Acute anterior shoulder dislocations, when managed non-operatively, have traditionally been treated by placing the arm in a sling. There is no formal evidence that this treatment is of benefit. Three recently reported studies, one in cadavers and two in patients, suggest that the detachment of the structures in the front of the shoulder is made worse when the shoulder is placed in internal rotation, as when the arm is in a sling. By contrast, the structures are realigned when the arm is placed in external rotation. Shoulder dislocations, if managed non-operatively, should not be treated by pla

Infants are born with flexible flatfeet, and the normal arch develops in the first decade of life. Flexible flatfeet rarely cause disability, and asymptomatic children should not be burdened with orthotics or corrective shoes. Flexible flatfeet with tight heelcords may become symptomatic and can be addressed with a stretching program. Surgical intervention for flexible flatfeet is reserved for patients who have persistent localized symptoms despite conservative care. Rigid or pathologic flatfeet have multiple etiologies and many will require treatment to alleviate symptoms or improve function.

The purpose of this article is to attend to the nonsurgical treatment of adult symptomatic flexible flatfoot and its sequelae. Other articles in this issue thoroughly discuss the etiology, radiographic changes, and nonconservative treatment of the congenital and acquired flatfoot presented in children and adults.

The growth of assistive technology has increased the use of smart wearable orthoses. This is driven by improved sensors, actuators, cheaper integrated circuits, better data connectivity for data acquisition, and advanced manufacturing techniques. These smart devices correct, support, and monitor physical deformities and movements. This study reviews recent literature on smart wearable orthotic devices' design, application, and fabrication. The focus is on biomedical uses and advanced manufacturing methods. A total of 79 peer-reviewed articles were identified. After screening based on titles, a…

Background and Objectives: In-toeing and out-toeing gait are rotational deformities commonly observed in children with neuromuscular conditions. These gait abnormalities often result from internal tibial torsion, increased femoral anteversion, and metatarsus adductus. This study was conducted to create a comprehensive evaluation of the effectiveness of lower extremity orthotics as a non-operative treatment option, given their regular use in clinical settings. The aim of this literature review was to understand the efficacy of various orthotic devices in correcting rotational deformities in the…

BACKGROUND: Determining adherence with orthoses is important for clinicians prescribing devices. Measuring orthotic use often relies on patient recall which has poor agreement with objective measures. Measuring step count whilst wearing an orthosis could help objectively quantifying adherence. The Odstock Drop Foot Stimulator (ODFS) Pace, used in foot drop, has an integral activity logger which provides data on step count. The PALite, an accelerometer, measures step count and can be fixed to an ankle foot orthoses (AFO). Both have the potential to provide objective measures of adherence; howev

This case study reports a 5-year follow-up of a 32-year-old male service member who suffered polytrauma in 2007 following a Humvee rollover in Afghanistan. The service member's injured left lower extremity was salvaged, but severe damage to the lumbosacral plexus and significant injuries to the pelvis, hip, and femur resulted in near total paralysis and foot drop of the left lower limb. Two years of multiple substandard ankle-foot orthotic devices pushed him to investigate a dynamic ankle-foot orthotic (DAFO) with energy storing capability, which allowed him to remain on active duty and deploy…

The majority of children with traumatic injuries of the brachial plexus recover with only minor sequelae. Some patients develop residual deformity, however. This deformity may consist only of internal rotation contractures, but in some patients subluxation of the humeral head may be present. CT is useful in the evaluation of: the presence or absence of subluxation, deformity of the glenoid and the proper placement of the humeral head in plaster or orthotic devices.

Assisting carpal instability patients with regaining pain free and symptom free movement is a primary goal of therapy. This author describes her version of an orthotic device for carpal instability non-dissociative. She also describe her typical treatment plan for these patients.

Osteogenesis imperfecta (OI) is a disease caused by a genetic defect in the qualitative and quantitative synthesis of type I collagen. There is a wide variation in its clinical signs, characterized by bone fragility, resulting in a bone vulnerable to external and internal forces, determining the occurrence of frequent fractures with minimal or no trauma. The therapeutic objective is directed to improve the functional capacity of the child or adult concerned, adopting those compensatory strategies to optimise their independence. In this sense, the use of different orthoses and assistive technol

INTRODUCTION: The aim of this work was to proceed to a literature review to determine the physiological justifications underlying knee orthosis use in medical practice.
METHOD: An analysis of the international literature relating to the years 1980-2000 was carried out with the Medline data bank. We added extra articles focusing on the validity of the protocols used in the different evaluations.
RESULTS: Five hundred and twelve articles were selected and 46 articles of experimental validation were retained. The experimental protocols are divided into in vitro studies and clinical studies carr

Excessive pronation of the feet is almost a universal condition among children, which is demonstrated as an abnormal weight-bearing position on hard, flat surfaces. Excessive pronation of the feet of children is closely associated with intrinsic and extrinsic skeletal deformity in adults; consequently, it must be neutralized as a basic necessity to most proper footcare, as the child grows. It must be a priority of podiatric medicine to develop a practical plan for supervision of foot health from the earliest weight bearing in infancy and to work in close cooperation with parents and pediatrici

Oral wounds and burns may result in microstomia with significant management problems. An important early management decision involves the selection of an appliance capable of applying forces that will halt and reverse the condition. A selection appropriate to the patient may be based on the need for teeth suitably placed to retain and position a particular device. Limited access to the patient's mouth because of recent trauma, surgery, wiring, inability to tolerate anesthetic, etc., may further restrict choices. Thus, the process of selection involves a weighing of the specific patient's condi

Although the total contact cast is the criterion standard for the pressure redistribution of challenging foot ulcers, in practice it is not often feasible. The following is a case series that demonstrates an encouraging possibility for the Charcot restraint orthotic walker as a device to be used in more challenging forefoot ulcers.

OBJECTIVE: Evaluate from the literature, the evidence of comparative efficiency of non-surgical treatments (orthotics or head repositioning therapy) in posterior positional plagiocephaly.
MATERIAL AND METHODS: Systematic review from scientific articles (original cohort studies and review of literature), published in French or in English, searched on five online literature data bases, comparing non-chirurgical treatments (repositioning and orthotics therapy) for deformational plagiocephaly. A standardized method guidelines (Critical Review Form-Quantitative Studies) has been used.
