilation diso…

Localized nodular myositis.

Two patients presenting with painful, localized swelling in a single muscle are described. Biopsies of involved areas showed inflammatory changes characteristic of polymyositis, but serum levels of muscle-derived enzymes were consistently normal. Both patients later developed evidence of mild diffuse myositis. A review of 4 similar cases in the literature suggests that localized nodular myositis is a heterogeneous condition, in which vascular inflammation may be a prominent feature, with frequent evolution into a generalized myopathy.
