How dirty is your QWERTY? The risk of healthcare pathogen transmission from computer keyboards.

INTRODUCTION: Healthcare environmental surfaces may be contaminated with micro-organisms that cause healthcare-associated infections (HCAIs). Special attention is paid to near-patient surfaces but sites outside the patient zone receive less attention. This paper presents data on keyboard contamination and the risk of pathogen transmission from keyboards. METHODS: Keyboards from nursing stations in three hospitals and a dental practice were analysed for bacterial contamination. Surfaces were pre-treated to remove planktonic bacteria so that any remaining bacteria were presumed to be associated…

Impact of daily disinfection of portable equipment and common areas with a prototype handheld far ultraviolet-C light device on transmission of a DNA marker and bacteriophage MS2 on a long-term care ward.

BACKGROUND: Shared portable medical equipment is a vector for transmission of health care-associated pathogens, but cleaning and disinfection of equipment is often suboptimal. METHODS: We conducted a cross-over trial on a long-term care facility ward to determine the impact of daily low-level disinfection of portable equipment and common areas using a handheld far ultraviolet-C (UV-C) light device versus standard cleaning and disinfection practices. The primary outcome was transmission of a DNA marker and bacteriophage MS2 from inoculated portable vital signs equipment to surfaces on the ward

The cognitive processes employed by undergraduate nursing OSCE assessors: A qualitative research study.

OBJECTIVES: The Objective Structured Clinical Examination (OSCE) is an assessment format commonly used to assess undergraduate nursing students. However, in spite of its prominence, relatively little research has been conducted into how OSCE assessors form judgements about student performances, and whether divergent processes of judgement formation have the potential to negatively impact the inter-rater reliability (IRR) of awarded scores. This qualitative study aimed to uncover the cognitive processes which assessors employ when assessing OSCE performances. DESIGN, SETTING, PARTICIPANTS: In

Identification of occupational cancer risks in British Columbia. A population-based case-control study of 995 incident breast cancer cases by menopausal status, controlling for confounding factors.

Lifetime occupational histories as well as information on known and suspected breast cancer risk factors were collected by means of a self-administered questionnaire from 1018 women with incident breast cancer ascertained from the British Columbia Cancer Registry, and from 1020 population controls. A matched case-control study design was used. Conditional logistic regression for matched sets data and the likelihood ratio were used in a two-step procedure and were performed separately for pre-menopausal women, post-menopausal women, and for all cases combined. Excess risk was noted for several

Crawl, Walk Fast, Run Hard: Military-Civilian Partnership Training at the Army Trauma Training Course Improves Perceived Deployment Readiness.

INTRODUCTION: The Army Trauma Training Course (ATTC) has held the responsibility of training deploying U.S. Army Forward Resuscitative and Surgical Detachments (FRSDs) since September 2001. The program of instruction (POI) consists of didactics and skill stations (Crawl), a mass casualty (MASCAL) situational training exercise (STX) (Walk), and clinical integration into an urban Level I trauma center (Run). Over the program's 25-year existence, there has been no report on the subjective assessment of the rotator's perceived effectiveness of the program as a whole. We hypothesized that a post-co

A CDC WONDER Study on Disparities in Place of Death Among Patients With Systemic Lupus Erythematosus in the United States between 1999 and 2020.

INTRODUCTION: Systemic lupus erythematosus (SLE) is an autoimmune disease, more prevalent among African-American women, often associated with severe manifestations such as lupus nephritis and neuropsychiatric lupus. Both conditions contribute significantly to morbidity and mortality, though lupus nephritis is more commonly linked to direct disease-related deaths. Mortality can also result from other severe disease manifestations or treatment-related complications. Assessing disparities in place of death among lupus patients is essential for improving care quality through targeted physician and

Report on support activity for the East Japan Great Earthquake (May 27-29, 2011).

