Protocol-Based Resuscitation for Septic Shock: A Meta-Analysis of Randomized Trials and Observational Studies.

PURPOSE: Owing to the recommendations of the Surviving Sepsis Campaign guidelines, protocol-based resuscitation or goal-directed therapy (GDT) is broadly advocated for the treatment of septic shock. However, the most recently published trials showed no survival benefit from protocol-based resuscitation in septic shock patients. Hence, we aimed to assess the effect of GDT on clinical outcomes in such patients. MATERIALS AND METHODS: We performed a systematic review that included a meta-analysis. We used electronic search engines including PubMed, Embase, and the Cochrane database to find studi

A retrospective observational cohort study evaluating the postoperative outcomes of intracapsular coblation tonsillectomy in children.

Coblation intracapsular tonsillectomy (ICT) is becoming popular due to its decreased postoperative complications. However, a concern exists about the need for revision surgery. We conducted a retrospective observational cohort study, with a null hypothesis that Coblation ICT is not associated with recurrence of the preoperative symptoms, obstructive tonsillar regrowth, or the need for revision tonsillar surgery. We reviewed 345 patients (median age of 4.5 years; IQR 3.2-6.3), operated by the senior author between Feb 2017 and Sep 2020, for a median follow-up of 395.0 days (IQR 221.5-654.5). Mo

Probiotic Human Study Design and Reporting (PRODARE): Recommendations to Improve Research Practice.

The PRObiotic human study Design And REporting recommendations provide a structured framework for designing and reporting human studies with probiotics, addressing challenges related to product viability. Five main study types are identified. Exploratory studies are early-phase, first-in-human investigations designed to evaluate effects or generate hypotheses and require specification of strain, dose, stability, and assessment of acceptance and adverse events. Pilot studies assess the feasibility of protocols and logistics before larger trials, with clearly defined objectives, compliance measu

The latest research trends in primary biliary cholangitis: a bibliometric analysis.

The bibliometric analysis uses the citation count of an article to measure its impact in the scientific community, but no study has been undertaken to determine the most influential papers in the field of primary biliary cholangitis (PBC). This study aimed to investigate the global research interest regarding PBC in dentistry using a bibliometric approach. We searched the Web of Science Core Collection database to find the top 100 most cited (T100) articles focusing on PBC. The information about each article including citations, authors, journals, countries, institutions, and keywords was reco

Prospective Multicenter Observational Study of Patients in Shock Treated with Vasopressin: VASOPRES Registry Study Protocol.

INTRODUCTION: In cases of arterial hypotension and/or hypoperfusion that do not respond to fluid therapy, the administration of intravenous catecholamines, especially norepinephrine, is usually the treatment of choice. However, prolonged exposure to high doses of norepinephrine increases mortality, as well as the possibility of arrhythmias, organ damage, and tissue ischemia. An early multimodal vasopressor strategy, which involves adding vasopressin (a drug with a mechanism of action different from catecholamines) to low-dose norepinephrine, could improve the safety profile. OBJECTIVES: The m

BACKGROUND: A link between patent foramen ovale (PFO) and migraine as well as the utility of closure of PFO and its effect on migraine have been subjects of debate. The present review is an effort to gather the available evidence on this topic and formulate recommendations. METHODS: A systematic search of electronic databases (Medline, Embase, Cochrane Library) was performed. A separate search in associated reference lists of identified studies was done. Observational studies and clinical trials published in English using the International Headache Society criteria for diagnosis of migraine w

Challenges and outcomes of breast reconstruction in advanced-stage breast cancer: a comprehensive review.

Post-mastectomy breast reconstruction plays a crucial role in the psychological well-being and long term quality of life of patients with breast cancer. However, its role in advanced stages (stages III-IV) remains debatable. To analyze the clinical outcomes and potential benefits of breast reconstruction in patients with advanced-stage breast cancer, a systematic review of studies on the reconstructive approach published between January 2018 and March 2025 was conducted. There is a marked paucity of studies focused on this topic, and most available data derive from small case series or observa

Association between Eubacterium saphenum and periodontitis: A systematic review.

OBJECTIVE: Although established periodontal pathobionts are frequently detected, their presence alone does not fully explain variations in disease severity, highlighting the need to identify additional microbial contributors. This systematic review evaluates the available evidence on the association between Eubacterium saphenum and the severity of periodontitis. METHODS: The review was conducted in accordance with the PRISMA 2020 framework and registered in PROSPERO (CRD420251273182). Observational studies involving systemically healthy adults were included that compared individuals with peri

Loneliness in spinal cord injury: A systematic review and comparison with the general population.

