The so-called cavernous sinus: a review of the controversy and its implications for neurosurgeons.

The so-called cavernous sinus is a venous pathway, an irregular network of veins that is part of the extradural venous network of the base of the skull, not a trabeculated venous channel. This venous pathway, the internal carotid artery, and the oculomotor cranial nerves cross the medial portion of the middle cranial fossa in an extradural space formed on each side of the sella turcica by the diverging aspects of a dural fold. In this space the venous pathway has only neighborhood relations with the internal carotid artery and the cranial nerves. The space itself must be distinguished from the

Clinical perspectives for the practicing neurosurgeon on integrative multiomic analyses of aggressive meningiomas.

Meningiomas are the most common primary intracranial tumor and demonstrate marked heterogeneity in clinical behavior, ranging from indolent … gies, and adjuvant therapy considerations. As molecular diagnostics become increasingly accessible, neurosurgeons will be confronted with this information in routine practice. A practical understanding of the strengths, limitations, and clinical implications for molecular diagnostics will be essential for optimizing the management of patients with aggressive me …

Laparoscopic insertion of ventriculoperitoneal shunts by neurosurgeons: An audit of a tertiary hospital practice.

OBJECTIVE: Since 2013, at the Royal Brisbane and Women's Hospital (RBWH) some neurosurgeons have routinely used laparoscopy to insert the distal catheter during ventriculoperitoneal shunt surgery. The purpose of this study was to investigate the safety and outcomes of laparoscopic ventriculoperitoneal shunt insertion by neurosurgeons, compared with laparoscopic shunt insertion by general surgeons and traditional, open, minilaparotomy by neurosurgeons or general surgeons. METHODS: A retrospective audit was performed of all shunt surgery performed at RBWH over 9 years, collecting data from hosp…

Mapping the global neurosurgery workforce. Part 1: Consultant neurosurgeon density.

OBJECTIVE: It is unknown whether efforts to expand access to neurosurgery worldwide have translated to an increase in the global neurosurgery workforce, particularly in low- and middle-income countries. The main objective of this study was to quantify the number and distribution of consultant neurosurgeons worldwide, while also identifying temporal and geographic trends in the neurosurgery workforce in different income levels and WHO regions, and analyzing what factors might contribute to the growth of a national workforce. METHODS: This study was a subanalysis of an electronic cross-sectiona…

Benefits and limitations of a dual faculty neurosurgeon approach to resection of pediatric craniopharyngioma.

PURPOSE: The utility and safety of including two neurosurgeons for tumor resections is unknown. This study compares outcomes among pediatric patients with craniopharyngiomas operated on with a dual or single surgeon approach (DSA, SSA). METHODS: A single-center review identified all craniopharyngioma transsphenoidal or craniotomy resections from 2000 to 2020. Surgical years of experience (YOE) and rates of 5-year reoperations, complications, recurrence, and postoperative radiotherapy were analyzed. RESULTS: Twenty-six transsphenoidal and 68 craniotomies were identified among 62 patients. Ele…

Hypophosphatasia and cleidocranial dysplasia-a case report and review of the literature: the role of the neurosurgeon.

Hypophosphatasia (HPT) and cleidocranial dysplasia (CCD) are rare genetic disorders characterized by both defective ossification and bone mineralization. Patients usually present with craniosynostosis and cranial defects which in many cases require surgical repair. There is only 1 reported case of combined HPT and CCD in the literature. Our reported case involves a 3.5-year-old girl with concomitant homozygous CCD and heterozygous HPT. The child had an extended cranial defect since birth which improved with the administration of Strensiq and was followed until preschool age. Bone defects were

Neurosurgeon or spinal surgeon - who should treat cauda equina syndrome?

Suspected cauda equina syndrome (CES) requires urgent evaluation, investigation and treatment. Timing of cauda equina decompression is crucial, and delays in its management, leading to significant irreversible disability can be devastating for patient and surgeon alike. The standard of care in CES therefore needs to be clear and unambiguous. Todd and Dickson have written an excellent paper summarising the condition, and have outlined what they feel is the standard of care. We would ask the authors to clarify an important point.