The M9.0 great earthquake that occurred on March 11, 2011 triggered a huge tsunami on the Pacific coast in the Tohoku region and caused enormous damage, resulting in 15,854 deaths and 3,276 missing persons (as of March 1, 2012, according to the Japanese National Police Agency). Presently, inhabitants in Fukushima Prefecture continue to live as refugees because of radiation contamination caused by explosions at the Fukushima nuclear power plants. Immediately after the earthquake, DMATs (Disaster Medical Assistance Teams) went to the affected areas to begin relief operations. The Japan Medical A

Providing medical care at mass gathering sporting events: the 2023 Ryder Cup experience.

BACKGROUND: The Ryder Cup is a men's golf competition and one of the most high-profile sporting events worldwide, presenting significant challenges for healthcare providers, especially when hosted in non-urban areas. Medical professionals must address sport-related illness and injuries in elite athletes, exacerbations of pre-existing conditions in spectators, and occupational health issues among staff and volunteers. Proper healthcare planning and infrastructure are crucial to reduce the burden on local hospitals and ensure timely care for everyone involved. Aim of the study is to describe the

Do hotspots fuel malaria transmission: a village-scale spatio-temporal analysis of a 2-year cohort study in The Gambia.

BACKGROUND: Despite the biological plausibility of hotspots fueling malaria transmission, the evidence to support this concept has been mixed. If transmission spreads from high burden to low burden households in a consistent manner, then this could have important implications for control and elimination program development. METHODS: Data from a longitudinal cohort in The Gambia was analyzed. All consenting individuals residing in 12 villages across the country were sampled monthly from June (dry season) to December 2013 (wet season), in April 2014 (mid dry season), and monthly from June to De

Changes in Access to Alcohol-Based Hand Rub and Hand Hygiene Adherence among Healthcare Workers after a Hand Rub Production and Distribution Program in Rural Uganda before and during the COVID-19 Pandemic.

During the COVID-19 pandemic, the use of alcohol-based hand rubs (ABHRs) was critical for improving hand hygiene (HH) among healthcare workers (HCWs). Before and during the pandemic, we supported district-led production and district-wide distribution of ABHRs and one-time provision of portable handwashing stations to select healthcare facilities (HCFs) in five rural districts in Uganda. Comparison between baseline and follow-up assessments showed an overall increase in access to HH materials and HH adherence (HHA; handwashing with soap and water or use of ABHR) among HCWs. However, large diffe

Emergency physician awareness of prehospital procedures and medications.

INTRODUCTION: Maintaining patient safety during transition from prehospital to emergency department (ED) care depends on effective handoff communication between providers. We sought to determine emergency physicians' (EP) knowledge of the care provided by paramedics in terms of both procedures and medications, and whether the use of a verbal report improved physician accuracy. METHODS: We conducted a 2-phase observational survey of a convenience sample of EPs in an urban, academic ED. In this large ED paramedics have no direct contact with physicians for non-critical patients, giving their re

Views and perspectives toward implementing the Global Spine Care Initiative (GSCI) model of care, and related spine care program by the people in Cross Lake, Northern Manitoba, Canada: a qualitative study using the Theoretical Domain Framework (TDF).

BACKGROUND: Back pain is very common and a leading cause of disability worldwide. Due to health care system inequalities, Indigenous communities have a disproportionately higher prevalence of injury and acute and chronic diseases compared to the general Canadian population. Indigenous communities, particularly in northern Canada, have limited access to evidence-based spine care. Strategies established in collaboration with Indigenous peoples are needed to address unmet healthcare needs, including spine care (chiropractic and movement program) services. This study aimed to understand the views

The Bustelo Bolus: Caffeine Withdrawal Mimicking Status Migrainosus in a Patient With Systemic Lupus Erythematosus and Systemic Sclerosis Overlap.

Caffeine is the most widely consumed psychoactive substance in the world, and abrupt cessation in habitual users produces a stereotyped withdrawal syndrome whose dominant feature is headache. Although caffeine withdrawal is a familiar diagnosis in ambulatory and perioperative practice, it is easily overlooked in hospitalized patients on a complex medical service, where the differential for new headache is dominated by autoimmune, infectious, and vascular etiologies. We report a 33-year-old woman with overlap connective tissue disease (systemic lupus erythematosus, mixed connective tissue disea