CONTEXT: The psychosocial consequences of a Spinal Cord Injury or Disorder (SCI/D) can include increased loneliness. However, differences in the measurement of loneliness confound the available evidence. OBJECTIVES: To compare levels of loneliness between adults with a SCI/D and the general population (Touch Test data) using the UCLA Loneliness Scale (any version). A secondary aim was to explore potential subgroups vulnerable to loneliness. METHODS: Observational studies examining loneliness in SCI/D were identified via CINAHL, PubMed, PsycINFO, MEDLINE, Embase, Web of Science (up until 9th

Potential relationship between cytomegalovirus and immunosenescence: Evidence from observational studies.

Immunosenescence (IS) occurs as a natural outcome of ageing and may be described as a decline in immune system flexibility and adaptability to sufficiently respond to new, foreign antigens. Potential factors that may precipitate IS include persistent herpesvirus infections, such as cytomegalovirus (CMV). Here, we conducted a review of the literature evaluating the potential association between CMV and IS. Twenty-seven epidemiologic studies that included direct comparisons between CMV-seropositive and CMV-seronegative immunocompetent individuals were analysed. The majority of these studies (n =

Guillain-Barré syndrome and influenza vaccines: A meta-analysis.

Cases of Guillain-Barré syndrome (GBS) have been occasionally associated with influenza vaccines; this possible risk, even if rare, is a matter of much concern. To investigate the strength of this association, a systematic review and a meta-analysis have been conducted; for the purpose, controlled observational studies addressing the risk of GBS associated with different influenza vaccines were sought. We finally selected 39 studies of interest published between 1981 and 2014 (seasonal influenza vaccines, 22; pandemic influenza vaccines, 16; both vaccines simultaneously administered, 1); funne

Exploring the Role of Psychological Stress in TAKOTSUBO CARDIOMYOPATHY: The 'Broken Heart' Syndrome.

Takotsubo syndrome (TTS), or 'broken heart' syndrome, is an acute, reversible cardiomyopathy classically linked to emotional stress. Although physiological mechanisms have been extensively described, the psychological stress profile of patients with TTS remains incompletely synthesised. This study aimed to systematically review and meta-analyse observational studies examining psychological stress-related factors in patients with TTS compared with cardiac and non-cardiac control groups. We searched PubMed/MEDLINE, Embase, PsycINFO, Scopus, and Web of Science from 1 January 2011 to 31 March 2024

Systemic Glucocorticoid Use and Risk of Site-Specific Cancers: A Methodologic Systematic Review of Observational Studies.

Systemic glucocorticoids, widely prescribed for inflammatory and autoimmune diseases, have long been suspected of increasing cancer risk through immunosuppressive and metabolic effects. Randomized trials are often unethical and difficult to conduct, leaving observational studies the best option to determine whether the use of systemic glucocorticoids affects cancer development. However, these studies face significant methodologic issues that may compromise the validity of their findings. We systematically reviewed observational studies published up to May 1, 2025 (PubMed, Embase, Web of Scienc

Constipation is associated with an increased risk of depression: A systematic review and meta-analysis of observational studies.

BACKGROUND: Constipation is a prevalent functional gastrointestinal disorder that impairs quality of life and is frequently accompanied by psychological distress. Emerging evidence suggests a potential association between constipation and depression, possibly mediated by gut-brain axis dysfunction; however, epidemiological findings remain inconsistent. METHODS: We conducted a systematic review and meta-analysis of observational studies examining the association between constipation and depression in the general population. PubMed, Web of Science, Embase, and the Cochrane Library were searched

Prevalence of unipolar mania in bipolar I disorder: a systematic review and meta-analysis of observational studies.

AIMS: Unipolar mania (UM), defined by the occurrence of manic episodes without a history of depression, is a topic of debate within the classification of affective disorders. However, its epidemiological burden remains unclear. This systematic review and meta-analysis aimed to estimate the prevalence of UM among individuals with bipolar type I disorder (BD-I) while exploring potential sources of heterogeneity. METHODS: The study protocol was registered in Open Science Framework on 27 March 2025. Embase, MEDLINE and APA PsycInfo were searched. We included observational studies reporting data o
