Importance of clinical practice guidelines to practicing pediatric nephrologists and IPNA survey.

Clinical practice guidelines (CPGs) are systematically developed statements backed by scientific evidence to assist practitioners in management in clinical practice. An international cross-sectional survey was conducted by the IPNA to examine the perceptions of pediatric nephrologists on guidelines and their usage and to identify important diseases for future clinical practice guidelines (CPGs). The survey found that the majority of pediatric nephrologists find CPGs useful in clinical practice and admitted to using them most of the time. Developing CPGs is challenging and there are standards a…

Nephrologists' Perspectives on Recipient Eligibility and Access to Living Kidney Donor Transplantation.

BACKGROUND: Wide variations in access to living kidney donation are apparent across transplant centers. Such disparities may be in part explained by nephrologists' beliefs and decisions about recipient eligibility. This study aims to describe nephrologists' attitudes towards recipient eligibility and access to living kidney donor transplantation. METHODS: Face-to-face semistructured interviews were conducted from June to October 2013 with 41 nephrologists from Australia and New Zealand. Transcripts were analyzed thematically. RESULTS: We identified five major themes: championing optimal reci…

Knowledge of nephrologists and patients about autosomal dominant polycystic kidney disease (ADPKD).

Because of the availability of presymptomatic testing for autosomal dominant polycystic kidney disease (ADPKD), we assessed the response of nephrologists, patients, and at-risk relatives to the introduction of a genetic counselling program for ADPKD. Fifty-six of seventy-six nephrologists responded. Ninety-eight percent reported 'generally' telling their patients that the disease was hereditary, but fewer reported screening relatives (81% for children and 70% for siblings and parents). Ninety percent were interested in referring patients to the service. Fourteen of the 24 patients in one renal…

A Perspective on the Role of Nephrologists in the Cardio-Kidney-Metabolic Syndrome.

In order to appreciate and define the role of nephrologists in the newly defined cardio-kidney metabolic (CKM) syndrome, we acknowledge the important shift that must occur from a specialty that has traditionally focused on treating people with organ failure, to one that needs to now embrace the concept of prevention, early diagnosis, and delay of progression as complimentary to historical roles as providers of dialysis and transplantation. We describe the value that nephrology can play in further refining the CKM model to acknowledge the importance of the diagnostic precision of CKD, and kidne…

The Importance of the Nephrologist in the Treatment of the Diuretic-Resistant Heart Failure.

Heart failure is not only a global problem but also significantly limits the life prospects of these patients. The epidemiology and presenta … e peritoneal dialysis in diuretic-resistant heart failure remains underrepresented. We believe that nephrologists are uniquely positioned to help these patients by providing acute peritoneal dialysis to reduce hospitalization dependency and increase their quality of life.

Venous Excess Doppler Ultrasound for the Nephrologist: Pearls and Pitfalls.

The role of venous congestion in abnormal kidney function is being increasingly recognized. It is well known that unresolved congestion is associated with adverse kidney and overall outcomes in patients with heart failure. Similarly, any condition that leads to elevated central venous pressure, such as pulmonary hypertension, can result in impaired kidney perfusion by increasing its afterload. Point-of-care ultrasonography (POCUS) enables the clinician to objectively assess hemodynamics at the bedside and, thereby, guide patient management. Lung POCUS has received widespread attention in the r

Principles of Kidney Pharmacotherapy for the Nephrologist: Core Curriculum 2021.

Medications are an important part of the management of patients with kidney disease. When used appropriately, pharmacotherapy can slow disease progression and reduce morbidity and mortality. Unfortunately, reduced kidney function can significantly alter the pharmacokinetics and pharmacodynamics of many medications, putting patients at risk for drug toxicity if modifications to therapy are not appropriately managed. Adding complexity to the appropriateness of medication and dosage selection is the difficulty in estimating kidney function and the discordance between the Cockcroft-Gault-derived d

MICROSCOPIC POLYANGIITIS - A VIEW OF THE PROBLEM THROUGH THE LENS OF A NEPHROLOGIST.

The article presents two clinical cases of microscopic polyangiitis in patients with symptoms of glomerulonephritis with renal failure, which were preceded by such nonspecific symptoms as: abdominal syndrome, high blood pressure, arthralgia, myalgia, weight loss, uveitis, shortness of breath, general weakness. Clinical and laboratory-instrumental aspects of diseases are analyzed. Emphasized the feasibility of early diagnosis, adequate therapy appointment. The aim of the article is to show that only with timely prescribing of pathogenetic therapy it is possible to achieve clinical and laborator

Assessment of Interobserver Reliability of Nephrologist Examination of Urine Sediment.

IMPORTANCE: Urine sediment microscopy is commonly performed during the evaluation of kidney disease. Interobserver reliability of nephrologists' urine sediment examination has not been well studied. OBJECTIVE: Assess interobserver reliability of the urine sediment examination. DESIGN, SETTING, AND PARTICIPANTS: In this diagnostic test study, urine samples were prospectively collected from a convenience sample of adult patients from an academic hospital in the United States undergoing kidney biopsy from July 11, 2018, to March 20, 2019. Digital images and videos of urine sediment findings wer…

What the non-nephrologist needs to know about dialysis.

The End-Stage Renal Disease (ESRD) program now serves approximately 675,000 individuals in the United States at a cost of $26.1 billion to the Medicare system. Given the size of this population, healthcare providers from all disciplines will deliver care to patients on dialysis. Mortality remains high among patients on chronic dialysis, with 42.3% surviving 5 years. As this is a vulnerable population, it is important in the care of ESRD patients that non-nephrologists have a working knowledge of issues germane to dialysis. This review examines the physiology, mechanics, complications, and care…

[What the nephrologist should know about antiphospholipid syndrome?].

Antiphospholipid syndrome is often recognize autoimmune disease associated with arterial and venous thrombosis and pregnancy loss. It is caused by antibodies against proteins combined with cell membrane anionic phospholipids. Kidney are one of the most often affected organs in course of antiphospholipid syndrome. Renal manifestations include thrombotic microangiopathy and large vessel thrombosis. In patients with end-stage renal disease, antiphospholipid antibodies are prevalent and may increase in frequency with time on dialysis. The presence of anticardiolipin antibodies have been associated

[Differential diagnosis and therapy of pulmorenal syndrome--the nephrologist's perspective].

The "pulmorenal syndrome" is a clinically defined syndrome under which one summarises disease entities with acute renal failure due to acute glomerulonephritis and alveolar lung hemorrhage. Wegener's granulomatosis, other inflammatory renal diseases and Goodpasture's syndrome are the most frequent causes of this syndrome. The physical examination and histological examination, mostly of a kidney biopsy, represent the mainstay of securing the diagnosis. The diagnosis is completed by serological testing for autoantibodies. The therapy is determined by the underlying disease. Plasmaspheresis is al

Prevention and treatment of secondary hyperparathyroidism: still a challenge for the nephrologist?

Secondary hyperparathyroidism is a well known complication of chronic renal insufficency. It is not only a state of increased parathyroid hormone secre-tion but also a state of parathyroid gland hyperplasia. Prevention and treatment of secondary hyperpara-thyroidism is a huge challenge for the nephrologist, despite new agents for the treatment of hyperphos-phataemia, new vitamin D analogues and calci-mimetics. The importance of parathyroid gland hyperplasia and the possibility of its reduction frequently are neglected issues. Ultrasonography could be very useful in detection of parathyroid gla