Endoscopic endonasal anatomy and approaches to the anterior skull base: a neurosurgeon's viewpoint.

OBJECTIVES: The objective of this study was to review the endoscopic anatomy of the anterior skull base, defining the pitfalls of endoscopic endonasal approaches to this region. Recently, these approaches are gaining popularity among neurosurgeons, and the details of the endoscopic anatomy and approaches are highlighted from the neurosurgeons' point of view, correlated with demonstrative cases. MATERIALS AND METHODS: Twelve fresh adult cadavers were studied (n = 12). We used Karl Storz 0 and 30 degrees, 4 mm, 18- and 30-cm rod lens rigid endoscope in our dissections. After preparation of the …

Neurofibromatosis type 1 and the pediatric neurosurgeon: a 20-year institutional review.

Children with neurofibromatosis type 1 (NF1) undergo costly surveillance scanning for a variety of asymptomatic central nervous system lesions whose natural history is poorly understood. We performed a 20-year retrospective chart review of 25 patients with clinically proven NF1 who required surgery (group A) and contrasted this cohort with 150 NF1 patients who did not require surgery (group B). In group A, 52% of patients underwent multiple procedures for more than one lesion (p = 0.043). Group A patients were further distinguished from those in group B by exhibiting a greater number of optic

A disease warranting attention from neurosurgeons: primary central nervous system post-transplant lymphoproliferative disorder.

BACKGROUND: Primary central nervous system post-transplant lymphoproliferative disorder (PCNS-PTLD) is a rare condition, posing diagnostic and treatment challenges, with histological biopsy essential for diagnosis. Standardized treatment protocols are lacking. This disease requires urgent attention due to the increasing number of organ transplant surgeries and the use of immunosuppressive agents. METHODS: From 2020 to 2023, our center diagnosed five patients with PCNS-PTLD. We reviewed their clinical records and conducted a comprehensive analysis of 22 literatures on PCNS-PTLD cases following

Factors Associated With Pregnancy and Perinatal Outcomes in Female Neurosurgeons: A Cross-Sectional Study.

Neurosurgery is male dominated with women representing only 12% of residents and 5% of practicing neurosurgeons. The conflicting demands of training versus pregnancy and motherhood are significant deterrents to women entering the field. We examined pregnancy incidence and timing, perinatal complications, and the perceived career impact of motherhood on female neurosurgeons using an anonymous survey of 643 training, practicing, and retired female neurosurgeons from the United States. Among 260 respondents, 50.8% (132/260) reported pregnancies, with an average age at first pregnancy that was sig…

Low-dose aspirin before spinal surgery: results of a survey among neurosurgeons in Germany.

The main problem faced by the increasing numbers of patients presenting for spinal surgery are receiving concurrent medication with low-dose aspirin, leading to dysfunctional circulating platelets. The contribution of low-dose aspirin to increased peri-operative risk of bleeding and blood loss is a contentious issue with conflicting published results from different surgical groups. Data from neurosurgical spine patients is sparse, but aspirin has been identified as an important risk factor in the development of post-operative hematoma following intracranial surgery. We surveyed the opinions an

Preoperative Decision-Making in Microvascular Decompression for Trigeminal Neuralgia: A Survey of Practicing Neurosurgeons.

OBJECTIVE: Microvascular decompression (MVD) for trigeminal neuralgia (TN) results in durable pain freedom in a large percentage of appropriately selected patients. The decision to perform MVD is based on a combination of clinical symptomatic presentation and imaging findings demonstrating neurovascular compression (NVC) with surgeons weighting these variables differently. This study sought to determine the relative importance of clinical symptomatic presentation and imaging findings of NVC in decision-making to pursue MVD for TN among North American board-certified neurosurgeons. METHODS: An…
